Detaillierte Beschreibung
Das primäre Ziel der Studie ist der Vergleich der Wirksamkeit von Dato-DXd und Pembrolizumab mit Pembrolizumab allein im Hinblick auf das progressionsfreie Überleben (PFS) oder das Gesamtüberleben (OS) für Teilnehmer mit fortgeschrittenem oder metastasiertem NSCLC ohne umsetzbare genomische Veränderungen, deren Tumor aufweist hohe Expression des programmierten Todesliganden 1 (PD-L1) (TPS ≥50 %) und die zuvor noch keine systemische Therapie gegen fortgeschrittenes oder metastasiertes NSCLC erhalten haben.
Geeignete Teilnehmer werden im Verhältnis 1:1 randomisiert dem Kontrollarm (Pembrolizumab allein) oder dem Versuchsarm (Dato-DXd und Pembrolizumab) zugeteilt. Die Studie wird in 4 Zeiträume unterteilt: Gewebe-Screening-Zeitraum, Screening-Zeitraum, Behandlungszeitraum und Nachbeobachtungszeitraum.

Beschreibung
Einschlusskriterien:
Teilnehmer, die für die Aufnahme in die Studie in Frage kommen, müssen alle Aufnahmekriterien innerhalb von 28 Tagen nach Randomisierung in die Studie erfüllen.
- Unterzeichnen und datieren Sie das Gewebe-Screening und die wichtigsten Einverständniserklärungen vor Beginn studienspezifischer Qualifizierungsverfahren.
- Erwachsene ≥ 18 Jahre oder das gesetzliche Mindestalter für Erwachsene (je nachdem, welches höher ist) zum Zeitpunkt der Einverständniserklärung.
Histologisch dokumentiertes NSCLC, das alle folgenden Kriterien erfüllt:
- Erkrankung im Stadium IIIB oder IIIC und zum Zeitpunkt der Randomisierung keine Kandidaten für eine chirurgische Resektion oder definitive Radiochemotherapie oder NSCLC-Erkrankung im Stadium IV (basierend auf dem American Joint Committee on Cancer, Eighth Edition).
- Dokumentierte negative Testergebnisse für epidermalen Wachstumsfaktorrezeptor (EGFR), Lymphomkinase (ALK) und Proto-Onkogen1 (ROS1) umsetzbare genomische Veränderungen basierend auf einer Analyse von Tumorgewebe.
- Keine bekannten umsetzbaren genomischen Veränderungen bei neurotropher Tyrosinrezeptorkinase (NTRK), Proto-Onkogen B-raf (BRAF), Rearranged during Transfection (RET), mesenchymal-epithelial Transition Factor (MET) oder anderen umsetzbaren Treiberkinasen mit lokal zugelassenen Therapien.
- Hat eine formalinfixierte Tumorgewebeprobe zur Messung der Proteinexpression des Trophoblasten-Zelloberflächenproteins 2 (TROP2) und zur Bewertung anderer explorativer Biomarker bereitgestellt.
- Der Tumor weist eine hohe Expression des programmierten Todesrezeptors 1 (PD-L1) (TPS ≥50 %) auf, wie durch den PD-L1-Immunhistochemie (IHC) 22C3-PharmDx-Assay durch zentrale Tests (mindestens 6 Objektträger) bestimmt wurde.
- Hat vor Tag 1 von Zyklus 1 eine angemessene Auswaschphase der Behandlung.
- Messbare Krankheit basierend auf lokaler bildgebender Beurteilung unter Verwendung von RECIST Version 1.1.
- Hat innerhalb von 28 Tagen vor der Randomisierung eine linksventrikuläre Ejektionsfraktion (LVEF) von ≥ 50%, entweder durch ein Echokardiogramm (ECHO) oder einen Multigated Acquisition Scan (MUGA).
- Leistungsstatus (PS) der Eastern Cooperative Oncology Group (ECOG) von 0 oder 1 beim Screening.
- Hat eine Lebenserwartung von mindestens 3 Monaten.
- Angemessene Knochenmarkfunktion innerhalb von 7 Tagen vor der Randomisierung.
Ausschlusskriterien:
- Hat eine vorherige systemische Behandlung für fortgeschrittenes oder metastasiertes NSCLC erhalten.
Hat eine vorherige Behandlung mit einem der folgenden erhalten, einschließlich im adjuvanten/neoadjuvanten Setting:
- Jedes Mittel, einschließlich eines Antikörper-Wirkstoff-Konjugats, das ein Chemotherapeutikum enthält, das auf Topoisomerase I abzielt.
- TROP2-gerichtete Therapie.
- Jedes Mittel gegen den programmierten Todesrezeptor-1 (PD-1), Anti-PD-L1 oder Anti-PD-Ligand 2 (L2) oder mit einem Mittel, das auf einen anderen stimulierenden oder co-inhibitorischen T-Zell-Rezeptor gerichtet ist (z. CTLA-4, OX40, CD137).
- Alle anderen Immuncheckpoint-Inhibitoren. Teilnehmer, die eine andere adjuvante oder neoadjuvante Therapie als die oben aufgeführten erhalten haben, sind teilnahmeberechtigt, wenn die adjuvante/neoadjuvante Therapie mindestens 6 Monate vor der Diagnose einer fortgeschrittenen/metastasierten Erkrankung abgeschlossen wurde.
- Hat Rückenmarkskompression oder aktive und unbehandelte Metastasen des Zentralnervensystems (ZNS) und/oder karzinomatöse Meningitis. Teilnehmer mit vorbehandelten Hirnmetastasen können teilnehmen, sofern sie radiologisch stabil sind.
