De effectiviteit van Kneehab bij het versterken van de quadriceps van patiënten tijdens revalidatie na reconstructie van de voorste kruisband
Prospectieve, gecontroleerde, gerandomiseerde, enkelblinde studie bij patiënten die revalidatie ondergaan na reconstructie van de voorste kruisband. 96 patiënten werden willekeurig toegewezen aan een van de drie groepen. Groep Groep KH (Kneehab n=33) getraind met een nieuw type NMES met multipath activatie gedurende 20 minuten, 3 keer per dag, 5 dagen per week gedurende 12 weken; Groep PS (Poli-Stim n=29) getraind met standaard neuromusculaire elektrische stimulatie (NMES) gedurende 20 minuten per dag, 5 dagen per week gedurende 12 weken; Groep CO (controle - n=34), die geen gebruik maakte van spierstimulatie, voerde vrijwillige isometrische quadriceps-spiercontracties uit. Patiënten die een van beide vormen van NMES-behandeling kregen, kregen de instructie om de quadriceps-spier vrijwillig isometrisch samen te trekken bij elke elektrische spierstimulatie. Alle drie de groepen voegden dit trainingsschema toe aan het standaard revalidatieprotocol na VKB-reconstructie dat in de kliniek werd gebruikt.
De onderzoeksopzet kwam overeen met een tweerichtingsvariantieanalyse met drie groepen en 4 onderzoekstijden (basislijn, 6 weken, 12 weken en 24 weken na VKB-reconstructie). Het significantieniveau werd vastgesteld op 5%. De belangrijkste indicatoren van de quadriceps-spiertrainingsinterventie waren de isokinetische krachtontwikkeling van de extensoren van het geblesseerde been bij 90 graden/seconde en 180 graden/seconde en de afstand die de deelnemers bereikten bij het uitvoeren van de single-leg hop en de tijd om de shuttle te voltooien. loop. Het doel was om een verschil van 10% te laten zien in de relatieve gesprongen afstand voor de single-leg hop en voor de isokinetische krachttests en een verschil van 1,2 seconde in de tijd om de shuttle-run te voltooien. Het doel van 3 weken was voor de deelnemers om volledig belastbaar te zijn zonder gebruik te maken van een loophulpmiddel. Een vergelijking tussen de groepen van de tijd voor terugkeer naar normale werkactiviteiten was gepland.

Inclusiecriteria:
- Personen die een chirurgische reconstructie van de voorste kruisband hebben ondergaan
- Personen met aanvullende behandeling van kleine miniscale defecten
- Personen die bereid en in staat waren om geïnformeerde toestemming te geven
Uitsluitingscriteria:
- Personen die een multi-ligamentaire reconstructie hebben ondergaan
- VKB-patiënten met een microfractuur of extra belasting van de knie
- Personen die pijn registreerden boven niveau 3 op de IKDC subjectieve evaluatie pijnschaalvragen, 3 dagen na de operatie
- Deelnemers aan een vrijwilligersonderzoek in de afgelopen negentig (90) dagen
- Personen met ernstige hartritmestoornissen of andere geïmplanteerde apparaten
- Personen met pacemakers
- Personen met neurologische of psychiatrische stoornissen
- Personen die zwanger zijn of borstvoeding geven
- Inname van medicijnen die interfereren met het neuromusculaire systeem
- Personen met epilepsie
- Elke aandoening of geschiedenis die naar de mening van de hoofdonderzoeker het risico voor een persoon kan vergroten of de evaluatie van gegevens kan verstoren
