Neugeborene Thymustransplantation beim Menschen (SUPERTHYM)
Eine supermikrochirurgische Technik zum ersten Mal beim Menschen für die neonatale Thymustransplantation
Das Ziel dieser klinischen Studie ist es, die Sicherheit der neonatalen Thymustransplantation bei einem erwachsenen Patienten nachzuweisen. Die wichtigsten Fragen, die es beantworten soll, sind:
- Ist die neonatale Thymustransplantation ein sicheres Verfahren?
- Wie ist das Nebenwirkungsprofil der neonatalen Thymustransplantation?
Dies ist eine Ein-Fach-Studie; daher wird es keine Vergleichsgruppen geben. Der Teilnehmer wird vor, während und nach dem Eingriff multidisziplinär betreut.

Detaillierte Beschreibung
Eine unzureichende Thymopoese steht wahrscheinlich im Zusammenhang mit Autoimmunität, Immunschwäche und Immunseneszenz. Die Verbesserung der Thymopoese kann attraktiv sein, um solche immunologischen Phänomene zu handhaben oder zu untersuchen. Die Thymopoese wird durch die adulte Thymustransplantation unterstützt, wie in verschiedenen Tiermodellen gezeigt wurde. Es wurde über mehrere Ansätze zur Verbesserung der Thymopoese beim Menschen berichtet, die unter anderem die In-vitro- und In-vivo-Regeneration des Thymus und die avaskuläre Transplantation von allogen verarbeitetem Thymusgewebe umfassen.
Obwohl kürzlich eine supermikrochirurgische Technik für die Thymustransplantation bei Neugeborenen bei Kaninchen beschrieben wurde, wurde sie noch nie beim Menschen beschrieben. Diese Technik kann verwendet werden, um eine neonatale Thymustransplantation beim Menschen durchzuführen. Wenn sich diese Technik als sicher erweist, werden zukünftige Studien folgen, um ihre Wirkung auf die Thymopoese und ihre Wirksamkeit bei verschiedenen Gesundheitszuständen zu untersuchen.
Diese erstmals am Menschen durchgeführte klinische Ein-Personen-Studie zielt darauf ab, die Sicherheit zu demonstrieren und das Nebenwirkungsprofil der neonatalen Thymustransplantation bei einem Erwachsenen mit akuter myeloischer Leukämie zu untersuchen, der weder für eine Knochenmarktransplantation noch für eine intensive Chemotherapie in Frage kommt. Dieses Humanmodell ermöglicht nicht nur die Untersuchung der Sicherheit der neonatalen Thymustransplantation, sondern auch die Untersuchung der Wirkung des Verfahrens auf die Thymopoese.

Einschlusskriterien:
- Diagnose einer akuten Leukämie in der Palliativmedizin, entweder myeloid (AML) oder lymphozytär (ALL).
- Kein Kandidat für eine Knochenmarktransplantation.
- Kein Kandidat für eine intensive Chemotherapie.
Ausschlusskriterien:
- Vorgeschichte der Knochenmarktransplantation.
- Vorgeschichte von schwerem kognitiven Verfall oder Demenz.
- Vorgeschichte von schwerem Leberversagen.
- Vorgeschichte einer chronischen Nierenerkrankung (CKD) Stadium 4-5.
- Frühere Anamnese eines akuten Koronarsyndroms innerhalb der letzten 90 Tage.
- Frühere Anamnese eines akuten ischämischen Schlaganfalls innerhalb der letzten 90 Tage.
- Frühere Anamnese von dekompensierter kongestiver Herzinsuffizienz (CHF) innerhalb der letzten 90 Tage.

Experimental: Thymustransplantation bei Neugeborenen
Supermikrochirurgische Technik für die Thymustransplantation bei Neugeborenen in den radialen Unterarm des Patienten.
Der Spender wird ein Neugeborenes sein, das einer korrektiven Herzoperation mittels Sternotomie unterzogen wird, die eine routinemäßige partielle Thymektomie erfordert.

Die neonatalen Thymusgefäße des Spenders werden unmittelbar nach der routinemäßigen partiellen Thymektomie präpariert.
Diese Gefäße werden unter Verwendung einer supermikrochirurgischen Technik mit den durchdringenden Gefäßen des Empfängers innerhalb des radialen Unterarms anastomosiert.
Das Transplantat wird am Tag 14-21 nach dem Eingriff explantiert.
