Transkraniaalisen fotobiomodulaation (tPBM) turvallisuus ja tehokkuus henkilöille, joilla on autismikirjon häiriö ja epileptinen toiminta
Tätä tutkimusta varten ehdotettu toimenpide toimitetaan noninvasiivisesti lähellä infrapunavaloa (NIR) - transkraniaalista fotobiomodulaatiota (tPBM) - autististen lasten aivoihin.
Tämä tapahtuu kahdesti viikossa 10 viikon ajan.
Jelikaliten kehittämä Cognilum, puettava laite, toimittaa NIR-valon tietyille aivoalueille.
Odotettu tulos on parempi keskittyminen, parempi katsekontakti, parantunut puhe, parantunut käyttäytyminen ja toiminnallisten taitojen paraneminen.
Cognilum voi vaikuttaa autismin hoidon kliiniseen käytäntöön.
Alussa, viiden viikon kuluttua ja tutkimuksen lopussa, kliinikko suorittaa CARS-2-, SRS-, CGI- ja hoitajahaastattelun; Lisäksi omaishoitajille lähetetään kyselylomakkeita yhden tunnin viikoittaisen hoitokerran aikana.

Sisällyttämiskriteerit:
- Autismispektrihäiriö (diagnosoitu autistiseksi häiriöksi ADOS-2:ssa tai ADI-R:ssä).
- 4-12 vuoden ikä, lähtötilanteessa.
- Autismin vakavuus on kohtalainen tai korkeampi (≥4) 7-kohdan kliinisen maailmanlaajuisen vaikutelma-vakavuusasteikon alla. Kohtalainen autismin vakavuusaste (4) määritellään ASD-diagnoosilla, johon liittyy kielivamma.
- Kyky pitää kaikki jatkuvat täydentävät, ruokavalioon perustuvat, perinteiset ja käyttäytymishoidot vakiona tutkimusjakson ajan.
- Muuttumattomat täydentävät, ruokavalio-, perinte- ja käyttäytymishoidot kahden kuukauden ajan ennen tutkimukseen tuloa
- Kyky sietää toimenpiteitä tutkijan harkinnan mukaan.
- Vähintään yksi 24 tunnin EEG, jonka tiedot ovat EDF-muodossa, jotka ovat tutkijoiden käytettävissä.
Poissulkemiskriteerit:
- Merkittävä itsekuria käyttävä tai väkivaltainen käytös tai todisteet itsemurha-ajatuksista, -suunnitelmasta tai -käyttäytymisestä
- Vaikeasti sairaat lapset CGI-vakavuusstandardin mukaan = 7 (erittäin sairas)
- Vaikea keskosuus (<34 raskausviikkoa) sairaushistorian perusteella
- Nykyinen hallitsematon gastroesofageaalinen refluksitauti, koska GERD voi aiheuttaa liikkeitä, jotka näyttävät kohtauksilta
- Geneettiset oireyhtymät
- Synnynnäiset aivojen epämuodostumat
- Mikä tahansa PI:n määrittelemä lääketieteellinen tila voi vaarantaa tutkittavan turvallisuuden tai vaarantaa tietojen eheyden.
- Epäonnistuminen tai kehon massaindeksi < 5 %ile tai < 5 %ile painosta (uros <11,2 kg; nainen <10,8kg CDC 2000 -kasvukaavioiden mukaan) tutkimuksen aikaan.
