Los adultos puertorriqueños que viven en los Estados Unidos continentales tienen disparidades de salud documentadas; sin embargo, se han realizado pocas investigaciones con este segundo subgrupo hispano más grande.
Nuestro objetivo a largo plazo es comprender las complejas interacciones de la dieta y otros factores conductuales y ambientales, la genética y el estrés psicosocial en la alta y aparentemente creciente prevalencia de factores de riesgo de enfermedades cardiovasculares (ECV) en adultos puertorriqueños.
Dado que este grupo está creciendo rápidamente, comprender las razones de este riesgo es de gran importancia.
Nuestro período de financiación inicial para el Centro Puertorriqueño de Salud de la Población y Disparidades de Salud de Boston (BPR-CPHHD, 2003-08) se centró en el papel del estrés en la discapacidad física y el deterioro cognitivo, a través de la desregulación fisiológica o "carga alostática".
Durante esa investigación, quedó claro que los factores de riesgo de CVD eran muy frecuentes.
Estos hallazgos contrastan con la creencia común de que existe una paradoja hispana: menos enfermedades cardíacas y mortalidad a pesar de una mayor pobreza.
Es importante destacar que la población puertorriqueña difiere considerablemente en la historia genética ancestral y en la exposición a factores de riesgo conocidos de otros grupos hispanos.
Tienen patrones de ingesta dietética únicos, así como estructuras sociales, culturales y ambientales que contribuyen y afectan la reacción a los factores estresantes.
Durante nuestro período de financiación inicial, reunimos con éxito una cohorte de 1500 adultos puertorriqueños, con edades iniciales de 45 a 75 años, y completamos entrevistas de seguimiento de dos años con más de 1250 participantes.
A medida que esta población está envejeciendo y creciendo rápidamente, la alta prevalencia y el aparente efecto de cohorte del aumento de los factores de riesgo de enfermedad cardíaca sugieren que 1) esta población tiene serias disparidades de salud en los factores de riesgo de enfermedad cardíaca y 2) la enfermedad cardíaca se convertirá en un problema aún mayor para esta población. grupo en un futuro próximo.
Nuestro objetivo general para esta renovación es, por lo tanto, extender el seguimiento y medir y analizar las características relevantes y los factores de riesgo de CVD, y agregar medidas contextuales y de resultado adicionales para el riesgo de CVD en esta cohorte establecida de adultos puertorriqueños, de modo que podamos pueden comprender mejor la dinámica de estas disparidades.
Nuestro modelo sigue el concepto transdisciplinario de "células para la sociedad" desarrollado en conjunto con nuestros socios CPHHD durante el período de financiación inicial, considerando la variación genética en relación con el cambio longitudinal en la carga alostática y los indicadores bioquímicos de riesgo; con un enfoque adicional en las redes sociales, las características del vecindario (espacio físico y acceso a los alimentos) y los factores ambientales (contaminación del aire) como determinantes sociales de la salud.
Finalmente, utilizando técnicas participativas basadas en la comunidad, implementaremos y probaremos una intervención multidimensional que se centre en la dieta y el ejercicio, pero que también considere completamente el entorno social y físico para asegurar el éxito.
Con la participación de nuestros socios comunitarios y el apoyo de nuestros núcleos administrativos, bioestadísticos y de laboratorio, nuestro equipo está preparado de manera eficiente para hacer contribuciones significativas a la comprensión de los factores que contribuyen a la aparente amenaza creciente de enfermedades cardíacas en este grupo altamente desfavorecido, al tiempo que proporciona ideas que pueden ser útiles para otros grupos vulnerables.
La continuación de nuestra cohorte, con su rica constelación de medidas, nos permitirá desentrañar algunas de las complejas interacciones etiológicas que contribuyen al riesgo de ECV, de modo que se puedan implementar intervenciones efectivas.
Hasta la fecha, hemos completado aproximadamente 840 entrevistas de seguimiento de 5 años.
