Vroegtijdige behandeling van borstkanker met een laag risico met hooggedoseerde brachytherapie (HDR-BT) (volgens GEC-ESTRO, ASTRO, ABS-aanbevelingen) als enige adjuvante behandeling van het tumorbed na borstsparende chirurgie (BCS) wordt erkend door prominente oncologische verenigingen in Polen en de wereld (PTB - Polish Brachytherapy Society, PTRO - Polish Society for Radiation Oncology, PTO - Polish Society of Oncology, ESTRO, ABS, ASTRO, ASCO). Behandeling met interstitiële HDR-BT met meerdere katheters in de vorm van versnelde gedeeltelijke borstbestraling (APBI) werd met succes geverifieerd in multi-institutionele gerandomiseerde klinische onderzoeken (Hongaarse studie, GEC-ESTRO-studie). Het definieerde het momenteel algemeen gebruikte oncologisch effectieve en klinisch veilige bestralingsschema van 8 x Gy in 5 dagen of 7 x 4,3 Gy in 4 dagen. In de VS wordt het bestralingsschema van 10 x 3,4 Gy veel gebruikt in 5-10 dagen (bijv. NSABP-B39). Sommige Europese en Amerikaanse centra onderzoeken en publiceren voorlopige resultaten van extreem verkorte bestralingsschema's van 4 x 6,25 Gy of 3 x 7,5 Gy in 2-3 behandeldagen, perioperatief (vAPBI-studie - Valencia, TRIUMPH-T-studie - Phoenix ). Op de brachytherapieafdeling van het Groot-Polen Kankercentrum (GPCC) wordt sinds 2008 met succes het vermelde regime van 7-8 x 4-4,3 Gy in 4-5 dagen gebruikt. Het resulteert in zeer hoge lokale controlesnelheden en zeer lage stralingsgeïnduceerde toxiciteit. APBI na BSC zorgt bij correct geselecteerde patiënten met een laag risico voor een gelijkwaardige lokale controlesnelheid en lagere toxiciteit in vergelijking met volledige borstbestraling met uitwendige bestralingstherapie (EBRT). De onderzoekshypothese is dat een extra matig verkort bestralingsschema van 5 x 5 4 Gy in 3 opeenvolgende dagen (twee fracties per dag met minimaal 6 uur tussenpozen) verhoogt de vroege toxiciteit en 2 jaar late toxiciteit niet (primair eindpunt). Het zorgt ook voor dezelfde of mogelijk hogere kans op lokale controle na 2 jaar in vergelijking met het standaardregime (controlegroep).
Wat essentieel is, is dat alle hierboven vermelde regimes equivalent zijn in termen van EQD2-berekeningen. Het voordeel van hogere fractiedoses die verkorting van het schema mogelijk maken, is een relatief lage radiobiologische α/β-ratio, geschat op ongeveer 4 voor borstkanker.
Het primaire doel van de studie is om de mogelijkheid aan te tonen van een verdere geleidelijke verkorting van de totale tijd van postoperatieve adjuvante behandeling, waardoor, ondanks de toenemende morbiditeit van borstkanker, de toegankelijkheid ervan wordt vergroot. Indirect kan het verkorte regime de financiële last van de patiënt en het gezondheidssysteem verminderen, de oncologische behandelingsgerelateerde stress van de patiënt, het ziekteverzuim en de scheiding van familie verminderen. De haalbaarheid van het project is waarschijnlijk aangezien het enige verschil tussen standaard en experimentele procedures de dosis en het aantal is.
Het is een prospectieve mono-institutionele niet-gerandomiseerde open-label pilotstudie waarvan de resultaten een basis zouden vormen voor verder onderzoek bij grotere patiëntengroepen in een verwachte multi-institutionele gerandomiseerde studie.
Het primaire eindpunt is het vaststellen van de rol van adjuvante HDR-brachytherapie in het toegestane dubbel versnelde en radiobiologisch equivalente bestralingsschema van 5 x 5,4 Gy in 3 dagen behandeling (High Five, HiFi-APBI) in vergelijking met de aangenomen langetermijnstandaard van APBI gerealiseerd in 4-5 dagen (7-8 x 4-4,3 Gy) bij patiënten met invasieve borstkanker met laag risico of carcinoom in situ, in termen van 3 maanden vroege en 2 jaar late toxiciteit.
Secundaire eindpunten: 1. Beoordeling van 2-jaars lokale controlepercentage; 2. Beoordeling van cosmetisch resultaat na 2 jaar; 3. Evaluatie van de kwaliteit van leven (QOL); 4. Evaluatie van de totale overleving (OS), ziektevrije overleving (DFS) en metastasenvrije overleving op afstand (DFMS); 5. Analyse van de uitgevoerde chirurgische benaderingen (tumorectomie, quadrantectomie, oncoplastiek) voorafgaand aan HDR-BT en hun invloed op de cosmetische resultaten.
Er wordt aangenomen dat er binnen 2-3 jaar minimaal 60 deelnemers worden geworven. De historische controlegroep van de laatste drie jaar telt ruim honderd patiënten. Het is de bedoeling dat de studie maximaal 5 jaar wordt voortgezet om een substantieel verlengde follow-up van 2 jaar te bereiken.
Vermoedelijke oorzaken van studiebeëindiging: het optreden van twee of meer gevallen van vetnecrose in het behandelde gebied waarbij chirurgische interventie noodzakelijk zou zijn (conservatieve behandeling niet effectief); te lage werving (minder dan een derde van gepland); optreden van twee of meer vergelijkbare en eerder niet waargenomen gevallen.

