SHAPE-ENDO : Optimisation Pré-Chirurgicale Multimodale chez les Patient(e)s Obèses atteint(e)s d'un Cancer de l'Endomètre à un Stade Précoce (SHAPE-ENDO)
SHAPE-ENDO (Sémaglutide + Approche Hormonale et Préréadaptation dans le Cancer de l'Endomètre) : Étude Observationnelle Prospective pour l'Optimisation Métabolique et Chirurgicale chez les Patient(e)s avec Hyperplasie Endométriale Atypique ou Cancer de l'Endomètre de Stade Précoce et IMC ≥35.
SHAPE-ENDO est une étude observationnelle prospective menée à l'Hôpital Universitari de Bellvitge évaluant une stratégie d'optimisation pré-chirurgicale multimodale pour les femmes obèses (IMC ≥ 35) présentant une hyperplasie endométriale atypique ou un cancer de l'endomètre à un stade précoce. Les participantes reçoivent des interventions standard comprenant un traitement par sémaglutide, un dispositif intra-utérin au lévonorgestrel (avec ou sans progestatifs oraux), des programmes structurés de nutrition et d'exercice, ainsi qu'une surveillance endométriale programmée.
L'étude vise à évaluer si cette stratégie multimodale améliore la santé métabolique, favorise la perte de poids et augmente l'éligibilité à une chirurgie mini-invasive, tout en garantissant la sécurité oncologique pendant la période d'optimisation.
Les participantes sont suivies pendant 6 à 12 mois avec un suivi des paramètres anthropométriques et métaboliques, de la réponse histologique, de la qualité de vie et de l'adhésion au traitement. Toutes les interventions font partie des soins cliniques de routine. Les résultats de cette étude pourront orienter de futurs essais comparatifs évaluant les stratégies d'optimisation métabolique chez les patientes obèses atteintes d'un cancer de l'endomètre à un stade précoce.

Description détaillée
L'obésité est un facteur de risque majeur pour le cancer de l'endomètre et est associée à une complexité chirurgicale accrue, à une morbidité périopératoire et à une réduction de l'éligibilité à la chirurgie mini-invasive. Chez les patients souffrant d'obésité sévère, la mortalité est fréquemment influencée par les comorbidités métaboliques plutôt que par la progression du cancer. Les stratégies visant à améliorer la santé métabolique et l'état fonctionnel avant la chirurgie peuvent donc avoir d'importantes implications cliniques.
Les thérapies hormonales telles que le dispositif intra-utérin à libération de lévonorgestrel et les progestatifs systémiques sont couramment utilisées pour le contrôle de la maladie chez certains patients présentant une hyperplasie endométriale atypique ou un carcinome endométrioïde de stade précoce. Parallèlement, les agonistes des récepteurs du peptide-1 de type glucagon, notamment le sémaglutide, ont démontré des bénéfices significatifs en termes de perte de poids et d'avantages cardiométaboliques. Les approches de préhabilitation multimodales combinant une thérapie pharmacologique, une optimisation nutritionnelle et des programmes d'exercice structurés peuvent améliorer la préparation chirurgicale et l'état de santé général.
SHAPE-ENDO est une étude de cohorte observationnelle prospective menée à l'Hospital Universitari de Bellvitge. L'étude évalue une stratégie d'optimisation pré-chirurgicale multimodale qui inclut la gestion pondérale pharmacologique avec le sémaglutide, le traitement endométrial hormonal avec un dispositif intra-utérin à lévonorgestrel avec ou sans progestatifs oraux, et des interventions sur le mode de vie incluant un conseil diététique supervisé et des programmes d'exercice. Les patients bénéficient d'un suivi clinique programmé avec surveillance métabolique, imagerie et prélèvement endométrial selon la pratique clinique habituelle.

