Rifaximin voor de secundaire preventie van recidiverende pouchitis

Hoewel veel mensen recidiverende pouchitis (ontsteking van de ileale pouch-anale anastomose of J-pouch na colectomie voor colitis ulcerosa) zullen ontwikkelen na een eerste episode van pouchitis, zijn er momenteel geen effectieve behandelingen om recidiverende pouchitis te voorkomen.
Het doel van deze studie is om het potentieel van rifaximin, een antibioticum, te evalueren om recidiverende pouchitis te voorkomen na behandeling van een eerste episode van pouchitis.
In dit onderzoek krijgen alle patiënten gedurende één jaar dagelijks rifaximin toegediend nadat ze zijn behandeld voor een eerste episode van pouchitis.
In deze studie wordt onderzocht of mensen bereid zijn om een jaar lang rifaximin te gebruiken met als doel recidiverende pouchitis te voorkomen.
Daarnaast zal in dit onderzoek worden onderzocht of patiënten onverwachte bijwerkingen ondervinden van de behandeling met rifaximine.
De informatie die door dit onderzoek wordt verkregen, zal mogelijk nuttig zijn bij het verbeteren van ons vermogen om recidiverende pouchitis te voorkomen bij patiënten die een colectomie ondergaan voor colitis ulcerosa.

Inclusiecriteria:
- Voorafgaand aan eventuele studiegerelateerde procedures zal geïnformeerde toestemming worden verkregen
- Leeftijd > 18 en <75 jaar
- Deelnemers met een bewezen voorgeschiedenis van colitis ulcerosa en een voorgeschiedenis van 1,2, gemodificeerde -2 of 3 stadium Ileal pouch anale anastomose (IPAA) en verwijdering van een ileostomie
- Diagnose van de eerste episode van pouchitis binnen de eerste 12 maanden na verwijdering van de ileostoma/laatste fase van een IPAA-operatie
Uitsluitingscriteria:
- Bekende overgevoeligheid voor rifaximin of zijn metabolieten
- Bekende ziekte van Crohn
- Geschiedenis van perianale fistel
- Bekende incontinentie als gevolg van disfunctie van de anale sluitspier
- Bekend prikkelbare buidelsyndroom
- Actieve, aanhoudende bekkeninfectie/sepsis tijdens het basisbezoek
- Nieuw begin van hoge stoelgangfrequentie bij acute pouchitis in de eerste 4 weken na IPAA
- Bekende infectie met Clostridoides difficile
- Noodzaak van langdurige antibioticatherapie (bijv. doxycycline voor acne)
- Bekende actieve Hepatitis B, C, HIV
- Klinisch significante leverziekte (primaire scleroserende cholangitis met LFT's <1,5 bovengrens van normaal kan worden opgenomen)
- Ernstige leverfunctiestoornis, gedefinieerd als Child-Pugh-klasse C
- Gelijktijdig gebruik van p-glycoproteïne (P-gp)-remmers (bijv. cyclosporine)
- Bekende verminderde nierfunctie met een glomerulaire filtratiesnelheid <60 ml/min/1,732
- Fecale microbiota-transplantatie binnen 16 weken vóór verwijdering van de ileostoma
- Voorgeschiedenis van maligniteit, behalve basaalcelcarcinoom, niet-gemetastaseerd plaveiselcelcarcinoom van de huid, of eerdere maligniteit met curatieve therapie voltooid ten minste 5 jaar voorafgaand aan de screening en geen recidief.
- Klinisch significante laboratoriumresultaten bij screening of baseline, zoals beoordeeld door de onderzoeker op basis van lokale tests.
- Vrouwen die zwanger zijn, borstvoeding geven, van plan zijn zwanger te worden of zwanger kunnen worden en die geen zeer effectieve anticonceptiemethode gebruiken. Vrouwen die zwanger kunnen worden, moeten een negatieve urinezwangerschapstest ondergaan voordat het geneesmiddel wordt verstrekt.
- Deelname aan een klinische proef met een goedgekeurd of niet-goedgekeurd onderzoeksgeneesmiddel binnen 30 dagen vóór de screening.
- Elke aandoening die naar de mening van de onderzoeker de veiligheid van de deelnemer of de naleving van het protocol in gevaar zou kunnen brengen.
