SHAPE-ENDO: Otimização Pré-Cirúrgica Multimodal em Doentes com Obesidade e Cancro do Endométrio em Fase Inicial (SHAPE-ENDO)
SHAPE-ENDO (Semaglutida + Abordagem Hormonal e Pré-habilitação no Cancro do Endométrio): Estudo Observacional Prospectivo para Otimização Metabólica e Cirúrgica em Doentes com Hiperplasia Endometrial Atípica ou Cancro do Endométrio em Fase Inicial e IMC ≥35.
SHAPE-ENDO é um estudo observacional prospetivo realizado no Hospital Universitari de Bellvitge, que avalia uma estratégia multimodal de otimização pré-cirúrgica para mulheres com obesidade (IMC ≥35) e hiperplasia endometrial atípica ou cancro do endométrio em estágio inicial. Os participantes recebem intervenções padrão, incluindo terapia com semaglutida, dispositivo intrauterino com levonorgestrel (com ou sem progestinas orais), programas estruturados de nutrição e exercício, e vigilância endometrial programada.
O estudo tem como objetivo avaliar se esta estratégia multimodal melhora a saúde metabólica, promove a perda de peso e aumenta a elegibilidade para cirurgia minimamente invasiva, mantendo a segurança oncológica durante o período de otimização.
Os participantes são acompanhados durante 6 a 12 meses, com monitorização de parâmetros antropométricos e metabólicos, resposta histológica, qualidade de vida e adesão ao tratamento. Todas as intervenções fazem parte dos cuidados clínicos de rotina. As conclusões deste estudo podem informar futuros ensaios comparativos que avaliem estratégias de otimização metabólica em pacientes com obesidade e cancro do endométrio em estágio inicial.

Descrição detalhada
A obesidade é um importante fator de risco para o cancro do endométrio e está associada a um aumento da complexidade cirúrgica, morbilidade perioperatória e redução da elegibilidade para cirurgia minimamente invasiva. Em doentes com obesidade grave, a mortalidade é frequentemente influenciada por comorbilidades metabólicas em vez da progressão do cancro. Estratégias destinadas a melhorar a saúde metabólica e o estado funcional antes da cirurgia podem, portanto, ter importantes implicações clínicas.
As terapias hormonais, como o dispositivo intrauterino libertador de levonorgestrel e os progestagénios sistémicos, são frequentemente utilizadas para controlo da doença em doentes selecionados com hiperplasia endometrial atípica ou carcinoma endometrióide em estádio inicial. Paralelamente, os agonistas do recetor do péptido-1 semelhante ao glucagon, incluindo a semaglutida, demonstraram benefícios significativos na perda de peso e na saúde cardiometabólica. Abordagens de pré-habilitação multimodal que combinam terapia farmacológica, otimização nutricional e programas de exercício estruturados podem melhorar a preparação cirúrgica e o estado geral de saúde.
O SHAPE-ENDO é um estudo prospetivo de coorte observacional realizado no Hospital Universitari de Bellvitge. O estudo avalia uma estratégia de otimização pré-cirúrgica multimodal que inclui gestão farmacológica do peso com semaglutida, tratamento hormonal endometrial com dispositivo intrauterino de levonorgestrel com ou sem progestagénios orais, e intervenções no estilo de vida, incluindo aconselhamento dietético supervisionado e programas de exercício. Os doentes são submetidos a seguimento clínico programado com monitorização metabólica, imagiologia e amostragem endometrial de acordo com a prática clínica de rotina.
O estudo tem como objetivo avaliar a viabilidade e o impacto clínico desta estratégia de otimização multimodal em mulheres com obesidade e doença endometrial em estádio inicial. Os resultados incluem alterações metabólicas e antropométricas, resposta histológica, qualidade de vida e elegibilidade para cirurgia minimamente invasiva. Os dados são recolhidos através de registos médicos eletrónicos, testes laboratoriais, estudos de imagiologia e questionários validados de resultados relatados pelos doentes. O estudo é aprovado pelo comité de ética institucional e envolve apenas intervenções padrão de cuidados.

Critérios de Inclusão:
- Mulheres ≥18 anos.
- Hiperplasia Endometrial Atípica / EIN ou carcinoma endometrial endometrióide de baixo risco (G1-G2) confirmado histologicamente, confinado ao corpo uterino.
- Classificado como baixo/risco intermédio segundo ESGO-ESTRO-ESP 2025; estádios pré-cirúrgicos IA1, IA2 ou IB.
- LVSI negativo ou focal (se disponível).
- Subgrupos moleculares: POLEmut, p53wt, MMRd ou NSMP ER+.
- IMC ≥35 kg/m² na inclusão.
- Aceitação de gestão conservadora temporária e capacidade de seguir estratégia de otimização multimodal.
- Capacidade de compreender e assinar o consentimento informado.
Critérios de Exclusão:
- Estádios FIGO IA3, IC, II ou superior.
- Invasão linfovascular positiva.
- Risco molecular elevado: p53mut ou NSMP ER-negativo.
- Histologias não endometrióides (seroso, células claras, carcinossarcoma, misto, etc.).
- Doença metastática ou extra-uterina.
- Contraindicação para GLP-1RA ou progestogénios.
- Pancreatite prévia, MEN-2, carcinoma medular da tiroide.
- Participação noutro ensaio farmacológico.
- Qualquer condição que, na opinião do investigador, comprometa a segurança ou a adesão.

Comparador Ativo: Arm A - Standard Immediate Surgery
Participants randomized to the control arm will undergo standard immediate surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge. Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and minimally invasive or robotic approach whenever technically possible according to clinical judgment. Intervention: Procedure/Surgery - Standard Immediate Surgery Standard surgical management according to institutional practice for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer. Perioperative outcomes, surgical approach, conversion to laparotomy, estimated blood loss, operative time, hospital stay, transfusion, sentinel lymph node detection, intraoperative complications, and 30-day postoperative complications graded according to Clavien-Dindo will be recorded. |
Standard surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer.
Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and a minimally invasive or robotic approach whenever technically possible according to clinical judgment.
Surgical approach, operative time, estimated blood loss, conversion to laparotomy, transfusion, hospital stay, intraoperative complications, 30-day postoperative complications, readmission, and sentinel lymph node detection will be recorded.

Experimental: Arm B - Experimental: SHAPE-ENDO Multimodal Strategy Before Surgery
Participants randomized to the experimental arm will receive the SHAPE-ENDO multimodal pre-surgical optimization strategy before surgery. The strategy includes semaglutide/Wegovy®, levonorgestrel-releasing intrauterine device/Mirena® with or without oral medroxyprogesterone acetate/Progevera®, structured nutritional intervention, adapted physical exercise, and scheduled oncologic surveillance. The strategy will last 28 weeks initially and may be extended up to 54 weeks if there is clinical, metabolic, or anthropometric benefit, adequate tolerance, and no tumor progression. Intervention: Drug - Semaglutide / Wegovy® Weekly subcutaneous semaglutide administered according to approved labeling, clinical indication, tolerance, and endocrinology assessment, with standard dose escalation up to the tolerated therapeutic dose.
