Uno studio di continuazione di TAK-279 negli adulti con colite ulcerosa (UC) e malattia di Crohn (CD)
Uno studio di estensione in aperto di fase 2 per valutare la sicurezza e la tollerabilità a lungo termine di zasocitinib orale (TAK-279) in partecipanti con colite ulcerosa attiva da moderatamente a grave e malattia di Crohn attiva da moderatamente a grave
La malattia di Crohn e la colite ulcerosa sono due tipi di malattia infiammatoria intestinale (IBD), che è una condizione grave a lungo termine dell'intestino che può causare dolore e gonfiore (infiammazione) nell'intestino. TAK-279 è un medicinale che aiuta a bloccare l'infiammazione.
Questo studio è un'estensione degli studi principali, TAK-279-CD-2001 (NCT06233461) e TAK-279-UC-2001 (NCT06254950). Ciò significa che i partecipanti che hanno risposto al trattamento con TAK-279 in uno degli studi principali potrebbero continuare a beneficiare del trattamento in questo studio.
Lo scopo principale di questo studio è scoprire quanto TAK-279 sia sicuro per l'uso a lungo termine e verificare se riduce l'infiammazione e i sintomi intestinali se usato per un periodo di tempo più lungo negli adulti con CU o MC attiva da moderata a grave.
I partecipanti saranno trattati con TAK-279 per un massimo di 2 anni (108 settimane).
Durante lo studio, i partecipanti visiteranno la clinica dello studio 11 volte.

Criteri di inclusione:
A giudizio dello sperimentatore, il partecipante è disposto e in grado di comprendere e rispettare pienamente le procedure e i requisiti dello studio (compresi strumenti e applicazioni digitali).
Il partecipante ha fornito il consenso informato (ovvero, per iscritto, documentato tramite un modulo di consenso informato firmato e datato [ICF]) e qualsiasi autorizzazione alla privacy richiesta prima dell'avvio di qualsiasi procedura di sperimentazione.
Criteri di inclusione specifici per la malattia:
- Completamento della settimana 52 negli studi principali di fase 2 su CD e UC con dati validi del diario elettronico (e) per la settimana 52 (TAK-279-CD-2001 e TAK-279-UC-2001).
Responder clinico o sintomatico alla settimana 52 dello studio principale come definito di seguito:
- TAK-279-CD-2001: Risposta clinica nello studio PRO2 alla settimana 52 dello studio genitore, valutata come diminuzione >=30% della media giornaliera di feci molto molli o liquide e/o diminuzione >=30% dell'AP medio rispetto al basale dello studio genitore.
- TAK-279-UC-2001: Risposta sintomatica alla settimana 52 dello studio parentale, valutata come riduzione del punteggio Mayo modificato parziale (pmMS) di >=1 punto e >=30% rispetto al basale dello studio parentale; e una diminuzione rispetto al basale dello studio originale nel sottopunteggio del sanguinamento rettale di >=1 punto o di un sottopunteggio assoluto del sanguinamento rettale di <=1 punto.
Altri criteri generali di inclusione:
- I partecipanti devono soddisfare le raccomandazioni sulla contraccezione.
Criteri di esclusione:
- Partecipante considerato dallo sperimentatore non idoneo allo studio OLE a causa della conformità allo studio e dei problemi di aderenza ai farmaci.
Partecipanti con tumore maligno o displasia per endoscopia in qualsiasi momento durante lo studio dei genitori o all'inizio dell'OLE.
Criteri di esclusione relativi alle indagini di laboratorio:
Partecipanti che soddisfano i criteri di esclusione relativi alle indagini di laboratorio come definiti nel protocollo.
Criteri di esclusione relativi ad altri farmaci concomitanti proibiti:
- Partecipanti che assumevano corticosteroidi orali per CD o CU durante lo studio con i genitori alla o dopo la settimana 48.

The mMS is a composite score of 3 assessments consisting of stool frequency (SF), rectal bleeding (RB), and endoscopic score (ES).
Each component sub-score ranges from 0 to 3 and total score range of the mMS is from 0 to 9, with higher scores indicating more severe disease.
Clinical remission is defined as Mayo RB sub-score of 0, Mayo SF sub-score of 0 or 1, and ES sub-score 1 or 0 (score of 1 modified to exclude friability).

The FACIT-Fatigue is a reliable and valid instrument for measuring fatigue.
The responses to the 13 items on the FACIT-Fatigue questionnaire are each measured on a 5-point Likert scale, where 0=Not at all, 1=A little bit, 2=Somewhat, 3=Quite a bit and 4=Very much.
The total score ranges from 0 to 52.
High scores represent less fatigue.
