Yksityiskohtainen kuvaus
Lasten kaltoinkohtelu (CM) on vakava kansanterveysongelma Yhdysvalloissa, ja sen tiedetään vaarantavan lasten kehittyviä itsesääntelytaitoja ja lisäävän päihteiden käytön ja muiden sääntelyhäiriöiden riskiä. Vanhemman ja lapsen vuorovaikutusterapian (PCIT), intensiivisen, 20 istunnon vanhemmuuden interventio, on osoitettu parantavan CM-vanhemmuuden laatua, parantavan positiivista vanhemmuutta ja lasten käyttäytymistä sekä vähentävän CM-rikosten uusiutumista (Chaffin et al., 2004). vaikka sen vaikutusten taustalla olevia mekanismeja ymmärretään vähän. Opintojen tavoitteet ovat:
Tavoite 1: Testaa PCIT:n tärkeimmät vaikutukset CM-spesifisiin tuloksiin, mukaan lukien ankaran, vastenmielisen vanhemmuuden ja CM-rikollisuuden vähentäminen ja lasten käyttäytymisen sopeutumisen edistäminen. Oletetaan, että interventioperheet osoittavat huomattavasti enemmän käyttäytymisparannuksia (eli vähentynyt negatiivinen vanhemmuus, lisääntynyt positiivinen vanhemmuus ja vähentyneet lapsen sisäistämis-/ulkoistumisongelmat) testin jälkeen ja pienempi CM-rikosten uusiminen 6 kuukauden seurannassa verrattuna palveluja saaviin perheisiin. kuten tavallisesti (SAU).
Erityinen tavoite 2: Selvitä PCIT:n vaikutus CM-vanhempien itsesääntelyn indekseihin. Ensinnäkin tutkijat testaavat hypoteesia, jonka mukaan PCIT:llä on suoria vaikutuksia CM-vanhempien itsesääntelykyvyn parantamiseen vanhemmuuden yhteydessä. Toiseksi tutkijat tutkivat, kuinka vanhempien itsesääntelytoimenpiteet välittävät PCIT-interventiovaikutuksia CM:n vähenemiseen ja vanhemmuuden parantamiseen. Kolmanneksi tutkijat selvittävät, rajoittavatko vanhempien interventiota edeltävän itsesääntelyn tasot interventiovaikutuksia tuloksiin. CM-alatyypin (eli fyysinen pahoinpitely vs. laiminlyönti), vakavuus ja muut keskeiset sosiokontekstuaaliset tekijät huomioidaan myös kiinnostaviin tuloksiin.
Erityinen tavoite 3: Tutkia PCIT:n vaikutusta CM-lasten itsesäätelyn hermokäyttäytymisindekseihin. CM:llä on haitallisia vaikutuksia lasten kehittyviin kykyihin säädellä huomiota, tunteita, fysiologiaa ja käyttäytymistä. Puutteet näillä aloilla lisäävät riskiä psykopatologiaan, varhain ilmeneviin käyttäytymisongelmiin ja myöhemmin päihteiden väärinkäyttöön. Tutkijat olettavat, että vaikka PCIT on ensisijaisesti suunnattu vanhemmuuteen, se parantaa CM-lasten itsesäätelyn neurobiologisia indikaattoreita hoidon jälkeen verrattuna kontrolliryhmään. Seuraavaksi tutkijat testaavat, missä määrin lasten tulokset välittyvät interventiovaikutusten kautta vanhemmuuteen. Oletuksena on, että lasten sääntelyn puutteita ja käyttäytymisongelmia heikennetään PCIT-pohjaisilla aversiivisen vanhemmuuden vähentämisellä, joka johtuu interventiosta.

Kokeellinen: Vanhempien lasten vuorovaikutusterapia (PCIT)
Osallistujat saavat vanhempien lasten vuorovaikutusterapian (PCIT). PCIT on suunniteltu parantamaan lasten toimintaa katkaisemalla ankaran, pakottavan vuorovaikutuksen malleja ja parantamalla vanhempien lämmintä, positiivista vanhemmuutta, autonomian tukea ja asiantuntevia lasten hallintataitoja. . |
PCIT on 16-20 istunnon live-valmennus vanhemmuuteen.
