L'étude sur la santé des Portoricains de Boston : Centre pour la santé de la population et les disparités en matière de santé
L'objectif à long terme des chercheurs est de comprendre les interactions complexes entre l'alimentation et d'autres facteurs comportementaux et environnementaux, la génétique et le stress psychosocial sur la prévalence élevée et apparemment croissante des facteurs de risque de maladies cardiovasculaires (MCV) chez les adultes portoricains.

Les adultes portoricains vivant sur le continent américain ont documenté des disparités en matière de santé ; cependant, peu de recherches ont été menées avec ce deuxième plus grand sous-groupe hispanique.
Notre objectif à long terme est de comprendre les interactions complexes entre l'alimentation et d'autres facteurs comportementaux et environnementaux, la génétique et le stress psychosocial sur la prévalence élevée et apparemment croissante des facteurs de risque de maladies cardiovasculaires (MCV) chez les adultes portoricains.
Comme ce groupe se développe rapidement, comprendre les raisons de ce risque est d'une grande importance.
Notre période de financement initiale pour le Boston Puerto Rican Center for Population Health and Health Disparities (BPR-CPHHD, 2003-08) s'est concentrée sur le rôle du stress sur l'incapacité physique et le déclin cognitif, par le biais d'une dérégulation physiologique ou "charge allostatique".
Au cours de cette enquête, il est devenu clair que les facteurs de risque de MCV étaient très répandus.
Ces résultats vont à l'encontre de la croyance répandue selon laquelle il existe un paradoxe hispanique - une baisse des maladies cardiaques et de la mortalité malgré une plus grande pauvreté.
Il est important de noter que la population portoricaine diffère considérablement dans l'histoire génétique ancestrale et dans l'exposition aux facteurs de risque connus des autres groupes hispaniques.
Ils ont des habitudes alimentaires uniques, ainsi que des structures sociales, culturelles et environnementales qui contribuent et affectent la réaction aux facteurs de stress.
Au cours de notre période de financement initiale, nous avons réuni avec succès une cohorte de 1500 adultes portoricains, âgés de 45 à 75 ans au départ, et avons réalisé des entretiens de suivi de deux ans sur plus de 1250 participants.
Comme cette population vieillit et croît rapidement, la prévalence élevée et l'effet de cohorte apparent des facteurs de risque accrus de maladies cardiaques suggèrent que 1) cette population présente de graves disparités en matière de santé dans les facteurs de risque de maladies cardiaques et 2) les maladies cardiaques deviendront un problème encore plus important pour cette population. groupe dans un futur proche.
Notre objectif général pour ce renouvellement est donc d'étendre le suivi et de mesurer et d'analyser les caractéristiques pertinentes et les facteurs de risque de MCV, et d'ajouter des mesures contextuelles et de résultats supplémentaires pour le risque de MCV dans cette cohorte établie d'adultes portoricains, afin que nous mieux comprendre la dynamique de ces disparités.
Notre modèle suit le concept transdisciplinaire « des cellules à la société » développé conjointement avec nos partenaires CPHHD au cours de la période de financement initiale, en tenant compte de la variation génétique en relation avec le changement longitudinal de la charge allostatique et des indicateurs biochimiques de risque ; avec un accent supplémentaire sur les réseaux sociaux, les caractéristiques du quartier (espace physique et accès à la nourriture) et les facteurs environnementaux (pollution de l'air) en tant que déterminants sociaux de la santé.
Enfin, en utilisant des techniques participatives communautaires, nous mettrons en œuvre et testerons une intervention multidimensionnelle qui se concentre sur l'alimentation et l'exercice, mais qui tient également pleinement compte de l'environnement social et physique pour assurer le succès.
Avec la participation de nos partenaires communautaires et le soutien de nos noyaux administratifs, biostatistiques et de laboratoire, notre équipe est efficacement prête à apporter des contributions significatives à la compréhension des facteurs qui contribuent à la menace croissante apparente de maladie cardiaque dans ce groupe très défavorisé - tout en fournissant informations qui peuvent être utiles à d'autres groupes vulnérables.
La poursuite de notre cohorte, avec sa riche constellation de mesures, nous permettra de démêler certaines des interactions étiologiques complexes qui contribuent au risque de MCV, afin que des interventions efficaces puissent être mises en œuvre.
À ce jour, nous avons réalisé environ 840 entretiens de suivi sur 5 ans.
