Le traitement précoce du cancer du sein à faible risque par curiethérapie à haut débit de dose (HDR-BT) (selon les recommandations GEC-ESTRO, ASTRO, ABS) comme seul traitement adjuvant du lit tumoral après chirurgie mammaire conservatrice (BCS) est reconnu par d'éminentes sociétés d'oncologie en Pologne et dans le monde (PTB - Société polonaise de curiethérapie, PTRO - Société polonaise de radio-oncologie, PTO - Société polonaise d'oncologie, ESTRO, ABS, ASTRO, ASCO). Le traitement par HDR-BT interstitiel multicathéter sous forme d'irradiation partielle accélérée du sein (APBI) a été vérifié avec succès dans des essais cliniques randomisés multi-institutionnels (essai hongrois, essai GEC-ESTRO). Il a défini le schéma d'irradiation actuellement largement utilisé, efficace sur le plan oncologique et cliniquement sûr, de 8 x Gy en 5 jours ou de 7 x 4,3 Gy en 4 jours. Aux États-Unis, le schéma d'irradiation de 10 x 3,4 Gy est largement utilisé en 5 à 10 jours (par exemple, NSABP-B39). Certains centres européens et américains étudient et publient des résultats préliminaires de schémas d'irradiation extrêmement raccourcis de 4 x 6,25 Gy ou 3 x 7,5 Gy en 2-3 jours de traitement, en périopératoire (essai vAPBI - Valencia, essai TRIUMPH-T - Phoenix ). Dans le service de curiethérapie du Greater Poland Cancer Center (GPCC), le régime indiqué de 7-8 x 4-4,3 Gy en 4-5 jours est utilisé avec succès depuis 2008. Il en résulte des taux de contrôle locaux très élevés et une très faible toxicité radio-induite. APBI après BSC assure, chez des patientes à faible risque correctement sélectionnées, un taux de contrôle local équivalent et une toxicité plus faible par rapport à l'irradiation du sein entier avec radiothérapie externe (EBRT) L'hypothèse de l'étude est qu'un schéma d'irradiation supplémentaire modérément raccourci de 5 x 5, 4 Gy en 3 jours consécutifs (deux fractions par jour avec un intervalle minimal de 6 heures entre les fractions) n'augmente pas la toxicité précoce et la toxicité tardive de 2 ans (critère principal). En outre, il garantit une probabilité de contrôle local identique ou potentiellement supérieure à 2 ans par rapport au schéma thérapeutique standard (groupe témoin).
Ce qui est essentiel, tous les régimes énumérés ci-dessus sont équivalents en termes de calculs EQD2. L'avantage des doses de fraction plus élevées qui permettent de raccourcir le schéma est un rapport α/β radiobiologique relativement faible, estimé pour le cancer du sein à environ 4.
L'objectif principal de l'étude est de prouver la possibilité d'un raccourcissement progressif supplémentaire de la durée totale du traitement adjuvant postopératoire, ainsi, face à l'augmentation de la morbidité du cancer du sein, d'augmenter son accessibilité. Indirectement, le régime raccourci peut diminuer le fardeau financier du patient et du système de santé, réduire le stress lié au traitement oncologique du patient, l'absence du travail et la séparation de la famille. La faisabilité du projet est probable puisque la seule différence entre les procédures standard et expérimentales réside dans la dose et son nombre.
Il s'agit d'une étude pilote ouverte prospective mono-institutionnelle non randomisée dont les résultats serviraient de base à d'autres recherches sur des groupes de patients plus importants dans une étude randomisée multi-institutionnelle anticipée.
Le critère principal est d'établir le rôle de la curiethérapie HDR adjuvante dans le schéma d'irradiation admissible doublement accéléré et radiobiologiquement équivalent de 5 x 5,4 Gy en 3 jours de traitement (High Five, HiFi-APBI) par rapport à la norme à long terme adoptée d'APBI réalisée en 4-5 jours (7-8 x 4-4,3 Gy) chez des patientes atteintes d'un cancer du sein invasif à faible risque ou d'un carcinome in situ, en termes de toxicité précoce à 3 mois et tardive à 2 ans.
Critères secondaires : 1. Évaluation du taux de contrôle local à 2 ans ; 2. Examen du résultat cosmétique à 2 ans ; 3. Évaluation de la qualité de vie (QOL); 4. Évaluation de la survie globale (OS), de la survie sans maladie (DFS) et de la survie sans métastases à distance (DFMS) ; 5. Analyse des abords chirurgicaux pratiqués (tumorectomie, quadrantectomie, oncoplastie) précédant HDR-BT et leur influence sur les résultats esthétiques.
Il est supposé recruter un minimum de 60 participants en 2-3 ans. Le groupe témoin historique des trois dernières années compte plus d'une centaine de patients. Il est prévu de poursuivre l'étude jusqu'à 5 ans pour atteindre un suivi sensiblement prolongé de 2 ans.
Causes présumées de l'arrêt de l'étude : la survenue de deux cas ou plus de nécrose graisseuse de la zone traitée dans laquelle une intervention chirurgicale serait nécessaire (traitement conservateur inefficace) ; recrutement trop faible (moins du tiers prévu) ; apparition de deux ou plusieurs cas similaires et antérieurs non observés.

