Cerebro durante el esfuerzo: efectos de la hipoxia en pacientes respiratorios (NEUROX)
La oxigenación cerebral está determinada por el producto del CaO2 y el flujo sanguíneo cerebral (FSC), la modificación de uno u otro puede afectar la disponibilidad de O2 neuronal.
Además del efecto de la PaO2, el CBF también está regulado por la PaCO2. Durante el esfuerzo en estado de hipoxia, la caída de la PaO2 asociada a una potencial disminución de la PaCO2 y por tanto del FSC, puede crear un importante vértigo entre la demanda y la oferta de O2 cerebral.
Parece que la hipoxia puede perturbar de manera significativa la respuesta de las neuronas centrales, al igual que la producción de una orden motora generada por la corteza motora.
Los estudios sugieren que el ejercicio en condiciones de hipoxia severa puede constituir una amenaza necesaria para la oxigenación cerebral y el comando motor, con la consecuencia de una disminución del rendimiento del ejercicio.
Este proyecto tiene como objetivo estudiar los efectos de la hipoxia en la función cerebral de pacientes que padecen enfermedades respiratorias crónicas. Respuestas neurofisiológicas del cerebro mientras descansa o hace ejercicio, incluido el goteo y la oxigenación cerebral, la excitación cortical y el comando motor resultantes para sujetos hipóxicos antes y después de un tratamiento para corregir anomalías de la gas en sangre.
El estudio utilizará un enfoque metodológico multidisciplinario y complementario: la espectroscopia de infrarrojo cercano (NIRS) para apreciar el goteo y la oxigenación cerebral, CBF, procedimientos de neuroestimulación y electromiografía (EMG) para apreciar la excitabilidad cortical, medir el nivel de activación central y motor dominio.
Los objetivos de este estudio serán:
- Medir el goteo y oxigenación cerebral, la excitabilidad cortical, mecanismos de activación voluntaria y fatiga central al esfuerzo para el paciente hipoxémico crónico en comparación con sujetos control sanos.
- Analizar alteraciones de los parámetros de locomoción y posturografíac, en tarea simple y doble, involucrando diferentes niveles de tarea cerebral.
- Analizar los efectos agudos de una mejora de la oxigenación arterial en pacientes con enfermedad pulmonar obstructiva crónica (EPOC) sobre el goteo y la oxigenación cerebral, la excitabilidad cortical, los mecanismos de activación voluntaria y la fatiga central.
- Evaluar los efectos de un tratamiento por presión positiva continua en las vías respiratorias (CPAP) para pacientes que padecen apnea obstructiva del sueño (AOS) con los mismos parámetros.

pacientes con EPOC
- estadios GOLD III - IV, cociente FEV1/FVC < 0,7 y ventilación voluntaria máxima (MVV) < 50% de los valores predichos
- IMC < 30 kg/m²
- Edad entre 18 y 80 años
- No fumador o exfumador (parado desde hace más de 3 meses)
- Condición estable desde hace más de 3 meses.
- PaCO2 < 45 mmHg en reposo con aire ambiente
- Sin diagnóstico de AOS
pacientes con AOS
- AOS grave diagnosticado recientemente (Índice de apnea-hipopnea (IAH) > 30)
- Puntuación en la escala de somnolencia de Epworth (ESS) > 10
- Edad entre 18 y 80 años
- IMC < 30 kg/m²

- No fumador
- Sin patología respiratoria crónica, cardiovascular, metabólica, renal o neuromuscular, vestibular y/o visual
Criterio de exclusión:
Pacientes con AOS y EPOC
- Patologías cardiovasculares, neuromusculares, metabólicas, renales
- Alcoholismo
- IMC > 30 kg/m²
- Trastornos psiquiátricos o antecedentes de trastornos del comportamiento, trastornos de la visión, trastornos vestibulares, enfermedad neurológica sensible a alterar el control postural y la marcha, trastornos cognitivos
- Inestabilidad desde hace menos de 3 meses
- Contraargumento a la aplicación de un campo magnético externo

- Patologías respiratorias, cardiovasculares, neuromusculares, metabólicas, renales
- Alcoholismo
- IMC > 30 kg/m²
- Trastornos psiquiátricos o antecedentes de trastornos del comportamiento
