TAK-279:n jatkotutkimus aikuisilla, joilla on haavainen paksusuolitulehdus (UC) ja Crohnin tauti (CD)
Vaihe 2, avoin jatkokoe suun kautta otettavan tsasocitinibin (TAK-279) pitkäaikaisen turvallisuuden ja siedettävyyden arvioimiseksi potilailla, joilla on kohtalaisen tai vaikeasti aktiivinen haavainen paksusuolitulehdus ja kohtalaisen tai vaikeasti aktiivinen Crohnin tauti
Crohnin tauti ja haavainen paksusuolitulehdus ovat kahden tyyppisiä tulehduksellisia suolistosairauksia (IBD), jotka ovat vakava, pitkäaikainen sairaus suolistossa, joka voi aiheuttaa kipua ja turvotusta (tulehdusta) suolistossa. TAK-279 on lääke, joka auttaa estämään tulehdusta.
Tämä tutkimus on jatkoa emotutkimuksille TAK-279-CD-2001 (NCT06233461) ja TAK-279-UC-2001 (NCT06254950). Tämä tarkoittaa, että osallistujat, jotka reagoivat TAK-279-hoitoon kummassakin emotutkimuksessa, voivat edelleen hyötyä hoidosta tässä tutkimuksessa.
Tämän tutkimuksen päätavoitteena on selvittää, kuinka turvallinen TAK-279 on pitkäaikaisessa käytössä ja tarkistaa, vähentääkö se suolistotulehdusta ja -oireita, kun sitä käytetään pidemmän aikaa aikuisilla, joilla on keskivaikea tai vaikea aktiivinen UC tai CD.
Osallistujia hoidetaan TAK-279:llä enintään 2 vuotta (108 viikkoa).
Tutkimuksen aikana osallistujat vierailevat opintoklinikallaan 11 kertaa.

Kuvaus
Sisällön kriteerit:
Osallistuja on halukas ja kykenevä ymmärtämään ja täysin noudattamaan koemenettelyjä ja -vaatimuksia (mukaan lukien digitaaliset työkalut ja sovellukset), tutkijan mielestä.
Osallistuja on antanut tietoon perustuvan suostumuksen (eli kirjallisen, allekirjoitetun ja päivätyn tietoisen suostumuslomakkeen [ICF] kautta) ja tarvittavan tietosuojavaltuutuksen ennen kokeilumenettelyn aloittamista.
Tautikohtaiset sisällyttämiskriteerit:
- Viikon 52 loppuun saattaminen emovaiheen 2 CD- ja UC-kokeissa kelvollisilla sähköisillä (e) Päiväkirjatiedot viikolle 52 (TAK-279-CD-2001 ja TAK-279-UC-2001).
Kliininen tai oireenmukainen hoitovaste vanhempaintutkimuksessa viikolla 52, kuten alla on määritelty:
- TAK-279-CD-2001: Kliininen vaste PRO2:ssa vanhempaintutkimuksessa viikolla 52, arvioituna >=30 %:n vähenemisenä keskimääräisessä päivittäisessä erittäin pehmeässä tai nestemäisessä ulosteessa ja/tai >=30 %:n laskuna keskimääräisessä AP:ssa vanhempien tutkimuksen lähtötasosta.
- TAK-279-UC-2001: Oireellinen vaste vanhempaintutkimuksessa viikolla 52, arvioituna osittaisen modifioidun Mayo-pisteen (pmMS) pienenemisenä >=1 pisteellä ja >=30 %:lla vanhempaintutkimuksen lähtötasosta; ja peräsuolen verenvuodon alapistemäärän >=1 pisteen tai absoluuttisen peräsuolen verenvuodon alapisteen <=1 pisteen lasku emotutkimuksen lähtötasosta.
Muut yleiset sisällyttämiskriteerit:
- Osallistujien tulee noudattaa ehkäisyä koskevia suosituksia.
Poissulkemiskriteerit:
- Osallistuja, jonka tutkija katsoi sopimattomaksi OLE-tutkimukseen, koska hänellä on ongelmia kokeen noudattamiseen ja lääkityksen noudattamiseen.
Osallistujat, joilla on pahanlaatuinen kasvain tai dysplasia endoskopiaa kohti milloin tahansa emotutkimuksen aikana tai OLE:n alussa.
Laboratoriotutkimuksiin liittyvät poissulkemiskriteerit:
Osallistujat, jotka täyttävät pöytäkirjassa määritellyt laboratoriotutkimuksiin liittyvät poissulkemiskriteerit.
Muihin kiellettyihin samanaikaisiin lääkkeisiin liittyvät poissulkemiskriteerit:
- Osallistujat, jotka ottavat oraalisia kortikosteroideja CD:n tai UC:n hoitoon vanhempaintutkimuksen aikana viikolla 48 tai sen jälkeen.

The SES-CD score ranges from 0 to 56 which includes 4 endoscopic variables (intestinal surface affected by ulcers, intestinal surface affected by other inflammatory lesions, presence of ulcers, and presence of narrowing).

The mMS is a composite score of 3 assessments consisting of stool frequency (SF), rectal bleeding (RB), and endoscopic score (ES).
Each component sub-score ranges from 0 to 3 and total score range of the mMS is from 0 to 9, with higher scores indicating more severe disease.
Clinical remission is defined as Mayo RB sub-score of 0, Mayo SF sub-score of 0 or 1, and ES sub-score 1 or 0 (score of 1 modified to exclude friability).

The FACIT-Fatigue is a reliable and valid instrument for measuring fatigue.
The responses to the 13 items on the FACIT-Fatigue questionnaire are each measured on a 5-point Likert scale, where 0=Not at all, 1=A little bit, 2=Somewhat, 3=Quite a bit and 4=Very much.
The total score ranges from 0 to 52.
High scores represent less fatigue.
