Eine Fortsetzungsstudie zu TAK-279 bei Erwachsenen mit Colitis ulcerosa (UC) und Morbus Crohn (CD)
Eine offene Phase-2-Verlängerungsstudie zur Bewertung der langfristigen Sicherheit und Verträglichkeit von oralem Zasocitinib (TAK-279) bei Teilnehmern mit mittelschwerer bis schwerer aktiver Colitis ulcerosa und mittelschwerem bis schwer aktivem Morbus Crohn
Morbus Crohn und Colitis ulcerosa sind zwei Arten von entzündlichen Darmerkrankungen (IBD), bei denen es sich um eine schwerwiegende, langfristige Erkrankung des Darms handelt, die Schmerzen und Schwellungen (Entzündungen) im Darm verursachen kann. TAK-279 ist ein Arzneimittel, das Entzündungen hemmt.
Diese Studie ist eine Erweiterung der Elternstudien TAK-279-CD-2001 (NCT06233461) und TAK-279-UC-2001 (NCT06254950). Dies bedeutet, dass Teilnehmer, die in einer der Elternstudien auf die Behandlung mit TAK-279 angesprochen haben, möglicherweise weiterhin von der Behandlung in dieser Studie profitieren können.
Das Hauptziel dieser Studie besteht darin, herauszufinden, wie sicher TAK-279 bei Langzeitanwendung ist, und zu überprüfen, ob es Darmentzündungen und -symptome bei längerer Anwendung bei Erwachsenen mit mittelschwerer bis schwerer aktiver UC oder CD reduziert.
Die Teilnehmer werden bis zu 2 Jahre (108 Wochen) mit TAK-279 behandelt.
Während der Studie besuchen die Teilnehmer ihre Studienklinik 11 Mal.

Beschreibung
Einschlusskriterien:
Der Teilnehmer ist nach Meinung des Prüfers bereit und in der Lage, die Verfahren und Anforderungen der Studie (einschließlich digitaler Tools und Anwendungen) zu verstehen und vollständig einzuhalten.
Der Teilnehmer hat seine Einverständniserklärung (d. h. schriftlich, dokumentiert über ein unterschriebenes und datiertes Einverständnisformular [ICF]) und alle erforderlichen Datenschutzgenehmigungen vor Beginn eines Testverfahrens abgegeben.
Krankheitsspezifische Einschlusskriterien:
- Abschluss von Woche 52 in den übergeordneten Phase-2-CD- und UC-Studien mit gültigen elektronischen (e) Tagebuchdaten für Woche 52 (TAK-279-CD-2001 und TAK-279-UC-2001).
Klinisches oder symptomatisches Ansprechen in Woche 52 der Elternstudie wie unten definiert:
- TAK-279-CD-2001: Klinische Reaktion in PRO2 in Woche 52 der Elternstudie, bewertet als >=30 %ige Abnahme des durchschnittlichen täglichen sehr weichen oder flüssigen Stuhls und/oder >=30 % Abnahme der durchschnittlichen AP gegenüber dem Ausgangswert der Elternstudie.
- TAK-279-UC-2001: Symptomatische Reaktion in Woche 52 der Elternstudie, bewertet als Verringerung des partiellen modifizierten Mayo-Scores (pmMS) um >=1 Punkte und >=30 % gegenüber dem Ausgangswert der Elternstudie; und ein Rückgang des Subscores für rektale Blutungen um >=1 Punkt oder ein absoluter Subscore für rektale Blutungen um <=1 Punkt gegenüber dem Ausgangswert der Elternstudie.
Weitere allgemeine Einschlusskriterien:
- Die Teilnehmer müssen die Verhütungsempfehlungen einhalten.
Ausschlusskriterien:
- Der Prüfer hält den Teilnehmer aufgrund seiner Bedenken bezüglich der Einhaltung der Studie und der Medikamenteneinhaltung für ungeeignet für die OLE-Studie.
Teilnehmer mit Malignität oder Dysplasie per Endoskopie jederzeit während der Elternstudie oder zu Beginn der OLE.
Ausschlusskriterien im Zusammenhang mit Laboruntersuchungen:
Teilnehmer, die die im Protokoll definierten Ausschlusskriterien für Laboruntersuchungen erfüllen.
Ausschlusskriterien im Zusammenhang mit anderen verbotenen Begleitmedikamenten:
- Teilnehmer, die während der Elternstudie in oder nach Woche 48 orale Kortikosteroide gegen CD oder UC einnehmen.

The mMS is a composite score of 3 assessments consisting of stool frequency (SF), rectal bleeding (RB), and endoscopic score (ES).
Each component sub-score ranges from 0 to 3 and total score range of the mMS is from 0 to 9, with higher scores indicating more severe disease.
Clinical remission is defined as Mayo RB sub-score of 0, Mayo SF sub-score of 0 or 1, and ES sub-score 1 or 0 (score of 1 modified to exclude friability).

The FACIT-Fatigue is a reliable and valid instrument for measuring fatigue.
The responses to the 13 items on the FACIT-Fatigue questionnaire are each measured on a 5-point Likert scale, where 0=Not at all, 1=A little bit, 2=Somewhat, 3=Quite a bit and 4=Very much.
The total score ranges from 0 to 52.
High scores represent less fatigue.
