Una prima tecnica supermicrochirurgica nell'uomo per il trapianto di timo neonatale
L'obiettivo di questo studio clinico è dimostrare la sicurezza del trapianto di timo neonatale in un paziente adulto. Le principali domande a cui intende rispondere sono:
- Il trapianto di timo neonatale è una procedura sicura?
- Qual è il profilo degli eventi avversi del trapianto di timo neonatale?
Questo è uno studio a soggetto singolo; quindi, non ci saranno gruppi di confronto. Il partecipante riceverà cure di supporto multidisciplinari prima, durante e dopo la procedura.

Una timopoiesi inadeguata è probabilmente correlata ad autoimmunità, immunodeficienza e immunosenescenza. L'aumento della timopoiesi può essere interessante per la gestione o lo studio di tali fenomeni immunologici. La timopoiesi è supportata dal trapianto di timo adulto, come dimostrato in diversi modelli animali. Sono stati segnalati diversi approcci per migliorare la timopoiesi nell'uomo, che includono la rigenerazione in vitro e in vivo del timo e l'innesto avascolare di tessuto timico trattato allogenico, tra gli altri.
Sebbene una tecnica supermicrochirurgica per il trapianto di timo neonatale sia stata recentemente descritta nei conigli, non è mai stata descritta nell'uomo. Questa tecnica può essere utilizzata per eseguire un trapianto di timo neonatale nell'uomo. Se questa tecnica si dimostrerà sicura, seguiranno studi futuri per indagare il suo effetto sulla timopoiesi e la sua efficacia su diverse condizioni di salute.
Questo studio clinico su soggetto singolo, primo nell'uomo, mira a dimostrare la sicurezza e a studiare il profilo degli effetti avversi del trapianto di timo neonatale in un adulto con leucemia mieloide acuta che non è candidato al trapianto di midollo osseo, né alla chemioterapia intensiva. Questo modello umano non solo permette di studiare la sicurezza del trapianto di timo neonatale, ma permetterà anche di studiare l'effetto della procedura sulla timopoiesi.

Criterio di inclusione:
- Diagnosi di leucemia acuta in cure palliative, mieloide (AML) o linfocitaria (ALL).
- Non candidato al trapianto di midollo osseo.
- Non candidato a chemioterapia intensiva.
Criteri di esclusione:
- Storia medica passata di trapianto di midollo osseo.
- Storia medica passata di grave declino cognitivo o demenza.
- Storia medica passata di grave insufficienza epatica.
- Storia medica passata di malattia renale cronica (CKD) fase 4-5.
- Storia medica passata di sindrome coronarica acuta negli ultimi 90 giorni.
- Storia medica passata di ictus ischemico acuto negli ultimi 90 giorni.
- Storia medica passata di insufficienza cardiaca congestizia (CHF) scompensata negli ultimi 90 giorni.

Sperimentale: Trapianto di timo neonatale
Tecnica supermicrochirurgica per il trapianto di timo neonatale nell'avambraccio radiale del soggetto.
Il donatore sarà un neonato soggetto a chirurgia cardiaca correttiva tramite sternotomia che richiede timectomia parziale di routine.

I vasi del timo neonatale del donatore saranno sezionati subito dopo la timectomia parziale di routine.
Questi vasi saranno anastomizzati nei vasi penetranti del ricevente all'interno dell'avambraccio radiale utilizzando una tecnica supermicrochirurgica.
L'innesto verrà espiantato al giorno 14-21 dopo la procedura.