Inclusiecriteria:
- stadium 0-II borstkanker
- Invasief ductaal carcinoom (NOS, NST, andere subtypes), invasief lobulair carcinoom, ductaal carcinoom in situ (DCIS)
- Invasief carcinoom/DCIS, dat unifocaal en unicentrisch is
- pT1-2, maximale tumorgrootte < 3 cm
- pN0 (geen uitzaaiingen naar de knooppunten)
- LVI(-) - geen lymfovasculaire invasie
- M0 (geen metastasen op afstand)
- Chirurgische marges vrij van kanker (geen tumor op inkt) in elke richting; in het geval van DCIS, de minimale marge van 2 mm in elke richting
- Gebrek aan indicaties voor chemotherapie/immunotherapie pre- of postoperatief (bijv. triple-negatieve, HER2-positieve borstkanker)
- Behandeling start uiterlijk 12 weken na de operatie (optimaal 4-8 weken, na wondgenezing)
- Geïnformeerde handgeschreven ondertekende toestemming van de patiënt

- stadium III-IV borstkanker
- pT2-4, maximale tumorgrootte ≥ 3 cm
- chirurgische marges kunnen microscopisch niet goed worden beoordeeld
- EIC(+) - aanwezigheid van uitgebreide intraductale component
- de ziekte van Paget of microscopisch beoordeelde huidbetrokkenheid
- pN1-3M1 (aanwezigheid van knooppunten of metastasen op afstand)
- pre- of postoperatieve indicaties voor chemotherapie/immunotherapie
- andere kanker (minder dan vijf jaar op het moment van werving) behalve huidkanker of genezen FIGO 0-I baarmoederhalskanker
- tijd van zwangerschap of borstvoeding
- collageenaandoeningen (aangeboren of verworven)
- psychiatrische stoornis waardoor de therapietrouw van de patiënt wordt uitgeschakeld
- uiterlijk van de borst of postoperatieve status die veilige interstitiële multikatheterimplantatie onmogelijk maken
- gebrek aan geïnformeerde handgeschreven ondertekende toestemming van de patiënt

Experimenteel: Dubbel versnelde gedeeltelijke borstbestraling na borstsparende chirurgie
Alle geworven deelnemers zullen worden behandeld met adjuvans voor borstsparende chirurgie, versnelde gedeeltelijke borstbestraling met interstitiële brachytherapietechniek met meerdere katheters en vijf keer 5,4 Gy voorgeschreven in 3 opeenvolgende dagen (minimaal 6 uur tussen de fracties). De behandeling start niet later dan 12 weken na de operatie (optimaal 4-8 weken), na wondgenezing en het verkrijgen van het definitieve pathologische rapport met volledige immunohistochemie en juiste toewijzing van de risicogroep. De controlegroep bestaat uit standaard adjuvante APBI met multikatheter interstitiële brachytherapietechniek 8 x 4 Gy in 5 opeenvolgende dagen of 7 x 4,3 Gy in 4 opeenvolgende dagen (minimaal 6 uur tussen de fracties). |
