Détection, éducation, recherche et décolonisation sans isolement dans les établissements de soins de longue durée pour contrôler (DERAIL) SARM
Notre hypothèse pour le programme DERAIL MRSA est que l'on peut éliminer en toute sécurité le risque de colonisation de presque tous les résidents (patients) d'une manière qui n'interfère pas avec le mode de vie souhaité pour les personnes dans ces établissements et ainsi réduire le risque d'infection et réduire le coût des soins en évitant les maladies évitables.

Description détaillée
Le critère d'évaluation principal mesurera la réduction de la prévalence de la colonisation par le SARM (taux de pourcentage) dans le groupe d'intervention par rapport au groupe témoin.
Un critère d'évaluation secondaire consiste à comparer le taux de maladie à SARM (nombre d'infections pour 1 000 patients et 10 000 jours-patients) dans les bras d'intervention et de contrôle.
L'autre critère d'évaluation secondaire consiste à déterminer le coût du dépistage et de la décolonisation du SARM et à le comparer au coût du traitement de la maladie à SARM (à la fois le coût des médicaments et les frais de toute hospitalisation nécessaire pour le traitement de l'infection à SARM) dans le groupe témoin.
La réalisation de notre objectif principal de contrôle du SARM démontrera également deux objectifs secondaires que les chercheurs visent : l'un étant qu'une approche scientifique et planifiée de la question des infections nosocomiales spécifiques dans les établissements de soins de longue durée peut résoudre ces problèmes et l'autre qu'un partenariat entre les et les soins de longue durée (par exemple, une relation continue s'étendant sur au moins 5 ans) sont bénéfiques pour gérer la sécurité des patients et les pratiques de qualité dans l'ensemble du continuum des soins de santé aux États-Unis.