Les données sont collectées via les dossiers médicaux électroniques, les tests de laboratoire, les études d'imagerie et des questionnaires validés rapportés par les patients. Critères d'inclusion :
- Femmes âgées ≥ 18 ans.
- Hyperplasie endométriale atypique / EIN ou carcinome endométrioïde de l'endomètre à faible risque (G1-G2) confirmé histologiquement, limité au corps utérin.
- Classifié comme risque faible/intermédiaire selon ESGO-ESTRO-ESP 2025 ; stades préchirurgicaux IA1, IA2 ou IB.
- LVSI négatif ou focal (si disponible).
- Sous-groupes moléculaires : POLEmut, p53wt, MMRd ou NSMP ER+.
- IMC ≥ 35 kg/m² à l'inclusion.
- Acceptation d'une prise en charge conservatrice temporaire et capacité à suivre une stratégie d'optimisation multimodale.
- Capacité à comprendre et signer le consentement éclairé.
Critères d'exclusion :
- Stades FIGO IA3, IC, II ou supérieurs.
- Invasion lymphovasculaire positive.
- Risque moléculaire élevé : p53mut ou NSMP ER-négatif.
- Histologies non endométrioïdes (séreux, à cellules claires, carcinosarcome, mixte, etc.).
- Maladie métastatique ou extra-utérine.
- Contre-indication aux GLP-1RA ou aux progestatifs.
- Antécédents de pancréatite, MEN-2, carcinome médullaire de la thyroïde.
- Participation à un autre essai pharmacologique.
- Toute condition qui, selon l'appréciation de l'investigateur, compromet la sécurité ou l'observance.

Comparateur actif: Arm A - Standard Immediate Surgery
Participants randomized to the control arm will undergo standard immediate surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge. Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and minimally invasive or robotic approach whenever technically possible according to clinical judgment. Intervention: Procedure/Surgery - Standard Immediate Surgery Standard surgical management according to institutional practice for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer. Perioperative outcomes, surgical approach, conversion to laparotomy, estimated blood loss, operative time, hospital stay, transfusion, sentinel lymph node detection, intraoperative complications, and 30-day postoperative complications graded according to Clavien-Dindo will be recorded. |
Standard surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer.
Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and a minimally invasive or robotic approach whenever technically possible according to clinical judgment.
Surgical approach, operative time, estimated blood loss, conversion to laparotomy, transfusion, hospital stay, intraoperative complications, 30-day postoperative complications, readmission, and sentinel lymph node detection will be recorded.

Expérimental: Arm B - Experimental: SHAPE-ENDO Multimodal Strategy Before Surgery
Participants randomized to the experimental arm will receive the SHAPE-ENDO multimodal pre-surgical optimization strategy before surgery. The strategy includes semaglutide/Wegovy®, levonorgestrel-releasing intrauterine device/Mirena® with or without oral medroxyprogesterone acetate/Progevera®, structured nutritional intervention, adapted physical exercise, and scheduled oncologic surveillance. The strategy will last 28 weeks initially and may be extended up to 54 weeks if there is clinical, metabolic, or anthropometric benefit, adequate tolerance, and no tumor progression. Intervention: Drug - Semaglutide / Wegovy® Weekly subcutaneous semaglutide administered according to approved labeling, clinical indication, tolerance, and endocrinology assessment, with standard dose escalation up to the tolerated therapeutic dose. Plan de régime hypocalorique personnalisé supervisé par l'équipe de nutrition clinique dans le cadre de la prise en charge standard de l'obésité et des troubles métaboliques.
Le programme comprend une restriction calorique basée sur les besoins métaboliques de base, avec l'option de régimes très hypocaloriques (VLCD) pendant 4 à 6 semaines dans certains cas sélectionnés.

The intervention is used for weight loss and metabolic optimization in participants with severe obesity.

The LNG-IUD is used within the protocolized SHAPE-ENDO strategy according to clinical indication, approved labeling, current guidelines, and physician judgment, for local disease control in atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage, low-risk endometrioid endometrial cancer.
Tolerability, continuation, adverse events, and local histological response will be recorded prospectively.

Recurrence may be defined by histological, radiological, or clinical evidence according to standard follow-up criteria.
Participants without recurrence will be censored at the date of last available follow-up.
