Une étude de phase II sur la radiothérapie à base de protons avec irradiation nodale pelvienne élective, docétaxel concomitant et privation androgénique adjuvante pour l'adénocarcinome de la prostate à haut risque
Le but de cette étude est de voir quels effets, bons et/ou mauvais, les rayonnements à base de protons combinés à une chimiothérapie et une hormonothérapie à faible dose ont sur les patients et leur cancer.

Critère d'intégration:
* Adénocarcinome de la prostate.
Critère d'exclusion:
- Traitement antérieur du cancer de la prostate, comme la chimiothérapie et/ou la radiothérapie pelvienne.
- Maladie intestinale inflammatoire active (diverticulite, maladie de Crohn ou rectocolite hémorragique) affectant le rectum. (La diverticulite non active et la maladie de Crohn n'affectant pas le rectum sont autorisées.)
- Antécédents de remplacement de la hanche.
- Chirurgie intrapelvienne antérieure. Cela inclut les éléments suivants :
- Chirurgie transrectale ou rectale autre que la polypectomie ou l'ablation ou l'anneau des hémorroïdes
- Chirurgie pelvienne transabdominale
- Chirurgie de la vessie
- Antécédents d'infarctus du myocarde (IM) ou d'insuffisance cardiaque congestive (ICC).
- Prendre du Saw Palmetto ou du méthotrexate et ne pas pouvoir ou ne pas vouloir interrompre son utilisation pendant la radiothérapie.

< 15 % de risque de + ganglions lymphatiques (LN)
|
Total de 54 équivalents de gris cobalt (CGE) sur 30 traitements de la prostate + vésicules séminales (SV), puis proton boost total de 23,4-27 CGE sur 13-15 traitements de la prostate +/- SV.
Docétaxel à faible dose chaque semaine pendant la radiothérapie (RT) suivie d'une thérapie de suppression androgénique pendant 6 mois.

> 15% risque de + LN

Total de 45 Gy sur 25 traitements sur prostate + SV + LN, puis proton boost total de 32,4-36 CGE sur 18-20 traitements sur prostate + SV.
Docétaxel à faible dose chaque semaine pendant la RT suivi d'un traitement par privation androgénique pendant 6 mois.
