Rifaximine pour la prévention secondaire de la pochite récurrente

Bien que de nombreuses personnes développent une pochite récurrente (inflammation de l'anastomose poche iléale-anale ou poche en J après colectomie pour la colite ulcéreuse) après un premier épisode de pochite, il n'existe actuellement aucun traitement efficace pour prévenir la pochite récurrente.
Le but de cette étude est d'évaluer le potentiel de la rifaximine, un antibiotique, pour prévenir les pochites récurrentes après le traitement d'un premier épisode de pochite.
Dans cette étude, tous les patients recevront quotidiennement de la rifaximine pendant un an après avoir été traités pour un premier épisode de pochite.
Cette étude examinera si les gens sont prêts à prendre de la rifaximine pendant un an dans le but de prévenir les pochites récurrentes.
De plus, cette étude examinera si les patients ressentent des effets secondaires inattendus du traitement par la rifaximine.
Les informations obtenues grâce à cette étude seront potentiellement utiles pour améliorer notre capacité à prévenir les pochites récurrentes chez les patients ayant subi une colectomie pour colite ulcéreuse.

Critère d'intégration:
- Un consentement éclairé sera obtenu avant toute procédure liée à l'étude
- Âge > 18 et < 75 ans
- Participants ayant des antécédents avérés de colite ulcéreuse et des antécédents d'anastomose anale de la poche iléale modifiée (IPAA) en 1, 2, 2 ou 3 stades modifiés et de retrait d'iléostomie
- Diagnostic de l'épisode initial de pochite dans les 12 premiers mois suivant le retrait de l'iléostomie/stade final de la chirurgie IPAA
Critère d'exclusion:
- Hypersensibilité connue à la rifaximine ou à ses métabolites
- Maladie de Crohn connue
- Histoire de la fistule périanale
- Incontinence connue due à un dysfonctionnement du sphincter anal
- Syndrome connu de la poche irritable
- Infection pelvienne/septicémie active en cours lors de la visite initiale
- Nouvelle apparition d'une fréquence intestinale élevée dans le cadre d'une pochite aiguë au cours des 4 premières semaines suivant l'IPAA
- Infection connue à Clostridoides difficile
- Nécessité d'un traitement antibiotique à long terme (par ex. doxycycline pour l'acné)
- Hépatite B, C, VIH active connue
- Maladie hépatique cliniquement significative (une cholangite sclérosante primitive avec une limite supérieure de la normale du LFT <1,5 peut être incluse)
- Insuffisance hépatique sévère, définie comme Child-Pugh classe C
- Utilisation concomitante d'inhibiteurs de la glycoprotéine P (P-gp) (par ex. cyclosporine)
- Diminution connue de la fonction rénale avec un débit de filtration glomérulaire <60 ml/min/1,732
- Transplantation de microbiote fécal dans les 16 semaines précédant le retrait de l'iléostomie
- Antécédents de tumeur maligne, à l'exception du carcinome basocellulaire, du carcinome épidermoïde non métastatique de la peau ou d'une tumeur maligne antérieure avec un traitement curatif terminé au moins 5 ans avant le dépistage et aucune récidive.
- Résultats de laboratoire cliniquement significatifs au moment du dépistage ou de référence, tels que jugés par l'enquêteur à partir de tests locaux.
- Femme enceinte, qui allaite, qui a l'intention de devenir enceinte ou qui est en âge de procréer et qui n'utilise pas de méthode contraceptive très efficace. Les femmes en âge de procréer doivent subir un test de grossesse urinaire négatif avant que le médicament ne soit délivré.
- Participation à tout essai clinique d'un médicament expérimental approuvé ou non approuvé dans les 30 jours précédant le dépistage.
- Tout trouble qui, de l'avis de l'enquêteur, pourrait compromettre la sécurité du participant ou le respect du protocole.
