SHAPE-ENDO: Monimuotoinen ennen leikkausta tehtävä optimointi potilailla, joilla on ylipaino ja varhaisen vaiheen kohdunrungon syöpä (SHAPE-ENDO)
SHAPE-ENDO (Semaglutiidi + Hormonaalinen Lähestymistapa ja Prehabilitaatio Endometriumkarsinoomassa): Prospektiivinen Observaatiotutkimus Metabolisen ja Kirurgisen Optimointiin Potilailla, Joilla on Epätyypillinen Endometriumhyperplasia tai Varhainen Vaiheen Endometriumkarsinooma ja BMI ≥35.
SHAPE-ENDO on prospektiivinen havainnoiva tutkimus, joka toteutetaan Hospital Universitari de Bellvitge -sairaalassa ja arvioi monimuotoista leikkaukseen valmistautumisen optimointistrategiaa ylipainoisten (painoindeksi ≥35) naisten kohdalla, joilla on epätyypillinen endometriumhyperplasia tai varhaisen vaiheen kohdunrungon syöpä. Osallistujat saavat vakiintuneita hoitointerventioita, kuten semaglutiditerapiaa, levonorgestrelin kohdunsisäisen laitteen (suun kautta otettavien progestiinien kanssa tai ilman), rakenteellista ravitsemus- ja liikuntaohjelmaa sekä suunniteltuja endometriumseurantoja.
Tutkimuksen tavoitteena on arvioida, parantaako tämä monimuotoinen strategia aineenvaihdunnan terveyttä, edistääkö painonpudotusta ja lisääkö se minimi-invasiivisen leikkauksen soveltuvuutta samalla kun säilytetään syöpäturvallisuus optimointijakson aikana.
Osallistujia seurataan 6–12 kuukauden ajan antropometristen ja aineenvaihduntaparametrien, histologisen vastemuutoksen, elämänlaadun ja hoidon noudattamisen seurannan avulla. Kaikki interventiot ovat osa rutiinikliinistä hoitoa. Tämän tutkimuksen tulokset voivat antaa tietoa tuleville vertaileville tutkimuksille, jotka arvioivat aineenvaihdunnan optimointistrategioita ylipainoisten potilaiden ja varhaisen vaiheen kohdunrungon syövän hoidossa.

Yksityiskohtainen kuvaus
Lihavuus on merkittävä riskitekijä kohdunrunsaassyöpälle ja liittyy lisääntyneeseen kirurgiseen monimutkaisuuteen, perioperatiiviseen sairastavuuteen ja vähentyneeseen kelpoisuuteen vähäinvasiiviseen leikkaukseen. Potilailla, joilla on vaikea lihavuus, kuolleisuuteen vaikuttaa usein metaboliset sairaudet enemmän kuin syövän eteneminen. Siksi kirurgiaa edeltävät strategiat, joiden tavoitteena on parantaa metabolista terveyttä ja toimintakykyä, voivat olla tärkeitä kliinisesti.
Hormonaalisia hoitoja, kuten levonorgestrelia vapauttavaa kohdunkaulalevyä ja systeemisiä progestiineja, käytetään yleisesti taudin hallintaan valituilla potilailla, joilla on epätyypillinen endometriumhyperplasia tai varhaisvaiheen endometrioidikarsinooma. Samanaikaisesti glukagonin kaltaiset peptidi-1-reseptoriagonistit, mukaan lukien semaglutiidi, ovat osoittaneet merkittävää painonpudotusta ja kardiometabolista hyötyä. Monimuotoiset prehabilitaatiomenetelmät, jotka yhdistävät lääkehoitoa, ravitsemuksen optimointia ja rakenteellisia liikuntaohjelmia, voivat parantaa kirurgista valmiutta ja kokonaisterveydentilaa.
SHAPE-ENDO on prospektiivinen havainnointikohorttitutkimus, jota suoritetaan Hospital Universitari de Bellvitgessä. Tuloksiin kuuluvat metaboliset ja antropometriset muutokset, histologinen vaste, elämänlaatu ja kelpoisuus vähäinvasiiviseen leikkaukseen. Tiedot kerätään sähköisten potilastietojen, laboratoriotestien, kuvantamistutkimusten ja validoitujen potilasarvioitujen tulosten kyselylomakkeiden avulla. Tutkimus on saanut hyväksynnän institutionaaliselta eettiseltä toimikunnalta ja sisältää vain standardinhoitointerventioita.

- Naiset ≥18 vuotta vanhoja.
- Histologisesti varmistettu Atypinen Endometriumhyperplasia / EIN tai matalariskinen endometrioidinen endometriumkarsinooma (G1-G2), rajoittunut kohdun runkoon.
- Luokiteltu matalaan/keskitasoon ESGO-ESTRO-ESP 2025:n mukaan; leikkauskohdistetut vaiheet IA1, IA2 tai IB.
- Negatiivinen tai fokaalinen LVSI (jos saatavilla).
- Molekulaariset alaryhmät: POLEmut, p53wt, MMRd tai NSMP ER+.
- BMI ≥35 kg/m² sisällyttämishetkellä.
- Väliaikaisen konservatiivisen hoidon hyväksyntä ja kyky seurata monimuotoista optimointistrategiaa.

Active Comparator: Arm A - Standard Immediate Surgery
Participants randomized to the control arm will undergo standard immediate surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge. Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and minimally invasive or robotic approach whenever technically possible according to clinical judgment. Intervention: Procedure/Surgery - Standard Immediate Surgery Standard surgical management according to institutional practice for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer. Perioperative outcomes, surgical approach, conversion to laparotomy, estimated blood loss, operative time, hospital stay, transfusion, sentinel lymph node detection, intraoperative complications, and 30-day postoperative complications graded according to Clavien-Dindo will be recorded. |

Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and a minimally invasive or robotic approach whenever technically possible according to clinical judgment.

Kokeellinen: Arm B - Experimental: SHAPE-ENDO Multimodal Strategy Before Surgery
Participants randomized to the experimental arm will receive the SHAPE-ENDO multimodal pre-surgical optimization strategy before surgery. The strategy includes semaglutide/Wegovy®, levonorgestrel-releasing intrauterine device/Mirena® with or without oral medroxyprogesterone acetate/Progevera®, structured nutritional intervention, adapted physical exercise, and scheduled oncologic surveillance. The strategy will last 28 weeks initially and may be extended up to 54 weeks if there is clinical, metabolic, or anthropometric benefit, adequate tolerance, and no tumor progression. Intervention: Drug - Semaglutide / Wegovy® Weekly subcutaneous semaglutide administered according to approved labeling, clinical indication, tolerance, and endocrinology assessment, with standard dose escalation up to the tolerated therapeutic dose.