- Hat eine vorherige Strahlentherapie ≤ 4 Wochen vor Beginn der Studienintervention oder mehr als 30 Gy (Einheit der ionisierenden Strahlendosis im Internationalen Einheitensystem) an der Lunge innerhalb von 6 Monaten nach Zyklus 1 Tag 1 erhalten.
Anamnese einer anderen primären Malignität (jenseits von NSCLC) mit Ausnahme von:
- Malignität, die mit kurativer Absicht und ohne bekannte aktive Erkrankung ≥ 3 Jahre vor der ersten Dosis des Studienmedikaments behandelt wurde und ein geringes potenzielles Rezidivrisiko aufweist.
- Angemessen behandelter nicht-melanozytärer Hautkrebs oder Lentigo maligna ohne Anzeichen einer Erkrankung.
- Angemessen behandeltes Karzinom in situ ohne Anzeichen einer Erkrankung.
- Teilnehmer mit einer Vorgeschichte von Prostatakrebs (Tumor-/Knoten-/Metastasen-Stadium) im Stadium ≤T2cN0M0 ohne biochemisches Wiederauftreten oder Fortschreiten, und die nach Ansicht des Ermittlers als nicht erforderlich erachtet werden aktive Intervention.
- Hat eine Vorgeschichte von (nicht infektiöser) interstitieller Lungenerkrankung (ILD) / Pneumonitis, die Steroide erforderte, hat eine aktuelle ILD / Pneumonitis oder wenn der Verdacht auf ILD / Pneumonitis nicht durch Bildgebung beim Screening ausgeschlossen werden kann.
- Klinisch schwere Lungenschädigung infolge interkurrenter Lungenerkrankungen, einschließlich, aber nicht beschränkt auf jede zugrunde liegende Lungenerkrankung oder jede Autoimmun-, Bindegewebs- oder entzündliche Erkrankung mit Lungenbeteiligung oder vorheriger vollständiger Pneumektomie.
Unkontrollierte oder signifikante Herz-Kreislauf-Erkrankungen, einschließlich:
- Mittleres QT-Intervall, korrigiert für die Herzfrequenz unter Verwendung des Intervalls der Fridericia-Formel (QTcF) > 470 ms, unabhängig vom Geschlecht (basierend auf dem Durchschnitt der 12-Kanal-Elektrokardiogramm-Bestimmung beim Screening).
- Myokardinfarkt innerhalb von 6 Monaten vor Randomisierung.
- Unkontrollierte Angina pectoris innerhalb von 6 Monaten vor Randomisierung.
- LVEF
- Kongestive Herzinsuffizienz (CHF) der New York Heart Association Klasse 2 bis 4 beim Screening.
- Unkontrollierte Hypertonie (systolischer Ruheblutdruck > 180 mmHg oder diastolischer Blutdruck > 110 mmHg) innerhalb von 28 Tagen vor Randomisierung.
Teilnehmer mit einer Vorgeschichte von CHF der Klasse 2 bis 4 vor dem Screening müssen zu CHF der Klasse 1 zurückgekehrt sein und eine LVEF ≥50 % haben (entweder durch einen ECHO- oder MUGA-Scan innerhalb von 28 Tagen vor der Randomisierung), um förderfähig zu sein.
- Klinisch signifikante Hornhauterkrankung.
- Hat innerhalb von 30 Tagen vor der ersten Dosis des Studienmedikaments einen Lebendimpfstoff oder einen attenuierten Lebendimpfstoff erhalten. Für alle Teilnehmer, die einen zugelassenen Impfstoff gegen das Coronavirus 2 (SARS-CoV2) des schweren akuten respiratorischen Syndroms erhalten, befolgen Sie bitte das Impfstoffetikett und/oder die örtlichen Richtlinien.
- Aktive, bekannte oder vermutete Autoimmunerkrankung (hat eine aktive Autoimmunerkrankung, die in den letzten 2 Jahren eine systemische Behandlung erfordert hat).
- Hat eine Diagnose von Immunschwäche oder erhält eine chronische systemische Steroidtherapie (in einer Dosierung von > 10 mg täglich Prednisonäquivalent) oder eine andere Form der immunsuppressiven Therapie ≤ 7 Tage vor der ersten Dosis des Studienmedikaments.
- Hat eine bekannte Infektion mit dem humanen Immundefizienzvirus (HIV), die nicht gut kontrolliert wird.
- Hat eine aktive Hepatitis oder eine unkontrollierte Hepatitis B- oder aktive Hepatitis C-Infektion; positiv für das Hepatitis-B- oder -C-Virus ist, basierend auf der Auswertung der Ergebnisse von Tests auf Hepatitis B (Hepatitis-B-Oberflächenantigen, Anti-HBs, Anti-HBc oder Hepatitis-B-Virus [HBV]-DNA) oder Hepatitis-C-Virus (HCV-RNA) Infektion.
- Hat eine unkontrollierte Infektion, die IV-Antibiotika, Virostatika oder Antimykotika erfordert.
- Hatte eine Transplantation von allogenem Gewebe/festem Organ.
- Hat eine Vorgeschichte von schweren Überempfindlichkeitsreaktionen entweder auf die Arzneimittelsubstanzen oder inaktiven Bestandteile (einschließlich, aber nicht beschränkt auf Polysorbat 80) von Dato-DXd oder Pembrolizumab.

Experimental: Pembrolizumab + Datopotamab Deruxtecan (Dato-DXd)
Die Teilnehmer werden randomisiert und erhalten 200 mg Pembrolizumab, gefolgt von 6,0 mg/kg Dato-DXd.
