Descripción detallada
La población objetivo del estudio consta de pacientes premenopáusicos y posmenopáusicos masculinos y femeninos con adenocarcinoma de mama HER2 positivo (excepto cáncer de mama inflamatorio, T4d) programados para recibir tratamiento citotóxico neoadyuvante.
Los pacientes deben tener cáncer de mama confirmado patológicamente con sobreexpresión de HER2 confirmada histológicamente. En la selección, los pacientes deben tener una fracción de eyección del ventrículo izquierdo (FEVI) adecuada; un estado funcional ECOG de 0 o 1; función hepática, renal y de la médula ósea adecuada; y estar libre de otras enfermedades graves que puedan afectar el cumplimiento del protocolo o la interpretación de los resultados.
Los pacientes no deben correr un mayor riesgo de perforación gastrointestinal, hipertensión, proteinuria, complicaciones de cicatrización de heridas, tromboembolismo o hemorragia. Los pacientes no deben haber tenido otra neoplasia maligna primaria que pudiera afectar el cumplimiento del protocolo o la interpretación de los resultados. Se excluyen los pacientes con metástasis en el sistema nervioso central (SNC). Se excluyen las hembras gestantes o lactantes. Se excluyen los pacientes con hipertensión (>150 mmHG sistólica o >100 mmHG diastólica) y los pacientes con antecedentes de perforación GI, fístula abdominal o absceso intraabdominal dentro de los 6 meses anteriores al ingreso al estudio.
Se permite la terapia anticoagulante completa al ingresar al estudio siempre que el paciente haya tenido un nivel estable de anticoagulantes durante al menos 2 semanas en el momento del inicio del tratamiento del estudio.

Descripción
Criterios de inclusión:
- Mujer o hombre, edad ≥ 18 años
- Adenocarcinoma de mama primario invasivo confirmado patológicamente (excepto cáncer de mama inflamatorio, T4d) programado para tratamiento sistémico neoadyuvante que contiene taxanos con/sin ganglios linfáticos palpables.
- Sobreexpresión documentada de la proteína HER2 determinada por inmunohistoquímica (IHC) 3+ o por amplificación demostrada del gen HER2/c-erbB2 del tumor primario por un laboratorio local.
- FEVI ≥ 55% medida por ecocardiografía o MUGA dentro de las 4 semanas previas a la aleatorización
- Estado de rendimiento ECOG ≤ 1
- Capaz y dispuesto a cumplir con las visitas programadas, los planes de tratamiento, las pruebas de laboratorio y otros procedimientos del estudio.
- Consentimiento informado por escrito
Criterio de exclusión:
Tratamiento actual
- Requisito para el uso concurrente del agente antiviral sorivudina o análogos químicamente relacionados, como brivudina.
- Tratamiento crónico diario con corticosteroides excl. esteroides inhalados.
- Tratamiento crónico diario con aspirina y análogos de aspirina o clopidogrel
- Procedimiento quirúrgico mayor, biopsia abierta o lesión traumática importante dentro de los 28 días anteriores a la aleatorización o anticipación de la necesidad de una cirugía mayor durante el curso del tratamiento del estudio
- Tratamiento actual o reciente (dentro de los 30 días anteriores a la aleatorización) con otro fármaco en investigación o participación en otro estudio de investigación.
Laboratorio
- Función inadecuada de la médula ósea
- Función hepática inadecuada
- Función renal inadecuada
- Pacientes que no reciben medicación anticoagulante que tienen tiempo de tromboplastina parcial activado (aPTT) dentro de los 7 días anteriores al Día 1 del ciclo 1.
Condiciones concomitantes
- Otras neoplasias malignas en los últimos 5 años antes de la aleatorización, excepto carcinoma in situ de cuello uterino tratado curativamente o cáncer de piel no melanomatoso
- Evidencia de metástasis a distancia juzgada clínicamente y al menos por radiografía de tórax, ecografía hepática y gammagrafía ósea. Si existe alguna sospecha clínica de metástasis cerebral, se debe realizar una tomografía computarizada o una resonancia magnética del cerebro dentro de las 4 semanas anteriores a la aleatorización.
Enfermedad concurrente grave que podría afectar el cumplimiento del protocolo o la interpretación de los resultados, incluidas, entre otras:
- Infección activa que requiere i.v. antibióticos
- Hipertensión no controlada
- Antecedentes clínicamente significativos de enfermedad cardiovascular indicados por: accidente cerebrovascular o accidente cerebrovascular; infarto de miocardio; angina inestable; ICC de grado II de la NYHA o superior; arritmia cardíaca que requiere medicación; valvulopatía cardiaca clínicamente significativa.
- Disnea en reposo que requiere oxigenoterapia de apoyo o con derrames pleurales significativos
- Diabetes mellitus mal controlada
- Antecedentes o evidencia en el examen físico/neurológico de enfermedad del SNC no relacionada con el cáncer (p. ej., convulsiones no controladas) a menos que se trate adecuadamente con terapia médica estándar
- Antecedentes o evidencia de diátesis hemorrágica hereditaria o coagulopatía con riesgo de hemorragia
- Antecedentes de fístula abdominal, perforación GI o absceso intraabdominal dentro de los 6 meses posteriores a la aleatorización
- Herida grave que no cicatriza, úlcera péptica o fractura ósea
- Síndrome de malabsorción clínicamente significativo, colitis ulcerosa, enfermedad que afecta la función GI, resección del estómago o intestino delgado, o incapacidad para tomar medicamentos orales
- Hipopotasemia o hipomagnesemia no corregida
- Aloinjertos de órganos que requieren terapia inmunosupresora
- La evidencia de cualquier otra enfermedad, disfunción metabólica o psicológica, resultado del examen físico o resultado del laboratorio clínico que dé una sospecha razonable de una enfermedad o condición que contraindique el uso de un fármaco en investigación, puede afectar el cumplimiento del paciente con las rutinas del estudio o colocar al paciente en riesgo de complicaciones relacionadas con el tratamiento.
- Hipersensibilidad conocida a cualquiera de los fármacos/excipientes del estudio.
- Hipersensibilidad a productos de células de ovario de hámster chino u otros anticuerpos recombinantes humanos o humanizados.

- Pacientes no accesibles para tratamiento o seguimiento.
