Rifaximin för sekundärt förebyggande av återkommande pouchitis

Även om många människor kommer att utveckla återkommande pouchit (inflammation i ileal pouch-anal anastomos eller J-pouch efter kolektomi för ulcerös kolit) efter en första episod av pouchit, finns det för närvarande inga effektiva behandlingar för att förhindra återkommande pouchit.
Målet med denna studie är att utvärdera potentialen för rifaximin, ett antibiotikum, för att förhindra återkommande pouchit efter behandling för en första episod av pouchit.
I denna studie kommer alla patienter att ges rifaximin dagligen under ett år efter att ha behandlats för en första episod av pouchit.
Denna studie kommer att undersöka om människor är villiga att ta rifaximin i ett år med målet att förhindra återkommande pouchitis.
Dessutom kommer denna studie att undersöka om patienter upplever några oväntade biverkningar av rifaximinbehandling.
Den information som erhålls genom denna studie kommer potentiellt att vara till hjälp för att förbättra vår förmåga att förhindra återkommande pouchit hos patienter som har en kolektomi för ulcerös kolit.

Inklusionskriterier:
- Informerat samtycke kommer att erhållas före eventuella studierelaterade procedurer
- Ålder > 18 och <75 år
- Deltagare med en dokumenterad historia av ulcerös kolit och historia av 1,2, modifierad -2 eller 3 steg Ileal pouch anal anastomosis (IPAA) och ileostomi borttagning
- Diagnos av initial episod av pouchit inom de första 12 månaderna efter ileostomiborttagning/slutstadiet av IPAA-kirurgi
Exklusions kriterier:
- Känd överkänslighet mot rifaximin eller dess metaboliter
- Känd Crohns sjukdom
- Historien om perianal fistel
- Känd inkontinens på grund av analsfinkterdysfunktion
- Känt irritabelt påsesyndrom
- Aktiv pågående bäckeninfektion/sepsis vid baslinjebesök
- Ny uppkomst av hög tarmfrekvens vid akut pouchit under de första 4 veckorna efter IPAA
- Känd Clostridoides difficile-infektion
- Behov av långtidsbehandling med antibiotika (t. doxycyklin mot akne)
- Känd aktiv Hepatit B, C, HIV
- Kliniskt signifikant leversjukdom (Primär skleroserande kolangit med LFTs <1,5 övre normalgräns kan inkluderas)
- Svårt nedsatt leverfunktion, definierat som Child-Pugh klass C
- Samtidig användning av p-glykoprotein (P-gp)-hämmare (t.ex. cyklosporin)
- Känd nedsatt njurfunktion med en glomerulär filtrationshastighet <60 ml/min/1,732
- Fekal mikrobiotatransplantation inom 16 veckor innan ileostomi avlägsnas
- Historik av malignitet, förutom basalcellscancer, icke-metastaserande skivepitelcancer i huden eller tidigare malignitet med botande behandling avslutad minst 5 år före screening och inget återfall.
- Kliniskt signifikanta laboratorieresultat vid screening eller baslinje, som bedömts av utredaren från lokala tester.
- Kvinna som är gravid, ammar eller har för avsikt att bli gravid eller är i fertil ålder och inte använder en mycket effektiv preventivmetod. Kvinnor i fertil ålder måste ha ett negativt uringraviditetstest innan läkemedlet dispenseras.
- Deltagande i någon klinisk prövning av ett godkänt eller icke-godkänt prövningsläkemedel inom 30 dagar före screening.
- Varje störning som enligt utredarens åsikt kan äventyra deltagarens säkerhet eller efterlevnad av protokollet.
