Detección, educación, investigación y descolonización sin aislamiento en cuidados a largo plazo para controlar (DERAIL) MRSA
Nuestra hipótesis para el programa DERAIL MRSA es que se puede eliminar de manera segura el riesgo de colonización de casi todos los residentes (pacientes) de una manera que no interfiere con el estilo de vida deseado para las personas en estas instalaciones y, por lo tanto, reduce el riesgo de infección y reduce el costo de la atención al evitar enfermedades prevenibles.

Descripción detallada
El criterio principal de valoración será medir la reducción en la prevalencia de colonización de MRSA (tasa porcentual) en el brazo de intervención en comparación con el brazo de control.
Un criterio de valoración secundario es comparar la tasa de enfermedad por MRSA (número de infecciones por 1000 pacientes y 10 000 días-paciente) en los brazos de intervención y control.
El otro criterio de valoración secundario es determinar el costo de la detección y descolonización de MRSA y compararlo con el costo del tratamiento de la enfermedad de MRSA (tanto el costo de la medicación como el gasto de cualquier hospitalización necesaria para el tratamiento de la infección por MRSA) en el brazo de control.
Alcanzar nuestro objetivo principal de control de MRSA también demostrará dos objetivos secundarios que los investigadores pretenden lograr: uno es que un enfoque científico y planificado del problema de la infección asociada a la atención médica específica en LTCF puede resolver estos problemas y el otro que una asociación entre pacientes agudos y la atención a largo plazo (p. ej., una relación continua que dure al menos 5 años) es beneficiosa para tratar la seguridad del paciente y las prácticas de calidad en todo el continuo de atención médica de los Estados Unidos.
