乳房温存手術(BCS)後の腫瘍床への唯一の補助療法として、高線量率小線源治療(HDR-BT)(GEC-ESTRO、ASTRO、ABSの推奨事項による)による早期の低リスク乳がん治療が認められているポーランドおよび世界の著名な腫瘍学会 (PTB - ポーランド小線源治療協会、PTRO - ポーランド放射線腫瘍学会、PTO - ポーランド腫瘍学会、ESTRO、ABS、ASTRO、ASCO) による。 加速部分乳房照射 (APBI) の形でのマルチカテーテルインタースティシャル HDR-BT による治療は、多施設無作為化臨床試験 (ハンガリー試験、GEC-ESTRO 試験) で首尾よく検証されました。 それは、現在広く使用されている腫瘍学的に効果的で臨床的に安全な照射スキームを定義し、8 x Gy を 5 日間、または 7 x 4.3 Gy を 4 日間で照射しました。 米国では、10 x 3.4 Gy の照射スキームが 5 ~ 10 日間で広く使用されています (例: NSABP-B39)。 ヨーロッパとアメリカのいくつかのセンターでは、周術期に 2 ~ 3 日間の治療で 4 x 6.25 Gy または 3 x 7.5 Gy という非常に短縮された照射スキームの予備結果を調査し、公表しています (vAPBI 試験 - バレンシア、TRIUMPH-T 試験 - フェニックス)。 グレーター ポーランドがんセンター (GPCC) の小線源治療部門では、リストにある 7-8 x 4-4.3 Gy で 4-5 日間のレジメンが 2008 年から使用され、成功しています。 その結果、局所制御率が非常に高くなり、放射線誘発毒性が非常に低くなります。 BSC 後の APBI は、適切に選択された低リスク患者において、外照射療法 (EBRT) による全乳房照射と比較して同等の局所制御率と低い毒性を保証します。連続 3 日間で 4 Gy (1 日 2 回、分割間隔を最小 6 時間空ける) では、早期毒性と 2 年後毒性は増加しません (主要評価項目)。 また、標準的なレジメン(対照群)と比較して、2 年間の局所制御確率が同じか、潜在的に高くなります。
重要なのは、上記のすべてのレジメンが EQD2 計算に関して同等であることです。 スキームの短縮を可能にするより高い分割線量の利点は、放射線生物学的 α/β 比が比較的低く、乳癌で約 4 と推定されることです。
この研究の主な目的は、術後補助療法の合計時間をさらに段階的に短縮する可能性を証明することです。これにより、乳がん罹患率の増加に直面して、そのアクセシビリティが向上します。 間接的に、レジメンの短縮は、患者と医療システムの経済的負担を軽減し、患者の腫瘍治療に関連するストレス、仕事の不在、および家族からの分離を軽減する可能性があります。 標準手順と実験手順の唯一の違いは線量とその数であるため、プロジェクトの実現可能性は高いと考えられます。
これは前向きな単一施設の非無作為化非盲検パイロット研究であり、その結果は、予想される多施設無作為化研究におけるより大規模な患者グループに関するさらなる研究の基礎となる.
主要評価項目は、採用された長期標準と比較して、3 日間の治療で 5 x 5.4 Gy の許容可能な二重加速放射線生物学的に同等の照射スキーム (ハイファイブ、HiFi-APBI) における補助 HDR 小線源治療の役割を確立することです。 3 か月早期および 2 年後期毒性に関して、乳房の低リスク浸潤がんまたは上皮内がんの患者で 4 ~ 5 日 (7 ~ 8 x 4 ~ 4.3 Gy) で APBI が実現します。
副次評価項目: 1. 2 年間の局所制御率の評価。 2. 2年間の美容結果のレビュー; 3.生活の質(QOL)の評価; 4. 全生存期間 (OS)、無病生存期間 (DFS)、および無遠隔転移生存期間 (DFMS) の評価。 5. HDR-BT の前に実施された外科的アプローチ (腫瘍切除術、四分円切除術、腫瘍形成術) の分析と、美容結果への影響。
2~3年で最低60名の参加者を募集することを想定しています。 過去 3 年間の歴史的対照群には、100 人以上の患者がいます。 この研究は、最大 5 年間継続され、2 年間の大幅な追跡調査に達する予定です。
試験終了の推定原因:治療部位脂肪壊死が2例以上発生し、外科的介入が必要となる(保存的治療無効)。採用が少なすぎる(計画の 3 分の 1 未満)。 2 つ以上の同様の、以前に観察されなかったケースの発生。

- 術前または術後の化学療法/免疫療法に対する適応症の欠如 (例: トリプルネガティブ、HER2 陽性乳癌)
