Detectie, onderwijs, onderzoek en dekolonisatie zonder isolatie in langdurige zorg om (DERAIL) MRSA onder controle te krijgen
Onze hypothese voor het DERAIL MRSA-programma is dat men het kolonisatierisico van bijna alle bewoners (patiënten) veilig kan wegnemen op een manier die de gewenste levensstijl van personen in deze instellingen niet verstoort en daardoor het risico op infectie vermindert en de de kosten van de zorg door vermijdbare ziekten te vermijden.

Het primaire eindpunt is het meten van de vermindering van de prevalentie van MRSA-kolonisatie (percentage) in de interventie-arm in vergelijking met de controle-arm.
Een secundair eindpunt is het vergelijken van het percentage MRSA-ziekte (aantal infecties per 1.000 patiënten en 10.000 patiëntdagen) in de interventie- en controlearmen.
Het andere secundaire eindpunt is het bepalen van de kosten van MRSA-screening en dekolonisatie en deze te vergelijken met de kosten van de behandeling van de MRSA-ziekte (zowel medicatiekosten als de kosten van eventuele ziekenhuisopname voor therapie of MRSA-infectietherapie) in de controle-arm.
Het bereiken van ons primaire doel van MRSA-bestrijding zal ook twee secundaire doelen aantonen die de onderzoekers nastreven: het ene is dat een wetenschappelijke, geplande aanpak van het probleem van specifieke zorggerelateerde infectie bij LTCF's deze problemen kan oplossen en het andere dat een samenwerking tussen acute en langdurige zorg (bijv. aanhoudende relatie van ten minste 5 jaar) is gunstig voor het omgaan met patiëntveiligheid en kwaliteitspraktijken in het hele zorgcontinuüm van de Verenigde Staten.
