Cérebro Durante o Esforço: Efeitos da Hipóxia em Pacientes Respiratórios (NEUROX)
A oxigenação cerebral é determinada pelo produto do CaO2 e do fluxo sanguíneo cerebral (FSC), a modificação de um ou de outro pode afetar a disponibilidade neuronal de O2.
Além do efeito da PaO2, o FSC também é regulado pela PaCO2. Durante o esforço em estado de hipóxia, a queda da PaO2 associada a uma potencial diminuição da PaCO2 e, portanto, do FSC, pode criar uma importante vertigem entre a demanda e a oferta de O2 cerebral.
Parece que a hipóxia pode perturbar de forma significativa a resposta dos neurônios centrais, assim como a produção de um comando motor gerado pelo córtex motor.
Estudos sugerem que o exercício em condições de hipóxia severa pode constituir uma ameaça necessária para a oxigenação cerebral e o comando motor, tendo como consequência uma diminuição do desempenho do exercício.
Este projeto tem como objetivo estudar os efeitos da hipóxia na função cerebral de pacientes com doença respiratória crônica. Respostas neurofisiológicas do cérebro durante o repouso ou exercício, incluindo gotejamento e oxigenação cerebral, excitação cortical e comando motor resultante para indivíduos hipóxicos antes e depois de um tratamento para corrigir anormalidades de gás no sangue.
O estudo utilizará uma abordagem metodológica multidisciplinar e complementar: a espectroscopia de infravermelho próximo (NIRS) para apreciar o gotejamento e a oxigenação cerebral, CBF, procedimentos de neuroestimulação e eletromiografia (EMG) para apreciar a excitabilidade cortical, medir o nível de ativação central e motor comando.
Os objetivos deste estudo serão:
- Medir o gotejamento e a oxigenação cerebral, a excitabilidade cortical, mecanismos de ativação voluntária e fadiga central ao esforço para o paciente hipoxêmico crônico em comparação com indivíduos controles saudáveis.
- Analisar perturbações dos parâmetros de locomoção e posturografia, em tarefa simples e dupla, envolvendo diferentes níveis de tarefa cerebral.
- Analisar os efeitos agudos de uma melhora da oxigenação arterial para pacientes que sofrem de doença pulmonar obstrutiva crônica (DPOC) em gotejamento e oxigenação cerebral, excitabilidade cortical, mecanismos de ativação voluntária e fadiga central.
- Avaliar os efeitos de um tratamento por pressão positiva contínua nas vias aéreas (CPAP) para pacientes que sofrem de apneia obstrutiva do sono (OSA) com os mesmos parâmetros.

Critério de inclusão:
pacientes com DPOC
- estágios GOLD III - IV, relação VEF1/CVF < 0,7 e ventilação voluntária máxima (VVM) < 50% dos valores previstos
- IMC < 30 kg/m²
- Idade entre 18 e 80 anos
- Não fumador ou ex-fumador (parado há mais de 3 meses)
- Condição estável há mais de 3 meses
- PaCO2 < 45 mmHg em repouso com ar ambiente
- Nenhum diagnóstico de AOS
pacientes com AOS
- AOS grave diagnosticada recentemente (índice de apneia-hipopneia (IAH) > 30)
- Pontuação na Escala de Sonolência de Epworth (ESS) > 10
- Idade entre 18 e 80 anos
- IMC < 30 kg/m²

- Sem qualquer patologia respiratória crónica, cardiovascular, metabólica, renal ou neuromuscular, vestibular e/ou visual
Critério de exclusão:
Pacientes com AOS e DPOC
- Patologias cardiovasculares, neuromusculares, metabólicas, renais
- Alcoolismo
- IMC > 30 kg/m²
- Distúrbios psiquiátricos ou histórico de distúrbios comportamentais, distúrbios da visão, distúrbios vestibulares, doença neurológica sensível a perturbar o controle postural e a marcha, distúrbios cognitivos
- Instabilidade desde menos de 3 meses
- Contra-argumento à aplicação de um campo magnético externo
