Aivot ponnistelun aikana: Hypoksian vaikutukset hengitystiepotilaille (NEUROX)
Aivojen hapettuminen määräytyy CaO2:n ja aivojen verenvirtauksen (CBF) tulon perusteella, jommankumman muutos voi vaikuttaa hermosolujen O2:n saatavuuteen.
PaO2:n vaikutuksen lisäksi CBF:ää säätelee myös PaCO2. Ponnistelun aikana hypoksiassa PaCO2:n ja siten CBF:n mahdolliseen laskuun liittyvä PaO2:n lasku voi aiheuttaa merkittävän huimauksen aivojen O2:n kysynnän ja tarjonnan välillä.
Vaikuttaa siltä, että hypoksia voi merkittävästi vaikeuttaa keskushermosolujen vastetta, aivan kuten motorisen aivokuoren tuotanto synnytti motorisen käskyn.
Tutkimukset viittaavat siihen, että harjoittelu vakavassa hypoksiatilassa voi muodostaa välttämättömän uhan aivojen hapettumiselle ja motoriselle käskylle, minkä seurauksena harjoituksen suorituskyky heikkenee.
Tämän hankkeen tavoitteena on tutkia hypoksian vaikutuksia aivojen toimintaan potilailla, jotka kärsivät kroonisesta hengitystiesairauksista. Aivojen neurofysiologiset vasteet levossa tai harjoittelun aikana, mukaan lukien tippuminen ja aivojen hapetus, aivokuoren viritys ja motoriset käskyt, jotka johtuvat hypoksisista koehenkilöistä ennen ja jälkeen hoidon, jolla korjataan verenkiertohäiriöitä.
Tutkimuksessa käytetään monialaista ja täydentävää metodologista lähestymistapaa: lähi-infrapunaspektroskopiaa (NIRS) arvostaa tippumista ja aivojen hapetusta, CBF:tä, neurostimulaatiomenetelmiä ja elektromyografiaa (EMG) aivokuoren kiihottumisen, keskusaktivaation ja motorisen toiminnan tason mittaamiseksi. komento.
Tämän tutkimuksen tavoitteet ovat:
- Mittaa tippuminen ja aivohapetus, aivokuoren kiihtyvyys, vapaaehtoisen aktivaation mekanismit ja keskusväsymys kroonisesta hypoksemiasta kärsivillä potilailla verrattuna terveisiin kontrollihenkilöihin.
- Analysoi liikeparametrien ja posturografian häiriöitä yksinkertaisessa ja kaksoistehtävässä, joka sisältää eri tasoisia aivotehtäviä.
- Analysoi valtimoiden hapetuksen paranemisen akuutteja vaikutuksia kroonista obstruktiivista keuhkosairautta (COPD) sairastavien potilaiden tippu- ja aivojen hapettumiseen, aivokuoren kiihtyvyyteen, vapaaehtoisen aktivoitumisen mekanismeihin ja keskusväsymykseen.
- Arvioi jatkuvan positiivisen hengitysteiden paineen (CPAP) hoidon vaikutuksia obstruktiivisesta uniapneasta (OSA) kärsiville potilaille samoilla parametreilla.

COPD-potilaat
- vaiheet GOLD III - IV, FEV1/FVC-suhde < 0,7 ja maksimivapaaehtoinen ventilaatio (MVV) < 50 % ennustetuista arvoista
- BMI < 30 kg/m²
- Ikä 18-80 vuotta
- Tupakoimaton tai entinen tupakoitsija (keskeytetty yli 3 kuukauden jälkeen)
- Kunto vakaa yli 3 kuukauden ajan
- PaCO2 < 45 mmHg ympäröivän ilman kanssa
- Ei OSA-diagnoosia
