ARRIÈRE-PLAN:
La corrélation entre des taux élevés de cholestérol sérique et une augmentation de l'incidence et de la prévalence des maladies coronariennes a été démontrée - avant le lancement du Coronary Drug Project - à plusieurs reprises dans des enquêtes épidémiologiques prospectives et transversales (par exemple, l'étude Tecumseh , l'étude sur les maladies cardiaques de Framingham). Ces résultats ont conduit à la question de savoir si la baisse à long terme des lipides sériques chez les individus avec et sans coronaropathie aurait un effet bénéfique sur la morbidité et la mortalité.
Le Coronary Drug Project a été conçu pour répondre à la question de la prévention secondaire. En 1961, le Dr Robert Wilkins (Boston University School of Medicine) a présidé un comité ad hoc qui a déterminé l'opportunité et la faisabilité de la conduite de cette étude. Suite au soutien du National Heart Advisory Council (NHAC), un conseil d'orientation, un comité directeur et un centre de coordination de l'étude ont été créés et un protocole détaillé a été rédigé.
En 1964, la NHAC a approuvé le projet et la recommandation de la NHI pour la mise en œuvre; l'étude a commencé en 1965. Soutenu par le mécanisme de subvention, l'essai a impliqué 53 cliniques participantes, un centre de coordination, un laboratoire central, un centre ECG, un centre d'approvisionnement et de distribution de médicaments et un bureau de liaison médicale de l'INSA, ainsi qu'un conseil d'orientation, un comité directeur et 12 autres comités (par exemple, un comité de suivi des données et de la sécurité).
Le premier patient a été assigné au hasard au traitement en mars 1966 et le dernier en octobre 1969. Chaque patient se présentait à la clinique tous les 4 mois pour une visite de suivi.
NARRATIF DE CONCEPTION :
Échantillon fixe randomisé en double aveugle. Au total, 8 341 patientes ont été assignées au hasard à six groupes de traitement consistant en 2,5 mg/jour d'œstrogènes conjugués, 5,0 mg/jour d'œstrogènes conjugués, 1,8 g/jour de clofibrate, 6,0 mg/jour de dextrothyroxine sodique, 3,0 g/jour de niacine ou 3,8 g/jour de lactose placebo.
