Rifaximin zur Sekundärprävention rezidivierender Pouchitis

Obwohl viele Menschen nach einer ersten Episode einer Pouchitis eine wiederkehrende Pouchitis (Entzündung des Ileum-Pouch-Anastomose oder J-Pouch nach Kolektomie bei Colitis ulcerosa) entwickeln, gibt es derzeit keine wirksamen Behandlungen zur Vorbeugung einer wiederkehrenden Pouchitis.
Das Ziel dieser Studie besteht darin, das Potenzial von Rifaximin, einem Antibiotikum, zur Vorbeugung einer wiederkehrenden Pouchitis nach der Behandlung einer ersten Episode einer Pouchitis zu bewerten.
In dieser Studie erhalten alle Patienten ein Jahr lang täglich Rifaximin, nachdem sie wegen einer ersten Episode einer Pouchitis behandelt wurden.
In dieser Studie wird untersucht, ob Menschen bereit sind, ein Jahr lang Rifaximin einzunehmen, um einer wiederkehrenden Pouchitis vorzubeugen.
Darüber hinaus wird in dieser Studie untersucht, ob bei Patienten unerwartete Nebenwirkungen der Rifaximin-Therapie auftreten.
Die durch diese Studie gewonnenen Informationen werden möglicherweise dazu beitragen, unsere Fähigkeit zu verbessern, wiederkehrende Pouchitis bei Patienten zu verhindern, die sich einer Kolektomie wegen Colitis ulcerosa unterziehen.

Beschreibung
Einschlusskriterien:
- Vor allen studienbezogenen Verfahren wird eine Einverständniserklärung eingeholt
- Alter > 18 und <75 Jahre
- Teilnehmer mit einer nachgewiesenen Vorgeschichte von Colitis ulcerosa und einer Vorgeschichte von 1, 2, modifizierter -2 oder 3 Stadium Ileum-Pouch-Analanastomose (IPAA) und Ileostomie-Abnahme
- Diagnose einer ersten Episode einer Pouchitis innerhalb der ersten 12 Monate nach Entfernung des Ileostomas/Endstadium der IPAA-Operation
Ausschlusskriterien:
- Bekannte Überempfindlichkeit gegen Rifaximin oder seine Metaboliten
- Bekannter Morbus Crohn
- Geschichte der perianalen Fistel
- Bekannte Inkontinenz aufgrund einer Funktionsstörung des Analsphinkters
- Bekanntes Reizbeutelsyndrom
- Aktive anhaltende Beckeninfektion/Sepsis beim Erstbesuch
- Neu auftretende hohe Stuhlfrequenz im Rahmen einer akuten Pouchitis in den ersten 4 Wochen nach IPAA
- Bekannte Clostridoides-difficile-Infektion
- Notwendigkeit einer Antibiotika-Langzeittherapie (z.B. Doxycyclin gegen Akne)
- Bekannte aktive Hepatitis B, C, HIV
- Klinisch signifikante Lebererkrankung (primär sklerosierende Cholangitis mit LFTs <1,5 Obergrenze des Normalwerts kann eingeschlossen werden)
- Schwere Leberfunktionsstörung, definiert als Child-Pugh-Klasse C
- Die gleichzeitige Anwendung von P-Glykoprotein (P-gp)-Inhibitoren (z. B. Cyclosporin)
- Bekannte verminderte Nierenfunktion mit einer glomerulären Filtrationsrate <60 ml/min/1,732
- Transplantation fäkaler Mikrobiota innerhalb von 16 Wochen vor der Entfernung des Ileostomas
- Malignität in der Anamnese, mit Ausnahme von Basalzellkarzinomen, nicht-metastasierten Plattenepithelkarzinomen der Haut oder früheren Malignitäten mit kurativer Therapie, die mindestens 5 Jahre vor dem Screening abgeschlossen wurde und kein Wiederauftreten aufweist.
- Klinisch signifikante Laborergebnisse beim Screening oder bei Studienbeginn, wie vom Prüfarzt anhand lokaler Tests beurteilt.
- Frauen, die schwanger sind, stillen oder eine Schwangerschaft planen oder im gebärfähigen Alter sind und keine hochwirksame Verhütungsmethode anwenden. Bei Frauen im gebärfähigen Alter muss vor der Verabreichung des Arzneimittels ein negativer Urin-Schwangerschaftstest vorliegen.
- Teilnahme an einer klinischen Studie mit einem zugelassenen oder nicht zugelassenen Prüfpräparat innerhalb von 30 Tagen vor dem Screening.
- Jede Störung, die nach Ansicht des Prüfers die Sicherheit des Teilnehmers oder die Einhaltung des Protokolls gefährden könnte.
