Efficacité de Kneehab dans le renforcement des quadriceps lors de la reconstruction post-ligament croisé antérieur (LCA) (NMES)
L'efficacité de Kneehab dans le renforcement des quadriceps des patients en rééducation après reconstruction du ligament croisé antérieur
Étude prospective, contrôlée, randomisée, en simple aveugle chez des patients en rééducation suite à une reconstruction du ligament croisé antérieur. 96 patients ont été répartis au hasard dans l'un des trois groupes. Groupe Groupe KH (Kneehab n=33) formé avec un nouveau type de NMES avec activation par trajets multiples pendant 20 minutes, 3 fois par jour, 5 jours par semaine pendant 12 semaines ; Groupe PS (Poli-Stim n = 29) formé avec la stimulation électrique neuromusculaire standard (NMES) pendant 20 minutes par jour, 5 jours par semaine pendant 12 semaines ; Le groupe CO (Contrôle - n = 34), qui n'a pas utilisé de stimulation musculaire, a effectué des contractions musculaires isométriques volontaires du quadriceps. Les patients recevant l'une ou l'autre forme de traitement NMES ont reçu pour instruction de contracter volontairement isométriquement le muscle quadriceps à chaque stimulation musculaire électrique. Les trois groupes ont superposé ce programme d'entraînement au protocole standard de rééducation post- reconstruction du LCA utilisé à la clinique.
La conception de l'étude correspondait à une analyse de variance à deux facteurs avec trois groupes et 4 temps d'examen (Base, 6 semaines, 12 semaines et 24 semaines après la reconstruction du LCA). Le seuil de signification a été fixé à 5 %. Les indicateurs clés de l'intervention d'entraînement des muscles quadriceps étaient le développement de la force isocinétique des extenseurs de la jambe blessée à 90 degrés/seconde et 180 degrés/seconde et la distance parcourue par les participants effectuant le saut à une jambe et le temps nécessaire pour effectuer la navette. courir. L'objectif était de montrer une différence de 10% dans la distance relative sautée pour le saut à une jambe et pour les tests de force isocinétique et une différence de 1,2 seconde dans le temps nécessaire pour effectuer la course navette. L'objectif de 3 semaines a été fixé pour que les participants atteignent une pleine capacité de mise en charge sans utiliser d'aide à la marche. Une comparaison entre les groupes du temps de retour aux activités habituelles de travail était prévue.

Critère d'intégration:
- Personnes ayant subi une reconstruction chirurgicale du ligament croisé antérieur
- Personnes bénéficiant d'un traitement supplémentaire pour les défauts miniscaux mineurs
- Les personnes qui étaient disposées et capables de fournir un consentement éclairé
Critère d'exclusion:
- Personnes ayant subi une reconstruction multi-ligamentaire
- Patients atteints du LCA avec une microfracture ou une charge supplémentaire sur le genou
- Personnes ayant enregistré une douleur supérieure au niveau 3 sur les questions de l'échelle d'évaluation subjective de la douleur de l'IKDC, 3 jours après la chirurgie
- Participants à une étude volontaire au cours des quatre-vingt-dix (90) jours précédents
- Personnes souffrant d'arythmies cardiaques graves ou d'autres dispositifs implantés
- Personnes portant un stimulateur cardiaque
- Personnes atteintes de troubles neurologiques ou psychiatriques
- Les personnes enceintes ou qui allaitent
- Prise de médicaments qui interfèrent avec le système neuromusculaire
- Les personnes épileptiques
- Toute condition ou histoire qui, de l'avis du chercheur principal, pourrait augmenter le risque pour un individu ou interférer avec l'évaluation des données
