Rifaximin pro sekundární prevenci rekurentní pouchitidy

Ačkoli se u mnoha lidí po počáteční epizodě pouchitidy rozvine rekurentní pouchitida (zánět anastomózy ileálního vaku-anální anastomózy nebo J-pouchu po kolektomii pro ulcerózní kolitidu), v současné době neexistuje žádná účinná léčba, která by rekurentní pouchitidě zabránila.
Cílem této studie je vyhodnotit potenciál rifaximinu, antibiotika, k prevenci rekurentní pouchitidy po léčbě počáteční epizody pouchitidy.
V této studii bude všem pacientům denně podáván rifaximin po dobu jednoho roku poté, co byli léčeni na počáteční epizodu pouchitidy.
Tato studie bude zkoumat, zda jsou lidé ochotni užívat rifaximin po dobu jednoho roku s cílem zabránit opakující se pouchitidě.
Kromě toho bude tato studie zkoumat, zda pacienti pociťují nějaké neočekávané vedlejší účinky léčby rifaximinem.
Informace získané prostřednictvím této studie budou potenciálně užitečné při zlepšování naší schopnosti předcházet rekurentní pouchitidě u pacientů, kteří mají kolektomii pro ulcerózní kolitidu.

Kritéria pro zařazení:
- Před jakýmikoli postupy souvisejícími se studií bude získán informovaný souhlas
- Věk > 18 a < 75 let
- Účastníci s prokázanou anamnézou ulcerózní kolitidy a anamnézou 1, 2, modifikované -2 nebo 3 fáze anální anastomózy ileálního vaku (IPAA) a odstranění ileostomie
- Diagnostika počáteční epizody pouchitidy během prvních 12 měsíců po odstranění ileostomie/konečné fázi operace IPAA
Kritéria vyloučení:
- Známá přecitlivělost na rifaximin nebo jeho metabolity
- Známá Crohnova choroba
- Historie perianální píštěle
- Známá inkontinence způsobená dysfunkcí análního svěrače
- Známý syndrom dráždivého váčku
- Aktivní probíhající pánevní infekce/sepse při vstupní návštěvě
- Nový nástup vysoké frekvence střev v podmínkách akutní pouchitidy v prvních 4 týdnech po IPAA
- Známá infekce Clostridoides difficile
- Potřeba dlouhodobé léčby antibiotiky (např. doxycyklin na akné)
- Známá aktivní hepatitida B, C, HIV
- Klinicky významné onemocnění jater (lze zahrnout primární sklerotizující cholangitidu s LFT <1,5 horní hranice normy)
- Těžká porucha funkce jater, definovaná jako Child-Pugh třída C
- Současné užívání inhibitorů p-glykoproteinu (P-gp) (např. cyklosporin)
- Známá snížená funkce ledvin s rychlostí glomerulární filtrace <60 ml/min/1,732
- Transplantace fekální mikroflóry do 16 týdnů před odstraněním ileostomie
- Malignita v anamnéze, s výjimkou bazaliomu, nemetastatického spinocelulárního karcinomu kůže nebo předchozí malignity s kurativní terapií dokončenou alespoň 5 let před Screeningem a bez recidivy.
- Klinicky významné laboratorní výsledky při screeningu nebo na začátku studie, jak posoudil zkoušející z místního testování.
- Žena, která je těhotná, kojí nebo zamýšlí otěhotnět nebo je ve fertilním věku a nepoužívá vysoce účinnou metodu antikoncepce. Ženy ve fertilním věku musí mít před výdejem léku negativní těhotenský test z moči.
- Účast na jakémkoli klinickém hodnocení schváleného nebo neschváleného hodnoceného léčivého přípravku do 30 dnů před screeningem.
- Jakákoli porucha, která by podle názoru vyšetřovatele mohla ohrozit bezpečnost účastníka nebo dodržování protokolu.
