Eine 32-wöchige Studie zur Bewertung der Wirksamkeit und Sicherheit von Tralokinumab bei Patienten mit mittelschwerem bis schwerem atopischem Handekzem, die für eine systemische Therapie in Frage kommen (ADHAND)
Eine interventionelle, adaptive, klinische Phase-3b-Studie zur Bewertung der Wirksamkeit und Sicherheit der Monotherapie mit Tralokinumab 300 mg alle zwei Wochen im Vergleich zu Placebo bei Patienten mit mittelschwerem bis schwerem atopischem Handekzem, die für eine systemische Therapie in Frage kommen
Der Zweck dieser Studie besteht darin, zu testen, ob die Behandlung mit Tralokinumab sicher und wirksam ist Behandlung von mittelschwerem bis schwerem atopischem Handekzem. Dies wird anhand einer Reihe von Untersuchungen beurteilt, die den Schweregrad und das Ausmaß des atopischen Handekzems und seiner Symptome sowie den allgemeinen Gesundheitszustand und die Lebensqualität bewerten.
Die Testphase dauert bis zu 40 Wochen. Es finden bis zu 15 Besuche statt, davon 3 telefonisch. Der erste Teil der Studie wird als Screening-Zeitraum bezeichnet und dauert bis zu 4 Wochen. In den ersten 16 Wochen nach dem Screening erhalten die Studienteilnehmer alle zwei Wochen entweder Tralokinumab oder Scheininjektionen. Nach den ersten 16 Wochen erhalten alle Studienteilnehmer 16 Wochen lang alle zwei Wochen Tralokinumab-Injektionen. Der letzte Teil der Studie ist ein Zeitraum von 4 Wochen nach Ende des Behandlungszeitraums, in dem die Studienteilnehmer aus Sicherheitsgründen das Medikament absetzen.

Einschlusskriterien:
- Alter 18 Jahre oder älter beim Screening
- Diagnose der atopischen Dermatitis (AD) gemäß den Kriterien von Hanifin und Rajka (1980) für AD
- AD-Anamnese seit ≥1 Jahr
- Vorliegen eines atopischen Handekzems (AHE), das länger als 3 Monate anhält oder innerhalb der letzten 12 Monate zweimal oder öfter wieder aufgetreten ist, unter Vermeidung aller bekannten und relevanten Reizstoffe und Allergene
- AD-Beteiligung mindestens einer anderen Körperstelle als den Händen und Handgelenken beim Screening
- Ein Investigator's Global Assessment AHE-Score von 3 oder 4 (mittel bis schwer) beim Screening und bei Studienbeginn
- Ein HESD-Juckreiz-Score (wöchentlicher Durchschnitt) von ≥4 zu Studienbeginn
- Probanden, bei denen in jüngster Zeit (innerhalb von 12 Monaten vor dem Screening-Besuch) dokumentiert wurde, dass sie nicht ausreichend auf die Behandlung von AHE mit topischen verschreibungspflichtigen Medikamenten angesprochen haben oder für die topische Behandlungen aus anderen medizinischen Gründen nicht ratsam sind (z. B. aufgrund schwerwiegender Nebenwirkungen oder Sicherheitsrisiken)
Ausschlusskriterien:
Probanden dürfen nicht an der Studie teilnehmen, wenn sie andere aktive Subtypen eines Handekzems als AHE haben, die als Hauptursache des aktuellen Handekzems gelten, darunter:
- Aktive irritative Kontaktdermatitis
- Aktive allergische Kontaktdermatitis
- Aktive Proteinkontaktdermatitis/Kontakturtikaria
- Aktives hyperkeratotisches Handekzem
- Aktives vesikuläres Handekzem (Pompholyx)
- Aktive dermatologische Erkrankungen, die die Diagnose von AD oder AHE verfälschen oder die Beurteilung der Behandlung beeinträchtigen könnten
- Verwendung von Solarien oder Phototherapie (z. B. UVB, UVA1, PUVA) oder Grenzstrahlentherapie an Händen oder Handgelenken innerhalb von 28 Tagen vor Studienbeginn oder Verwendung von Bleichbädern an Händen oder Handgelenken innerhalb von 7 Tagen vor Studienbeginn
- Behandlung mit systemischen immunsuppressiven/immunmodulierenden Arzneimitteln und/oder systemischen Kortikosteroiden innerhalb von 28 Tagen vor Studienbeginn
- Behandlung mit topischen Kortikosteroiden, topischen Calcineurin-Inhibitoren, topischen Phosphodiesterase-4-Inhibitoren oder topischen Januskinase-Inhibitoren innerhalb von 7 Tagen vor Studienbeginn
- Erhalt einer vermarkteten biologischen Therapie (d. h. Immunglobulin, Anti-Immunglobulin E), einschließlich Dupilumab oder biologischer Prüfpräparate, 3 bis 6 Monate vor Studienbeginn

Change in WPAI+CIQ:AHE Domain Scores
Zeitfenster: From baseline to Week 32

The impact of AHE on the subject's ability to work/school and perform regular activities was assessed by Work Productivity and Activity Impairment plus Classroom Impairment Questionnaire in atopic hand eczema (WPAI+CIQ:AHE), which is an instrument to measure impairments in both paid work and unpaid work/school attendance.
Only domain scores where an estimand analysis was possible are presented.
Scores are on a scale from 0 to 100 and reflect the percentage impairment due to AHE during the past 7 days, with higher numbers indicating greater impairment and less productivity/school attendance.
