A megcélzott vizsgálati populáció HER2-pozitív emlőadenokarcinómában (kivéve gyulladásos emlőrák, T4d) szenvedő, neoadjuváns citotoxikus kezelésben részesülő, pre- és posztmenopauzás férfiakból és nőkből áll.
A betegeknek patológiásan igazolt emlőrákban kell szenvedniük, szövettanilag igazolt HER2-túlexpresszióval. A szűréskor a betegeknek megfelelő bal kamrai ejekciós frakcióval (LVEF) kell rendelkezniük; az ECOG teljesítmény állapota 0 vagy 1; megfelelő máj-, vese- és csontvelőműködés; és mentesnek kell lennie más súlyos betegségektől, amelyek befolyásolhatják a protokollnak való megfelelést vagy az eredmények értelmezését.
A betegeknél nem lehet fokozott GI-perforáció, magas vérnyomás, proteinuria, sebgyógyulási szövődmények, thromboembolia vagy vérzés kockázata. A betegeknek nem lehet más elsődleges rosszindulatú daganata, amely befolyásolhatja a protokollnak való megfelelést vagy az eredmények értelmezését. A központi idegrendszeri (CNS) metasztázisokkal rendelkező betegek nem tartoznak ide. A terhes vagy szoptató nőstények nem tartoznak ide. A magas vérnyomásban szenvedő betegeket (>150 HG szisztolés vagy >100 HGmm diasztolés), valamint azokat a betegeket, akiknek a kórelőzményében GI-perforáció, hasi fisztula vagy intraabdominalis tályog szerepel a vizsgálatba való belépéstől számított 6 hónapon belül, kizárták.
A teljes véralvadásgátló terápia a vizsgálatba való belépéskor megengedett mindaddig, amíg a beteg a vizsgálati kezelés megkezdésekor legalább 2 hétig stabil antikoaguláns-szinten van.

Bevételi kritériumok:
- Nő vagy férfi, életkor ≥ 18 év
- Patológiailag igazolt invazív primer emlő adenokarcinóma (kivéve gyulladásos emlőrák, T4d) taxán tartalmú neoadjuváns szisztémás kezelésre tervezett tapintható nyirokcsomókkal vagy anélkül.
- Dokumentált HER2 fehérje túlzott expressziója immunhisztokémiával (IHC) 3+ vagy a primer tumor kimutatott HER2/c-erbB2 génamplifikációjával egy helyi laboratóriumban.
- LVEF ≥ 55% echokardiográfiával vagy MUGA-val mérve a randomizálás előtti 4 héten belül
- ECOG teljesítmény állapota ≤ 1
- Képes és hajlandó betartani a tervezett látogatásokat, kezelési terveket, laboratóriumi vizsgálatokat és egyéb vizsgálati eljárásokat.
- Írásbeli tájékoztatáson alapuló beleegyezés
Kizárási kritériumok:
Jelenlegi kezelés
- A szorivudin vírusellenes szer vagy kémiailag rokon analógjai, például a brivudin egyidejű alkalmazásának követelménye.
- Krónikus napi kezelés kortikoszteroidokkal, kivéve belélegzett szteroidok.
- Krónikus napi kezelés aszpirinnel és aszpirin analógokkal vagy klopidogrellel
- Jelentős műtéti beavatkozás, nyílt biopszia vagy jelentős traumás sérülés a randomizálást megelőző 28 napon belül, vagy a vizsgálati kezelés alatti jelentős műtét szükségességének előrejelzése
- Jelenlegi vagy közelmúltban (a randomizációt megelőző 30 napon belül) más vizsgált gyógyszerrel végzett kezelés vagy egy másik vizsgálati vizsgálatban való részvétel.
Laboratórium
- A csontvelő elégtelen működése
- Nem megfelelő májműködés
- Nem megfelelő veseműködés
- Olyan betegek, akik nem kapnak véralvadásgátló gyógyszert, és akiknél az 1. ciklus 1. napja előtt 7 napon belül aktiválódott a részleges tromboplasztin idő (aPTT).
Egyidejű állapotok
- Egyéb rosszindulatú daganatok a randomizáció előtti utolsó 5 évben, kivéve a gyógyítólag kezelt in situ méhnyakrákot vagy a nem melanomás bőrrákot
- Klinikailag és legalább mellkasröntgen-, máj-szonográfiával és csontvizsgálattal megítélt távoli metasztázis bizonyítéka. Ha az agyi metasztázis klinikai gyanúja merül fel, a randomizálást megelőző 4 héten belül CT-vizsgálatot vagy agy MRI-t kell végezni.
Súlyos egyidejű betegség, amely befolyásolhatja a protokollnak való megfelelést vagy az eredmények értelmezését, beleértve, de nem kizárólagosan:
- Aktív fertőzés, amely i.v. antibiotikumok
- Nem kontrollált magas vérnyomás

- Hasi fisztula, GI-perforáció vagy intraabdominalis tályog anamnézisében a randomizálást követő 6 hónapon belül
- Súlyos, nem gyógyuló seb, gyomorfekély vagy csonttörés
- Klinikailag jelentős felszívódási zavar szindróma, fekélyes vastagbélgyulladás, GI-működést befolyásoló betegség, gyomor vagy vékonybél reszekció vagy szájon át szedhető gyógyszeres kezelés képtelensége
- Nem korrigált hypokalaemia vagy hypomagnesemia
- Immunszuppresszív terápiát igénylő szervi allograftok
- Bármilyen más betegségre, anyagcsere- vagy pszichológiai zavarra, fizikális vizsgálati leletre vagy klinikai laboratóriumi leletre utaló bizonyíték, amely olyan betegség vagy állapot alapos gyanúját veti fel, amely ellenjavallt egy vizsgált gyógyszer alkalmazását, befolyásolhatja a beteget a vizsgálati rutinoknak való megfelelésben, vagy magas szintre helyezheti a beteget. kezeléssel összefüggő szövődmények kockázata.
- Ismert túlérzékenység bármely vizsgált gyógyszerrel/segédanyaggal szemben.
