Polvivarren tehokkuus nelipäisen reisilihaksen vahvistamisessa kuntoutuksessa eturistisiteen (ACL) rekonstruktiossa (NMES)
Polviharjan tehokkuus potilaan nelipäisen lihasten vahvistamisessa kuntoutuksessa etummaisen ristisiteen rekonstruoinnin jälkeen
Prospektiivinen, kontrolloitu, satunnaistettu, yksisokkotutkimus potilailla, jotka ovat kuntoutuksessa eturistisiteen rekonstruoinnin jälkeen. 96 potilasta jaettiin satunnaisesti yhteen kolmesta ryhmästä. Ryhmäryhmä KH (Kneehab n=33) harjoitteli uudentyyppisellä NMES:llä monitieaktivoinnilla 20 minuuttia, 3 kertaa päivässä, 5 päivää viikossa 12 viikon ajan; Ryhmän PS (Poli-Stim n = 29), jota harjoitettiin tavallisella neuromuskulaarisella sähköstimulaatiolla (NMES) 20 minuuttia päivässä, 5 päivää viikossa 12 viikon ajan; Ryhmä CO (kontrolli - n=34), joka ei käyttänyt lihasstimulaatiota, suoritti vapaaehtoisia isometrisiä nelipäisen lihassupistuksia. Potilaita, jotka saivat jompaakumpaa NMES-hoitoa, neuvottiin supistamaan isometrisesti nelipäistä lihasta vapaaehtoisesti jokaisen sähköisen lihasstimulaation yhteydessä. Kaikki kolme ryhmää sijoittivat tämän harjoitusaikataulun klinikalla käytetyn ACL-rekonstruktion jälkeisen kuntoutusprotokollan päälle.
Tutkimussuunnitelma vastasi kaksisuuntaista varianssianalyysiä, jossa oli kolme ryhmää ja 4 tutkimusaikaa (perustila, 6 viikkoa, 12 viikkoa ja 24 viikkoa ACL-rekonstruktion jälkeen). Merkitystasoksi asetettiin 5 %. Nelipäisen lihasharjoittelun avainindikaattoreita olivat loukkaantuneen jalan ojentajien isokineettinen voimankehitys nopeudella 90 astetta/sekunti ja 180 astetta/sekunti sekä yhden jalan hypyn suorittaneiden osallistujien saavuttama etäisyys ja aika sukkulan suorittamiseen juosta. Tavoitteena oli näyttää 10 % ero yhden jalan hypyn ja isokineettisen vahvuustestin suhteellisessa hyppymatkassa sekä 1,2 sekunnin ero sukkulan suoritukseen kuluvassa ajassa. Osallistujille asetettiin 3 viikon tavoite saavuttaa täysi painonkantokyky ilman kävelyapua. Ryhmien välillä suunniteltiin tavanomaisiin työtehtäviin paluuajan vertailu.

- Henkilöt, joilla on lisäkäsittely vähäisistä pienistä vioista
- Henkilöt, jotka halusivat ja pystyivät antamaan tietoisen suostumuksen
Poissulkemiskriteerit:
- Henkilöt, joilla on ollut monisiteiden rekonstruktio
- ACL-potilaat, joilla on mikromurtuma tai ylimääräinen taakka polvessa
- Henkilöt, jotka rekisteröivät kipua yli tason 3 IKDC:n subjektiivisen arvioinnin kipuasteikon kysymyksissä, 3 päivää leikkauksen jälkeen
- Vapaaehtoistutkimukseen osallistuneet edellisten yhdeksänkymmenen (90) päivän aikana
- Henkilöt, joilla on vakavia sydämen rytmihäiriöitä tai muita implantoituja laitteita
- Henkilöt, joilla on sydämentahdistin
- Henkilöt, joilla on neurologisia tai psykiatrisia häiriöitä
- Raskaana olevat tai imettävät henkilöt
- Hermo-lihasjärjestelmää häiritsevien lääkkeiden nauttiminen
- Henkilöt, joilla on epilepsia
