Um estudo de continuação de TAK-279 em adultos com colite ulcerativa (UC) e doença de Crohn (DC)
Um ensaio de extensão aberto de fase 2 para avaliar a segurança e tolerabilidade em longo prazo do zasocitinibe oral (TAK-279) em participantes com colite ulcerativa ativa moderada a grave e doença de Crohn ativa moderada a grave
A doença de Crohn e a colite ulcerativa são dois tipos de doença inflamatória intestinal (DII), que é uma doença grave e de longo prazo no intestino (intestino) que pode causar dor e inchaço (inflamação) no intestino. TAK-279 é um medicamento que ajuda a bloquear a inflamação.
Este estudo é uma extensão dos estudos principais, TAK-279-CD-2001 (NCT06233461) e TAK-279-UC-2001 (NCT06254950). Isto significa que os participantes que responderam ao tratamento com TAK-279 em qualquer um dos estudos parentais poderão continuar a beneficiar do tratamento neste estudo.
O objetivo principal deste estudo é descobrir o quão seguro o TAK-279 é para uso a longo prazo e verificar se ele reduz a inflamação e os sintomas intestinais quando usado por um longo período de tempo em adultos com UC ou DC ativa moderada a grave.
Os participantes serão tratados com TAK-279 por até 2 anos (108 semanas).
Durante o estudo, os participantes visitarão a clínica do estudo 11 vezes.

Critérios de inclusão:
O participante deseja e é capaz de compreender e cumprir integralmente os procedimentos e requisitos do ensaio (incluindo ferramentas e aplicativos digitais), na opinião do investigador.
O participante forneceu consentimento informado (isto é, por escrito, documentado por meio de um formulário de consentimento informado assinado e datado [CIF]) e qualquer autorização de privacidade necessária antes do início de qualquer procedimento de estudo.
Critérios de inclusão específicos para doenças:
- Conclusão da Semana 52 nos ensaios parentais de fase 2 de CD e UC com dados eletrônicos válidos do (e) Diário para a Semana 52 (TAK-279-CD-2001 e TAK-279-UC-2001).
Respondente clínico ou sintomático na semana 52 do ensaio parental, conforme definido abaixo:
- TAK-279-CD-2001: Resposta clínica em PRO2 no ensaio parental Semana 52, avaliada como >=30% de redução na média diária de fezes muito moles ou líquidas e/ou >=30% de redução na PA média em relação à linha de base do ensaio parental.
- TAK-279-UC-2001: Resposta sintomática no ensaio parental, semana 52, avaliada como uma redução na pontuação de Mayo parcial modificada (pmMS) de >=1 ponto e >=30% da linha de base do ensaio parental; e uma diminuição da linha de base do ensaio parental na subpontuação de sangramento retal >=1 ponto ou uma subpontuação absoluta de sangramento retal <=1 ponto.
Outros critérios gerais de inclusão:
- Os participantes devem atender às recomendações de contracepção.
Critérios de exclusão:
- Participante considerado pelo investigador como inadequado para o estudo OLE devido à sua conformidade com o estudo e preocupações com adesão à medicação.
Participantes com malignidade ou displasia por endoscopia a qualquer momento durante o estudo dos pais ou no início do OLE.
Critérios de exclusão relacionados a investigações laboratoriais:
Participantes que atendem aos critérios de exclusão relacionados a investigações laboratoriais definidas no protocolo.
Critérios de exclusão relacionados a outras medicações concomitantes proibidas:
- Participantes que tomam corticosteroides orais para DC ou UC durante o estudo dos pais na ou após a semana 48.

The mMS is a composite score of 3 assessments consisting of stool frequency (SF), rectal bleeding (RB), and endoscopic score (ES).
Each component sub-score ranges from 0 to 3 and total score range of the mMS is from 0 to 9, with higher scores indicating more severe disease.
Clinical remission is defined as Mayo RB sub-score of 0, Mayo SF sub-score of 0 or 1, and ES sub-score 1 or 0 (score of 1 modified to exclude friability).

The FACIT-Fatigue is a reliable and valid instrument for measuring fatigue.
The responses to the 13 items on the FACIT-Fatigue questionnaire are each measured on a 5-point Likert scale, where 0=Not at all, 1=A little bit, 2=Somewhat, 3=Quite a bit and 4=Very much.
The total score ranges from 0 to 52.
High scores represent less fatigue.
