SHAPE-ENDO: Multimodale präoperative Optimierung bei Patientinnen mit Adipositas und Endometriumkarzinom im Frühstadium (SHAPE-ENDO)
SHAPE-ENDO (Semaglutid + Hormoneller Ansatz & Prähabilitation bei Endometriumkarzinom): Prospektive Beobachtungsstudie zur metabolischen und chirurgischen Optimierung bei Patientinnen mit atypischer Endometriumhyperplasie oder Endometriumkarzinom im Frühstadium und BMI ≥35.
SHAPE-ENDO ist eine prospektive Beobachtungsstudie, die am Hospital Universitari de Bellvitge durchgeführt wird und eine multimodale präoperative Optimierungsstrategie für Frauen mit Adipositas (BMI ≥35) und atypischer endometrialer Hyperplasie oder Endometriumkarzinom im Frühstadium evaluiert. Die Teilnehmerinnen erhalten Standardinterventionen, einschließlich Semaglutid-Therapie, Levonorgestrel-Intrauterinpessar (mit oder ohne orale Gestagene), strukturierte Ernährungs- und Bewegungsprogramme sowie regelmäßige endometriale Überwachung.
Die Studie zielt darauf ab, zu bewerten, ob diese multimodale Strategie die metabolische Gesundheit verbessert, Gewichtsverlust fördert und die Eignung für minimalinvasive Chirurgie erhöht, während die onkologische Sicherheit während des Optimierungszeitraums gewährleistet bleibt.
Die Teilnehmerinnen werden über 6-12 Monate mit Überwachung anthropometrischer und metabolischer Parameter, histologischer Ansprechraten, Lebensqualität und Therapietreue begleitet. Alle Interventionen sind Teil der routinemäßigen klinischen Versorgung. Die Ergebnisse dieser Studie könnten zukünftige vergleichende Studien zur Bewertung metabolischer Optimierungsstrategien bei Patienten mit Adipositas und Endometriumkarzinom im Frühstadium informieren.

Detaillierte Beschreibung
Adipositas ist ein bedeutender Risikofaktor für Endometriumkarzinome und steht in Zusammenhang mit erhöhter chirurgischer Komplexität, perioperativer Morbidität und reduzierter Eignung für minimalinvasive Eingriffe. Bei Patientinnen mit schwerer Adipositas wird die Mortalität häufig eher durch metabolische Komorbiditäten als durch das Fortschreiten des Krebses beeinflusst. Strategien zur Verbesserung der metabolischen Gesundheit und des Funktionsstatus vor der Operation können daher wichtige klinische Implikationen haben.
Hormonelle Therapien wie das Levonorgestrel-freisetzende Intrauterinpessar und systemische Gestagene werden häufig zur Krankheitskontrolle bei ausgewählten Patientinnen mit atypischer Endometriumhyperplasie oder Endometrioidkarzinom im Frühstadium eingesetzt. Parallel dazu haben Glucagon-like-Peptide-1-Rezeptor-Agonisten, einschließlich Semaglutid, signifikante Gewichtsreduktion und kardiometabolische Vorteile gezeigt. Multimodale Prähabilitationsansätze, die pharmakologische Therapie, Ernährungsoptimierung und strukturierte Bewegungsprogramme kombinieren, können die Operationsbereitschaft und den allgemeinen Gesundheitszustand verbessern.
SHAPE-ENDO ist eine prospektive Beobachtungs-Kohortenstudie, die am Hospital Universitari de Bellvitge durchgeführt wird. Einschlusskriterien:
- Frauen ≥18 Jahre alt.
- Histologisch bestätigte atypische Endometriumhyperplasie / EIN oder niedrigrisiko endometrioides Endometriumkarzinom (G1-G2), auf den Uteruskörper beschränkt.
- Eingestuft als niedriges/intermediäres Risiko gemäß ESGO-ESTRO-ESP 2025; präoperative Stadien IA1, IA2 oder IB.
- Negative oder fokale LVSI (falls verfügbar).
- Molekulare Subgruppen: POLEmut, p53wt, MMRd oder NSMP ER+.
- BMI ≥35 kg/m² bei Einschluss.
- Akzeptanz eines temporären konservativen Managements und Fähigkeit zur Umsetzung einer multimodalen Optimierungsstrategie.
- Fähigkeit zur Verständnis und Unterzeichnung der Einwilligungserklärung.
Ausschlusskriterien:
- FIGO-Stadien IA3, IC, II oder höher.
- Positive lymphovaskuläre Invasion.
- Hochrisiko-Molekular: p53mut oder NSMP ER-negativ.
- Nicht-endometrioide Histologien (serös, klarzellig, Karzinosarkom, gemischt, etc.).
- Metastatische oder extrauterine Erkrankung.
- Kontraindikation für GLP-1RA oder Gestagene.
- Frühere Pankreatitis, MEN-2, medulläres Schilddrüsenkarzinom.
- Teilnahme an einer anderen pharmakologischen Studie.
- Jeder Zustand, der nach Einschätzung des Prüfers die Sicherheit oder Compliance beeinträchtigt.

Aktiver Komparator: Arm A - Standard Immediate Surgery
Participants randomized to the control arm will undergo standard immediate surgical treatment according to the institutional protocol of Hospital Universitari de Bellvitge. Surgery will usually include hysterectomy with bilateral salpingo-oophorectomy, sentinel lymph node assessment when indicated and feasible, and minimally invasive or robotic approach whenever technically possible according to clinical judgment. Intervention: Procedure/Surgery - Standard Immediate Surgery Standard surgical management according to institutional practice for atypical endometrial hyperplasia/endometrial intraepithelial neoplasia or early-stage low-risk endometrioid endometrial cancer. Perioperative outcomes, surgical approach, conversion to laparotomy, estimated blood loss, operative time, hospital stay, transfusion, sentinel lymph node detection, intraoperative complications, and 30-day postoperative complications graded according to Clavien-Dindo will be recorded. |

Experimental: Arm B - Experimental: SHAPE-ENDO Multimodal Strategy Before Surgery
Participants randomized to the experimental arm will receive the SHAPE-ENDO multimodal pre-surgical optimization strategy before surgery. The strategy includes semaglutide/Wegovy®, levonorgestrel-releasing intrauterine device/Mirena® with or without oral medroxyprogesterone acetate/Progevera®, structured nutritional intervention, adapted physical exercise, and scheduled oncologic surveillance. The strategy will last 28 weeks initially and may be extended up to 54 weeks if there is clinical, metabolic, or anthropometric benefit, adequate tolerance, and no tumor progression. Intervention: Drug - Semaglutide / Wegovy® Weekly subcutaneous semaglutide administered according to approved labeling, clinical indication, tolerance, and endocrinology assessment, with standard dose escalation up to the tolerated therapeutic dose.
