Étude multicentrique randomisée de phase II sur le trastuzumab plus docétaxel néoadjuvant avec et sans bévacizumab et le trastuzumab plus docétaxel plus doxorubicine encapsulée dans des liposomes non pégylés (NPLD) avec et sans bévacizumab dans le cancer du sein précoce HER2-positif

Description détaillée
La population cible de l'étude est constituée de patients masculins et féminins pré- et post-ménopausés atteints d'un adénocarcinome du sein HER2 positif (hors cancer inflammatoire du sein, T4d) devant recevoir un traitement cytotoxique néoadjuvant.
Les patientes doivent avoir un cancer du sein pathologiquement confirmé avec une surexpression de HER2 confirmée histologiquement. Lors du dépistage, les patients doivent avoir une fraction d'éjection ventriculaire gauche (FEVG) adéquate ; un statut de performance ECOG de 0 ou 1 ; fonction hépatique, rénale et médullaire adéquate; et être exempt d'autres maladies graves qui pourraient affecter la conformité au protocole ou l'interprétation des résultats.
Les patients ne doivent pas être exposés à un risque accru de perforation gastro-intestinale, d'hypertension, de protéinurie, de complications de cicatrisation, de thromboembolie ou d'hémorragie. Les patients ne doivent pas avoir eu d'autre tumeur maligne primaire qui pourrait affecter le respect du protocole ou l'interprétation des résultats. Les patients présentant des métastases du système nerveux central (SNC) sont exclus. Les femelles gestantes ou allaitantes sont exclues. Les patients souffrant d'hypertension (> 150 mmHG systolique ou >100 mmHG diastolique) et les patients ayant des antécédents de perforation gastro-intestinale, de fistule abdominale ou d'abcès intra-abdominal dans les 6 mois suivant l'entrée dans l'étude sont exclus.
Un traitement anticoagulant complet à l'entrée dans l'étude est autorisé tant que le patient a un niveau stable d'anticoagulants depuis au moins 2 semaines au moment du début du traitement de l'étude.

Critère d'intégration:
- Femme ou homme, âge ≥ 18 ans
- Adénocarcinome mammaire primitif invasif confirmé pathologiquement (sauf cancer du sein inflammatoire, T4d) programmé pour un traitement systémique néoadjuvant contenant du taxane avec/sans ganglions lymphatiques palpables.
- Surexpression documentée de la protéine HER2, déterminée par immunohistochimie (IHC) 3+ ou par amplification démontrée du gène HER2/c-erbB2 de la tumeur primaire par un laboratoire local.
- FEVG ≥ 55 % mesurée par échocardiographie ou MUGA dans les 4 semaines précédant la randomisation
- Statut de performance ECOG ≤ 1
- Capable et disposé à se conformer aux visites prévues, aux plans de traitement, aux tests de laboratoire et aux autres procédures d'étude.
- Consentement éclairé écrit
Critère d'exclusion:
Traitement actuel
- Nécessité d'une utilisation concomitante de l'agent antiviral sorivudine ou d'analogues chimiquement apparentés, tels que la brivudine.
- Traitement quotidien chronique par corticostéroïdes excl. stéroïdes inhalés.
- Traitement quotidien chronique par aspirine et analogues de l'aspirine ou clopidogrel
- Intervention chirurgicale majeure, biopsie ouverte ou lésion traumatique importante dans les 28 jours précédant la randomisation ou anticipation de la nécessité d'une intervention chirurgicale majeure au cours du traitement à l'étude
- Traitement actuel ou récent (dans les 30 jours précédant la randomisation) avec un autre médicament expérimental ou participation à une autre étude expérimentale.
Laboratoire
- Fonction inadéquate de la moelle osseuse
- Fonction hépatique inadéquate
- Fonction rénale inadéquate
- Patients ne recevant pas d'anticoagulants et ayant activé le temps de thromboplastine partielle (aPTT) dans les 7 jours précédant le jour 1 du cycle 1.
Conditions concomitantes
- Autre tumeur maligne au cours des 5 dernières années avant la randomisation, à l'exception d'un carcinome in situ du col de l'utérus traité curativement ou d'un cancer de la peau non mélanomateux
- Preuve de métastases à distance jugée cliniquement et au moins par radiographie pulmonaire, échographie hépatique et scintigraphie osseuse. En cas de suspicion clinique de métastases cérébrales, un scanner ou une IRM du cerveau doit être réalisé dans les 4 semaines précédant la randomisation.
Maladie concomitante grave pouvant affecter le respect du protocole ou l'interprétation des résultats, y compris, mais sans s'y limiter :
- Infection active nécessitant i.v. antibiotiques
- Hypertension non contrôlée
- Antécédents cliniquement significatifs de maladie cardiovasculaire tels qu'indiqués par : accident vasculaire cérébral ou accident vasculaire cérébral ; infarctus du myocarde; une angine instable; NYHA Grade II ou CHF supérieur ; arythmie cardiaque nécessitant des médicaments; cardiopathie valvulaire cliniquement significative.
- Dyspnée de repos nécessitant une oxygénothérapie de soutien ou avec épanchements pleuraux importants
- Diabète mal contrôlé
- Antécédents ou preuves à l'examen physique/neurologique d'une maladie du SNC non liée au cancer (par ex. convulsions incontrôlées) à moins d'être traité de manière adéquate avec un traitement médical standard
- Antécédents ou signes de diathèse hémorragique héréditaire ou de coagulopathie avec risque de saignement
- Antécédents de fistule abdominale, de perforation gastro-intestinale ou d'abcès intra-abdominal dans les 6 mois suivant la randomisation
- Plaie grave ne cicatrisant pas, ulcère peptique ou fracture osseuse
- Syndrome de malabsorption cliniquement significatif, colite ulcéreuse, maladie affectant la fonction gastro-intestinale, résection de l'estomac ou de l'intestin grêle ou incapacité à prendre des médicaments par voie orale
- Hypokaliémie ou hypomagnésémie non corrigée
- Allogreffes d'organes nécessitant un traitement immunosuppresseur
- Preuve de toute autre maladie, dysfonctionnement métabolique ou psychologique, résultat d'examen physique ou résultat de laboratoire clinique donnant une suspicion raisonnable d'une maladie ou d'un état qui contre-indique l'utilisation d'un médicament expérimental, peut affecter l'observance du patient avec les routines d'étude ou placer le patient à haute risque de complications liées au traitement.
- Hypersensibilité connue à l'un des médicaments/excipients à l'étude.
- Hypersensibilité aux produits de cellules ovariennes de hamster chinois ou à d'autres anticorps recombinants humains ou humanisés.

- Femmes enceintes, allaitantes ou femmes en âge de procréer sans test de grossesse négatif
- Hommes ou femmes fertiles en âge de procréer
- Patients non accessibles pour traitement ou suivi
