Une étude de continuation du TAK-279 chez des adultes atteints de colite ulcéreuse (CU) et de maladie de Crohn (MC)
Un essai d'extension ouvert de phase 2 pour évaluer l'innocuité et la tolérance à long terme du zasocitinib oral (TAK-279) chez les participants atteints de colite ulcéreuse modérément à sévèrement active et de maladie de Crohn modérément à sévèrement active
La maladie de Crohn et la colite ulcéreuse sont deux types de maladies inflammatoires de l'intestin (MII), qui sont une maladie grave et à long terme de l'intestin (intestin) qui peut provoquer des douleurs et un gonflement (inflammation) de l'intestin. TAK-279 est un médicament qui aide à bloquer l'inflammation.
Cette étude est une extension des études parentales TAK-279-CD-2001 (NCT06233461) et TAK-279-UC-2001 (NCT06254950). Cela signifie que les participants qui ont répondu au traitement par TAK-279 dans l'une ou l'autre des études parentales pourraient continuer à bénéficier du traitement dans cette étude.
L'objectif principal de cette étude est de déterminer dans quelle mesure le TAK-279 est sûr pour une utilisation à long terme et de vérifier s'il réduit l'inflammation et les symptômes intestinaux lorsqu'il est utilisé pendant une période plus longue chez les adultes atteints de CU ou de MC actives modérément à sévèrement.
Les participants seront traités avec TAK-279 pendant une durée maximale de 2 ans (108 semaines).
Au cours de l'étude, les participants visiteront leur clinique d'étude 11 fois.

La description
Critères d'intégration :
Le participant est disposé et capable de comprendre et de se conformer pleinement aux procédures et exigences de l'essai (y compris les outils et applications numériques), de l'avis de l'enquêteur.
Le participant a fourni son consentement éclairé (c'est-à-dire par écrit, documenté via un formulaire de consentement éclairé [ICF] signé et daté) et toute autorisation de confidentialité requise avant le lancement de toute procédure d'essai.
Critères d'inclusion spécifiques à la maladie :
- Achèvement de la semaine 52 dans les essais parents de phase 2 CD et UC avec des données de journal électronique (e) valides pour la semaine 52 (TAK-279-CD-2001 et TAK-279-UC-2001).
Répondeur clinique ou symptomatique à la semaine 52 de l'essai parent, tel que défini ci-dessous :
- TAK-279-CD-2001 : réponse clinique dans PRO2 lors de la semaine 52 de l'essai parent, évaluée comme une diminution >=30 % de la moyenne quotidienne des selles très molles ou liquides et/ou une diminution >=30 % de la PA moyenne par rapport à la ligne de base de l'essai parent.
- TAK-279-UC-2001 : réponse symptomatique à la semaine 52 de l'essai parent, évaluée comme une réduction du score Mayo modifié partiel (pmMS) de >=1 point et >=30 % par rapport à la ligne de base de l'essai parent ; et une diminution par rapport à la ligne de base de l'essai parent du sous-score de saignement rectal de >=1 point ou d'un sous-score de saignement rectal absolu de <=1 point.
Autres critères généraux d'inclusion :
- Les participants doivent respecter les recommandations en matière de contraception.
Critères d'exclusion :
- Participant considéré par l'investigateur comme inapte à l'essai OLE en raison de ses problèmes de conformité à l'essai et d'observance des médicaments.
Participants atteints d'une tumeur maligne ou d'une dysplasie par endoscopie à tout moment pendant l'essai parent ou au début de l'OLE.
Critères d'exclusion liés aux enquêtes de laboratoire :
Participants répondant aux critères d'exclusion liés aux analyses de laboratoire telles que définies dans le protocole.
Critères d'exclusion liés aux autres médicaments concomitants interdits :
- Participants prenant des corticostéroïdes oraux pour la MC ou la CU pendant l'essai parental à partir de la semaine 48.

Clinical remission is defined as Mayo RB sub-score of 0, Mayo SF sub-score of 0 or 1, and ES sub-score 1 or 0 (score of 1 modified to exclude friability).

The responses to the 13 items on the FACIT-Fatigue questionnaire are each measured on a 5-point Likert scale, where 0=Not at all, 1=A little bit, 2=Somewhat, 3=Quite a bit and 4=Very much.
The total score ranges from 0 to 52.
High scores represent less fatigue.