- cancer du sein stade 0-II
- Carcinome canalaire invasif (NOS, NST, autres sous-types), carcinome lobulaire invasif, carcinome canalaire in situ (CCIS)
- Carcinome invasif/CCIS, qui est unifocal et unicentrique
- pT1-2, taille tumorale maximale < 3 cm
- pN0 (pas de métastases aux nœuds)
- LVI(-) - pas d'envahissement lymphovasculaire
- M0 (pas de métastases à distance)
- Marges chirurgicales exemptes de cancer (pas de tumeur sur l'encre) dans chaque direction ; dans le cas du DCIS, la marge minimale de 2 mm dans chaque direction
- Manque d'indications à la chimiothérapie/immunothérapie pré- ou post-chirurgicale (par exemple, cancer du sein triple négatif, HER2 positif)
- Début du traitement au plus tard 12 semaines après la chirurgie (de manière optimale 4 à 8 semaines, après la cicatrisation de la plaie)
- Consentement manuscrit éclairé et signé du patient

- cancer du sein stade III-IV
- pT2-4, taille maximale de la tumeur ≥ 3 cm
- les marges chirurgicales ne peuvent pas être correctement évaluées au microscope
- EIC (+) - présence d'un composant intracanalaire étendu
- Maladie de Paget ou atteinte cutanée évaluée au microscope
- pN1-3M1 (présence de métastases ganglionnaires ou à distance)
- indications pré- ou post-chirurgicales pour la chimiothérapie/immunothérapie
- autre cancer (moins de cinq ans au moment du recrutement) sauf cancer de la peau ou cancer du col de l'utérus FIGO 0-I guéri
- moment de la grossesse ou de l'allaitement
- troubles du collagène (congénitaux ou acquis)
- trouble psychiatrique invalidant l'observance du patient
- aspect mammaire ou état postopératoire invalidant une implantation multicathéter interstitielle sûre
- absence de consentement écrit manuscrit éclairé du patient

Expérimental: Irradiation partielle du sein doublement accélérée après une chirurgie mammaire conservatrice
Toutes les participantes recrutées seront traitées par irradiation partielle accélérée du sein en adjuvant à la chirurgie mammaire conservatrice avec la technique de curiethérapie interstitielle multicathéter et prescrites cinq fois 5,4 Gy délivrés en 3 jours consécutifs (6 heures d'écart minimum entre les fractions). Le traitement commence au plus tard 12 semaines après la chirurgie (de manière optimale 4 à 8 semaines), après la cicatrisation de la plaie et l'obtention du rapport pathologique final avec immunohistochimie complète et attribution appropriée du groupe de risque. Le groupe témoin est constitué d'APBI adjuvante standard avec technique de curiethérapie interstitielle multicathéter 8 x 4 Gy en 5 jours consécutifs ou 7 x 4,3 Gy en 4 jours consécutifs (écart minimum de 6 heures entre les fractions). |
