Excursión del nervio mediano durante la movilización neurodinámica
Efecto de los movimientos del cuello sobre la excursión longitudinal del nervio mediano durante la movilización neurodinámica en pacientes con síndrome del túnel carpiano

Descripción detallada
La excursión del nervio mediano se evaluará mediante ecografía en tres posiciones: MNT1 estándar, flexión del cuello con MNT1 y rotación ipsilateral combinada con MNT1. Ambos brazos serán evaluados.
El brazo se mantendrá en abducción a 90° - 110° y rotación externa a 90°, supinación del antebrazo, extensión del codo y extensión de la muñeca y los dedos utilizando una férula hecha a medida.
El MNT1 estándar incluye flexión del lado contralateral del cuello. Luego, el investigador moverá el cuello pasivamente en rotación y flexión. El límite de rango pasivo se determinará antes de la prueba usando un goniómetro y una sensación final pasiva. El orden de movimiento se aleatorizará utilizando una función aleatoria de Excel.
Cada posición será probada tres veces. Un solo investigador hará todos los movimientos de forma pasiva. La excursión del nervio se medirá en tiempo real por el mismo ecografista que está cegado a la agrupación de pacientes.

Descripción
Criterios de inclusión:
- Diagnóstico clínico de STC leve a moderado.
- Debe ser capaz de tolerar toda la posición MNT1 estándar.
Criterio de exclusión:
- STC severo.
- Síntomas referidos al cuello.
- Rangos limitados de flexión, rotación y flexión lateral del cuello (>10%).
- Antecedentes de traumatismo, disfunción o cirugía del cuadrante superior.
- Antecedentes de neuropatía periférica o radiculopatía cervical.
- Historia de enfermedad sistémica asociada con neuropatías como diabetes mellitus, enfermedades del tejido conectivo, enfermedad de la tiroides u obesidad.

Pacientes con STC que cumplan los siguientes criterios de elegibilidad: Criterios de inclusión

Los participantes estarán en decúbito supino. El brazo estará en la posición estándar MNT1. El hombro, el codo, la muñeca y los dedos se mantendrán en la posición requerida utilizando una férula adaptada durante todas las pruebas. Todos los movimientos se limitarán al rango en el que no se provoquen dolor u otros síntomas. La flexión y rotación pasivas del cuello se agregarán a MNT1 y se aplicarán como movimientos de tensión en el nivel 3. Todos los movimientos se repetirán tres veces y se calculará un promedio. Se utilizará el goniómetro Myrin para evaluar la flexión y rotación del cuello. El orden de los movimientos se aleatorizará utilizando la función de generación aleatoria de Excel. Los movimientos serán realizados por un solo terapeuta capacitado. La posición final se mantendrá hasta 10 segundos hasta que la ecografía registre la excursión del nervio mediano. Para la captura de ecografía, el transductor se colocará de forma perpendicular y se alineará longitudinalmente con el nervio mediano en la muñeca. El evaluador estará cegado a la agrupación de participantes.

Control sano asintomático de la misma edad sin síntomas ni antecedentes de enfermedad, disfunción, traumatismo o cirugía del cuadrante superior.
