Adultos porto-riquenhos que vivem nos EUA continentais documentaram disparidades de saúde; no entanto, pouca pesquisa foi realizada com este segundo maior subgrupo hispânico.
Nosso objetivo de longo prazo é entender as complexas interações da dieta e outros fatores comportamentais e ambientais, genética e estresse psicossocial na alta e aparentemente crescente prevalência de fatores de risco para doenças cardiovasculares (DCV) em adultos porto-riquenhos.
Como esse grupo está crescendo rapidamente, entender as razões desse risco é de grande importância.
Nosso período inicial de financiamento para o Boston Puerto Rican Center for Population Health and Health Disparities (BPR-CPHHD, 2003-08) concentrou-se no papel do estresse na incapacidade física e no declínio cognitivo, por meio de desregulação fisiológica ou "carga alostática".
Durante essa investigação, ficou claro que os fatores de risco para DCV eram altamente prevalentes.
Essas descobertas contrastam com a crença comum de que existe um paradoxo hispânico - menor mortalidade e doenças cardíacas, apesar de maior pobreza.
É importante ressaltar que a população porto-riquenha difere consideravelmente na história genética ancestral e na exposição a fatores de risco conhecidos de outros grupos hispânicos.
Eles têm padrões únicos de ingestão alimentar, bem como estruturas sociais, culturais e ambientais que contribuem e afetam a reação aos estressores.
Durante nosso período inicial de financiamento, reunimos com sucesso uma coorte de 1.500 adultos porto-riquenhos, com idades entre 45 e 75 anos no início do estudo, e completamos entrevistas de acompanhamento de dois anos com mais de 1.250 participantes.
Como essa população está envelhecendo e crescendo rapidamente, a alta prevalência e o aparente efeito de coorte do aumento dos fatores de risco para doenças cardíacas sugerem que 1) essa população tem sérias disparidades de saúde nos fatores de risco para doenças cardíacas e 2) as doenças cardíacas se tornarão um problema ainda maior para essa população. grupo em um futuro próximo.
Nosso objetivo geral para esta renovação é, portanto, estender o acompanhamento e medir e analisar características relevantes e fatores de risco de DCV, e adicionar medidas contextuais e de resultados adicionais para o risco de DCV nesta coorte estabelecida de adultos porto-riquenhos, para que possamos podem entender melhor a dinâmica dessas disparidades.
Nosso modelo segue o conceito transdisciplinar de "células para a sociedade" desenvolvido em conjunto com nossos CPHHDs parceiros durante o período inicial de financiamento, considerando a variação genética em relação à mudança longitudinal na carga alostática e indicadores bioquímicos de risco; com foco adicional nas redes sociais, características do bairro (espaço físico e acesso a alimentos) e fatores ambientais (poluição do ar) como determinantes sociais da saúde.
Finalmente, usando técnicas participativas baseadas na comunidade, iremos implementar e testar uma intervenção multidimensional que se concentra na dieta e exercício, mas que também considera plenamente o ambiente social e físico para garantir o sucesso.
Com a participação de nossos parceiros comunitários e o apoio de nossos núcleos administrativos, bioestatísticos e laboratoriais, nossa equipe está preparada para fazer contribuições significativas para a compreensão dos fatores que contribuem para a aparente ameaça crescente de doenças cardíacas neste grupo altamente desfavorecido - enquanto fornece insights que podem ser úteis para outros grupos vulneráveis.
A continuação de nossa coorte, com sua rica constelação de medidas, nos permitirá desvendar algumas das complexas interações etiológicas que contribuem para o risco de DCV, para que intervenções eficazes possam ser implementadas.
Até o momento, completamos aproximadamente 840 entrevistas de acompanhamento de 5 anos.
