Descrição detalhada
A população-alvo do estudo consiste em pacientes masculinos e femininos na pré e pós-menopausa com adenocarcinoma de mama HER2-positivo (exceto câncer de mama inflamatório, T4d) programados para receber tratamento citotóxico neoadjuvante.
Os pacientes devem ter câncer de mama confirmado patologicamente com superexpressão de HER2 confirmada histologicamente. Na triagem, os pacientes devem ter uma fração de ejeção do ventrículo esquerdo (FEVE) adequada; um status de desempenho ECOG de 0 ou 1; função hepática, renal e da medula óssea adequadas; e estar livre de outras doenças graves que possam afetar o cumprimento do protocolo ou a interpretação dos resultados.
Os pacientes não devem apresentar risco aumentado de perfuração gastrointestinal, hipertensão, proteinúria, complicações na cicatrização de feridas, tromboembolismo ou hemorragia. Os pacientes não devem ter outra neoplasia primária que possa afetar a adesão ao protocolo ou a interpretação dos resultados. Pacientes com metástases no sistema nervoso central (SNC) são excluídos. Mulheres grávidas ou lactantes são excluídas. Pacientes com hipertensão (>150 mmHG sistólica ou >100 mmHG diastólica) e pacientes com história de perfuração GI, fístula abdominal ou abscesso intra-abdominal dentro de 6 meses após a entrada no estudo são excluídos.
A terapia de anticoagulação total na entrada do estudo é permitida, desde que o paciente esteja em um nível estável de anticoagulantes por pelo menos 2 semanas no momento do início do tratamento do estudo.

Descrição
Critério de inclusão:
- Mulher ou homem, idade ≥ 18 anos
- Adenocarcinoma de mama invasivo primário patologicamente confirmado (exceto câncer de mama inflamatório, T4d) agendado para tratamento sistêmico neoadjuvante contendo taxano com/sem linfonodos palpáveis.
- Superexpressão da proteína HER2 documentada conforme determinado por imuno-histoquímica (IHC) 3+ ou por amplificação do gene HER2/c-erbB2 demonstrada do tumor primário por um laboratório local.
- FEVE ≥ 55% medido por ecocardiografia ou MUGA dentro de 4 semanas antes da randomização
- Status de desempenho ECOG ≤ 1
- Capaz e disposto a cumprir consultas agendadas, planos de tratamento, exames laboratoriais e outros procedimentos do estudo.
- Consentimento informado por escrito
Critério de exclusão:
Tratamento Atual
- Requisito para uso concomitante do agente antiviral sorivudina ou análogos quimicamente relacionados, como a brivudina.
- Tratamento diário crônico com corticosteróides excl. esteróides inalados.
- Tratamento diário crônico com aspirina e análogos de aspirina ou clopidogrel
- Procedimento cirúrgico de grande porte, biópsia aberta ou lesão traumática significativa dentro de 28 dias antes da randomização ou antecipação da necessidade de cirurgia de grande porte durante o curso do tratamento do estudo
- Tratamento atual ou recente (dentro de 30 dias antes da randomização) com outro medicamento experimental ou participação em outro estudo experimental.
Laboratório
- Função inadequada da medula óssea
- Função hepática inadequada
- Função renal inadequada
- Pacientes que não receberam medicação anticoagulante e que ativaram o tempo de tromboplastina parcial (aPTT) nos 7 dias anteriores ao Dia 1 do ciclo 1.
Condições Concomitantes
- Outra malignidade nos últimos 5 anos antes da randomização, exceto carcinoma in situ do colo do útero tratado curativamente ou câncer de pele não melanoma
- Evidência de metástase à distância julgada clinicamente e pelo menos por radiografia de tórax, ultrassonografia hepática e cintilografia óssea. Se houver qualquer suspeita clínica de metástase cerebral, uma tomografia computadorizada ou ressonância magnética do cérebro deve ser realizada dentro de 4 semanas antes da randomização.
Doença concomitante grave que pode afetar a conformidade com o protocolo ou a interpretação dos resultados, incluindo, entre outros:
- Infecção ativa requerendo administração i.v. antibióticos
- hipertensão descontrolada
- História clinicamente significativa de doença cardiovascular indicada por: acidente vascular cerebral ou acidente vascular cerebral; infarto do miocárdio; angina instável; NYHA Grau II ou maior ICC; arritmia cardíaca requerendo medicação; valvopatia clinicamente significativa.
- Dispneia em repouso necessitando de oxigenoterapia de suporte ou com derrames pleurais significativos
- Diabetes melito mal controlado
- Histórico ou evidência no exame físico/neurológico de doença do SNC não relacionada ao câncer (por exemplo, convulsões descontroladas), a menos que sejam adequadamente tratadas com terapia médica padrão
- Histórico ou evidência de diátese hemorrágica hereditária ou coagulopatia com risco de sangramento
- História de fístula abdominal, perfuração GI ou abscesso intra-abdominal dentro de 6 meses após a randomização
- Ferida grave que não cicatriza, úlcera péptica ou fratura óssea
- Síndrome de má absorção clinicamente significativa, colite ulcerativa, doença que afeta a função gastrointestinal, ressecção do estômago ou intestino delgado ou incapacidade de tomar medicação oral
- Hipocalemia ou hipomagnesemia não corrigida
- Aloenxertos de órgãos que requerem terapia imunossupressora
- Evidência de qualquer outra doença, disfunção metabólica ou psicológica, achado de exame físico ou achado de laboratório clínico que dê suspeita razoável de uma doença ou condição que contraindique o uso de um medicamento em investigação, pode afetar a adesão do paciente às rotinas do estudo ou colocar o paciente em alta risco de complicações relacionadas ao tratamento.
- Hipersensibilidade conhecida a qualquer um dos medicamentos/excipientes do estudo.
- Hipersensibilidade a produtos de células de ovário de hamster chinês ou outros anticorpos recombinantes humanos ou humanizados.

- Mulheres grávidas, lactantes ou mulheres com potencial para engravidar sem teste de gravidez negativo
- Machos férteis ou fêmeas com potencial para engravidar
- Pacientes não acessíveis para tratamento ou acompanhamento
