Cerveau à l'effort : effets de l'hypoxie chez les patients respiratoires (NEUROX)
L'oxygénation du cerveau est déterminée par le produit du CaO2 et du débit sanguin cérébral (CBF), la modification de l'un ou de l'autre peut affecter la disponibilité neuronale en O2.
Outre l'effet du PaO2, le CBF est également régulé par le PaCO2. Lors d'un effort en état d'hypoxie, la baisse de la PaO2 associée à une baisse potentielle de la PaCO2 et donc du DSC, peut créer un important vertige entre la demande et l'offre d'O2 cérébral.
Il semble que l'hypoxie puisse perturber de manière significative la réponse des neurones centraux, tout comme la production d'un cortex moteur génère des commandes motrices.
Des études suggèrent que l'exercice dans des conditions d'hypoxie sévère peut constituer une menace nécessaire pour l'oxygénation du cerveau et la commande motrice, avec pour conséquence une diminution de la performance de l'exercice.
Ce projet vise à étudier les effets de l'hypoxie sur le fonctionnement cérébral de patients souffrant de maladies respiratoires chroniques. Réponses neurophysiologiques du cerveau au repos ou à l'exercice, y compris le goutte-à-goutte et l'oxygénation cérébrale, l'excitation corticale et la commande motrice résultant pour les sujets hypoxiques avant et après un traitement pour corriger les anomalies des gaz dans le sang.
L'étude utilisera une approche méthodologique pluridisciplinaire et complémentaire : la spectroscopie proche infrarouge (NIRS) pour apprécier le goutte-à-goutte et l'oxygénation cérébrale, la DSC, les procédures de neurostimulation et l'électromyographie (EMG) pour apprécier l'excitabilité corticale, mesurer le niveau d'activation centrale et motrice commande.
Les objectifs de cette étude seront :
- Mesurer le goutte-à-goutte et l'oxygénation cérébrale, l'excitabilité corticale, les mécanismes d'activation volontaire et la fatigue centrale à l'effort chez le patient hypoxémique chronique par rapport à des sujets témoins sains.
- Analyser les perturbations des paramètres de locomotion et de la posturographiec, en tâche simple et double, impliquant différents niveaux de tâche cérébrale.
- Analyser les effets aigus d'une amélioration de l'oxygénation artérielle des patients atteints de bronchopneumopathie chronique obstructive (BPCO) sur le goutte-à-goutte et l'oxygénation cérébrale, l'excitabilité corticale, les mécanismes d'activation volontaire et la fatigue centrale.
- Évaluer les effets d'un traitement par pression positive continue (PPC) chez des patients souffrant d'apnée obstructive du sommeil (AOS) avec les mêmes paramètres.

Patients atteints de MPOC
- stades GOLD III - IV, rapport VEMS/CVF < 0,7 et ventilation maximale volontaire (MVV) < 50 % des valeurs prédites
- IMC < 30 kg/m²
- Âge entre 18 et 80 ans
- Non-fumeur ou ex-fumeur (arrêt depuis plus de 3 mois)
- État stable depuis plus de 3 mois
- PaCO2 < 45 mmHg au repos à l'air ambiant
- Pas de diagnostic OSA
Patients atteints d'AOS
- AOS sévère récemment diagnostiquée (Index d'apnée-hypopnée (IAH) > 30)
- Score sur l'échelle de somnolence d'Epworth (ESS)> 10
- Âge entre 18 et 80 ans
- IMC < 30 kg/m²
Sujets témoins
- Âge entre 18 et 80 ans
- IMC < 30 kg/m²
- Non fumeur
- Sans pathologie respiratoire chronique, trouble cardiovasculaire, métabolique, rénal ou neuromusculaire, vestibulaire et/ou visuel
Critère d'exclusion:
Patients atteints d'AOS et de BPCO
- Pathologies cardiovasculaires, neuromusculaires, métaboliques, rénales
- Alcoolisme
- IMC > 30 kg/m²
- Troubles psychiatriques ou antécédents de troubles du comportement, troubles de la vision, troubles vestibulaires, maladie neurologique sensible perturbant le contrôle postural et la marche, troubles cognitifs
- Instabilité depuis moins de 3 mois
- Contre-argument à l'application d'un champ magnétique externe
