Рифаксимин для вторичной профилактики рецидивирующего пулита

Хотя у многих людей после первоначального эпизода резервуарита развивается рецидивирующий резервуар (воспаление подвздошно-анального анастомоза или J-образного резервуара после колэктомии по поводу язвенного колита), в настоящее время не существует эффективных методов лечения рецидива резервуарного резервуара.
Цель этого исследования — оценить потенциал рифаксимина, антибиотика, для предотвращения рецидива илеита после лечения начального эпизода илеита.
В этом исследовании всем пациентам будет ежедневно назначаться рифаксимин в течение одного года после лечения первоначального эпизода поухита.
В этом исследовании будет изучено, готовы ли люди принимать рифаксимин в течение одного года с целью предотвращения рецидивирующего поухита.
Кроме того, в этом исследовании будет изучено, испытывают ли пациенты какие-либо неожиданные побочные эффекты от терапии рифаксимином.
Информация, полученная в ходе этого исследования, потенциально будет полезна для улучшения нашей способности предотвращать рецидивирующий поухит у пациентов, перенесших колэктомию по поводу язвенного колита.

Критерии включения:
- Информированное согласие будет получено перед любыми процедурами, связанными с исследованием.
- Возраст > 18 и <75 лет
- Участники с подтвержденным язвенным колитом в анамнезе и историей 1, 2, модифицированной -2 или 3 стадии анального анастомоза подвздошной кишки (IPAA) и снятия илеостомы.
- Диагностика начального эпизода поухита в течение первых 12 месяцев после снятия илеостомы/завершающего этапа операции ИПБА.
Критерий исключения:
- Известная гиперчувствительность к рифаксимину или его метаболитам.
- Известная болезнь Крона
- История перианальной фистулы
- Известное недержание мочи из-за дисфункции анального сфинктера.
- Известный синдром раздраженного мешка
- Активная продолжающаяся инфекция органов малого таза/сепсис на исходном визите
- Новое появление учащенного дефекации на фоне острого резервуарного резервуара в первые 4 недели после ИППА.
- Известная инфекция Clostridoides difficile
- Необходимость длительной терапии антибиотиками (например, доксициклин от прыщей)
- Известный активный гепатит B, C, ВИЧ.
- Клинически значимое заболевание печени (можно включить первичный склерозирующий холангит с верхней границей нормы LFT <1,5)
- Тяжелая печеночная недостаточность, определяемая как класс C по Чайлд-Пью.
- Одновременное применение ингибиторов p-гликопротеина (P-gp) (например, циклоспорин)
- Известно снижение функции почек при скорости клубочковой фильтрации <60 мл/мин/1,732.
- Трансплантация фекальной микробиоты в течение 16 недель до снятия илеостомы
- Злокачественные новообразования в анамнезе, за исключением базальноклеточного рака, неметастатического плоскоклеточного рака кожи или предшествующего злокачественного новообразования с лечебной терапией, проведенной не менее чем за 5 лет до скрининга, и без рецидива.
- Клинически значимые результаты лабораторных исследований при скрининге или на исходном уровне, по оценке исследователя на основе местного тестирования.
- Женщины, которые беременны, кормят грудью или планируют забеременеть или имеют детородный потенциал и не используют высокоэффективный метод контрацепции. Женщины детородного возраста должны иметь отрицательный результат теста на беременность в моче перед назначением препарата.
- Участие в любом клиническом исследовании одобренного или неутвержденного исследуемого лекарственного препарата в течение 30 дней до скрининга.
- Любое расстройство, которое, по мнению исследователя, может поставить под угрозу безопасность участника или соблюдение протокола.
