Gedetailleerde beschrijving
De beoogde studiepopulatie bestaat uit mannelijke en vrouwelijke pre- en postmenopauzale patiënten met HER2-positief adenocarcinoom van de borst (behalve inflammatoire borstkanker, T4d) die gepland staan voor neoadjuvante cytotoxische behandeling.
Patiënten moeten pathologisch bevestigde borstkanker hebben met histologisch bevestigde HER2-overexpressie. Bij screening moeten patiënten een adequate linkerventrikelejectiefractie (LVEF) hebben; een ECOG-prestatiestatus van 0 of 1; adequate lever-, nier- en beenmergfunctie; en vrij zijn van andere ernstige ziekten die de naleving van het protocol of de interpretatie van resultaten kunnen beïnvloeden.
Patiënten mogen geen verhoogd risico lopen op gastro-intestinale perforatie, hypertensie, proteïnurie, complicaties bij wondgenezing, trombo-embolie of bloeding. Patiënten mogen geen andere primaire maligniteit hebben gehad die de naleving van het protocol of de interpretatie van de resultaten zou kunnen beïnvloeden. Patiënten met metastasen van het centrale zenuwstelsel (CZS) zijn uitgesloten. Zwangere of zogende vrouwtjes zijn uitgesloten. Patiënten met hypertensie (>150 mmHG systolisch of >100 mmHG diastolisch) en patiënten met een voorgeschiedenis van gastro-intestinale perforatie, abdominale fistel of intra-abdominaal abces binnen 6 maanden na deelname aan het onderzoek zijn uitgesloten.
Volledige antistollingstherapie bij aanvang van de studie is toegestaan zolang de patiënt op het moment van aanvang van de studiebehandeling gedurende ten minste 2 weken op een stabiel niveau van anticoagulantia is geweest.

Beschrijving
Inclusiecriteria:
- Vrouw of man, leeftijd ≥ 18 jaar
- Pathologisch bevestigd invasief primair borstadenocarcinoom (behalve inflammatoire borstkanker, T4d) gepland voor taxaanbevattende neoadjuvante systemische behandeling met/zonder voelbare lymfeklieren.
- Gedocumenteerde overexpressie van HER2-eiwit zoals bepaald door immunohistochemie (IHC) 3+ of door aangetoonde HER2/c-erbB2-genamplificatie van de primaire tumor door een lokaal laboratorium.
- LVEF ≥ 55% gemeten door middel van echocardiografie of MUGA binnen 4 weken vóór randomisatie
- ECOG-prestatiestatus ≤ 1
- In staat en bereid om te voldoen aan geplande bezoeken, behandelplannen, laboratoriumtests en andere onderzoeksprocedures.
- Schriftelijke geïnformeerde toestemming
Uitsluitingscriteria:
Huidige behandeling
- Vereiste voor gelijktijdig gebruik van het antivirale middel sorivudine of chemisch verwante analogen, zoals brivudine.
- Chronische dagelijkse behandeling met corticosteroïden excl. geïnhaleerde steroïden.
- Chronische dagelijkse behandeling met aspirine en aspirine-analogen of clopidogrel
- Grote chirurgische ingreep, open biopsie of significant traumatisch letsel binnen 28 dagen voorafgaand aan randomisatie of anticipatie op de noodzaak van een grote operatie tijdens de studiebehandeling
- Huidige of recente (binnen 30 dagen voorafgaand aan randomisatie) behandeling met een ander onderzoeksgeneesmiddel of deelname aan een andere onderzoeksstudie.
Laboratorium
- Ontoereikende beenmergfunctie
- Onvoldoende leverfunctie
- Onvoldoende nierfunctie
- Patiënten die geen antistollingsmedicatie krijgen en die een geactiveerde partiële tromboplastinetijd (aPTT) hebben binnen 7 dagen voorafgaand aan dag 1 van cyclus 1.
Gelijktijdige voorwaarden
- Andere maligniteiten in de laatste 5 jaar vóór randomisatie behalve curatief behandeld carcinoma in situ van de cervix of niet-melanomateuze huidkanker
- Bewijs van metastase op afstand beoordeeld klinisch en ten minste door middel van röntgenfoto's van de borst, echografie van de lever en botscan. Bij een klinisch vermoeden van hersenmetastasen dient binnen 4 weken voorafgaand aan randomisatie een CT-scan of MRI van de hersenen te worden gemaakt.
Ernstige gelijktijdige ziekte die de naleving van het protocol of de interpretatie van resultaten kan beïnvloeden, inclusief maar niet beperkt tot:
- Actieve infectie waarvoor i.v. antibiotica
- Ongecontroleerde hypertensie
- Klinisch significante voorgeschiedenis van hart- en vaatziekten zoals blijkt uit: cerebrovasculair accident of beroerte; hartinfarct; instabiele angina; NYHA Graad II of hoger CHF; hartritmestoornissen waarvoor medicatie nodig is; klinisch significante hartklepaandoening.
- Dyspnoe in rust die ondersteunende zuurstoftherapie noodzakelijk maakt of met significante pleurale effusies
- Slecht gecontroleerde diabetes mellitus
- Voorgeschiedenis of bewijs bij lichamelijk/neurologisch onderzoek van CZS-aandoening die geen verband houdt met kanker (bijv. ongecontroleerde aanvallen), tenzij adequaat behandeld met standaard medische therapie
- Geschiedenis of bewijs van erfelijke bloedingsdiathese of coagulopathie met het risico op bloedingen
- Geschiedenis van abdominale fistel, GI-perforatie of intra-abdominaal abces binnen 6 maanden na randomisatie
- Ernstige niet-genezende wond, maagzweer of botbreuk
- Klinisch significant malabsorptiesyndroom, colitis ulcerosa, ziekte die de gastro-intestinale functie beïnvloedt, resectie van de maag of dunne darm, of onvermogen om orale medicatie in te nemen
- Ongecorrigeerde hypokaliëmie of hypomagnesiëmie
- Orgaantransplantaten die immunosuppressieve therapie vereisen
- Bewijs van een andere ziekte, metabole of psychologische disfunctie, bevindingen bij lichamelijk onderzoek of klinische laboratoriumresultaten die een redelijk vermoeden geven van een ziekte of aandoening die een contra-indicatie vormt voor het gebruik van een onderzoeksgeneesmiddel, kunnen van invloed zijn op de naleving van onderzoeksroutines door de patiënt of kunnen de patiënt in een hoge positie brengen. risico op behandelingsgerelateerde complicaties.
- Bekende overgevoeligheid voor een van de onderzoeksgeneesmiddelen/hulpstoffen.
- Overgevoeligheid voor eierstokcelproducten van Chinese hamsters of andere recombinante humane of gehumaniseerde antilichamen.
Ander
- Zwangere, zogende vrouwen of vrouwen die zwanger kunnen worden zonder een negatieve zwangerschapstest
- Vruchtbare mannetjes of vrouwtjes die zwanger kunnen worden
- Patiënten niet bereikbaar voor behandeling of follow-up
