Dorośli Portorykańczycy mieszkający w kontynentalnej części Stanów Zjednoczonych mają udokumentowane różnice zdrowotne; jednak przeprowadzono niewiele badań z tą drugą co do wielkości podgrupą Latynosów.
Naszym długoterminowym celem jest zrozumienie złożonych interakcji diety i innych czynników behawioralnych i środowiskowych, genetyki i stresu psychospołecznego na wysoką i najwyraźniej rosnącą częstość występowania czynników ryzyka chorób sercowo-naczyniowych (CVD) u dorosłych z Puerto Rico.
Ponieważ ta grupa szybko rośnie, zrozumienie przyczyn tego ryzyka ma ogromne znaczenie.
Nasz początkowy okres finansowania dla Bostońskiego Portorykańskiego Centrum ds. Zdrowia Populacji i Rozbieżności w Zdrowiu (BPR-CPHHD, 2003-08) koncentrował się na roli stresu w niepełnosprawności fizycznej i pogorszeniu funkcji poznawczych, poprzez dysregulację fizjologiczną lub „obciążenie allostatyczne”.
Podczas tego dochodzenia stało się jasne, że czynniki ryzyka CVD są bardzo rozpowszechnione.
Odkrycia te stoją w sprzeczności z powszechnie panującym przekonaniem, że istnieje latynoski paradoks – mniejsza liczba chorób serca i śmiertelność pomimo większego ubóstwa.
Co ważne, populacja portorykańska różni się znacznie pod względem historii genetycznej przodków i narażenia na znane czynniki ryzyka z innych grup Latynosów.
Mają unikalne wzorce żywieniowe, a także struktury społeczne, kulturowe i środowiskowe, które przyczyniają się i wpływają na reakcję na stresory.
W początkowym okresie finansowania z powodzeniem zebraliśmy kohortę 1500 dorosłych Portorykańczyków w wieku wyjściowym 45-75 lat i przeprowadziliśmy dwuletnie wywiady uzupełniające z ponad 1250 uczestnikami.
Ponieważ populacja ta starzeje się i szybko rośnie, wysoka częstość występowania i widoczny efekt kohortowy zwiększonych czynników ryzyka chorób serca sugerują, że 1) w tej populacji występują poważne różnice zdrowotne w zakresie czynników ryzyka chorób serca oraz 2) choroby serca staną się jeszcze większym problemem dla tej populacji grupa w niedalekiej przyszłości.
Naszym ogólnym celem dla tego odnowienia jest zatem rozszerzenie obserwacji oraz pomiar i analiza istotnych cech i czynników ryzyka CVD, a także dodanie dodatkowych miar kontekstowych i wynikowych dla ryzyka CVD w tej ustalonej kohorcie dorosłych Portorykańczyków, tak abyśmy mogli lepiej zrozumieć dynamikę tych rozbieżności.
Nasz model jest zgodny z transdyscyplinarną koncepcją „z komórek do społeczeństwa” opracowaną wspólnie z naszymi partnerami CPHHD w początkowym okresie finansowania, z uwzględnieniem zmienności genetycznej w odniesieniu do podłużnej zmiany obciążenia allostatycznego i biochemicznych wskaźników ryzyka; z dodatkowym naciskiem na sieci społecznościowe, cechy sąsiedztwa (przestrzeń fizyczna i dostęp do żywności) oraz czynniki środowiskowe (zanieczyszczenie powietrza) jako społeczne determinanty zdrowia.
Wreszcie, stosując techniki partycypacyjne oparte na społeczności, wdrożymy i przetestujemy wielowymiarową interwencję, która koncentruje się na diecie i ćwiczeniach, ale w pełni uwzględnia również środowisko społeczne i fizyczne, aby zapewnić sukces.
Przy udziale naszych partnerów społecznych oraz przy wsparciu naszych rdzeni administracyjnych, biostatystycznych i laboratoryjnych, nasz zespół jest skutecznie przygotowany do wniesienia znaczącego wkładu w zrozumienie czynników, które przyczyniają się do widocznego rosnącego zagrożenia chorobami serca w tej wysoce niekorzystnej grupie – zapewniając jednocześnie spostrzeżeń, które mogą być przydatne dla innych wrażliwych grup.
Kontynuacja naszej kohorty, z jej bogatą konstelacją miar, pozwoli nam rozwikłać niektóre złożone interakcje etiologiczne, które przyczyniają się do ryzyka CVD, tak aby można było wdrożyć skuteczne interwencje.
Do tej pory przeprowadziliśmy około 840 5-letnich wywiadów uzupełniających.
