Il s'agit d'une étude clinique de phase 3, multicentrique, randomisée, en double aveugle, en groupes parallèles et contrôlée par placebo, visant à évaluer l'efficacité et la sécurité du HBS-301 dans le traitement des symptômes d'hypersomnie idiopathique (IH), incluant la somnolence diurne excessive (EDS), l'inertie du sommeil et la fatigue, chez des participants adultes (âgés de ≥18 ans) atteints d'IH.

Environ 248 participants sont prévus pour la randomisation dans cette étude. L'étude comprendra une période de dépistage/de référence (jusqu'à 28 jours), une période de traitement en double aveugle (8 semaines), une période d'extension en ouvert (OLE) facultative (1 an) et un suivi de sécurité de 30 jours.

Critères d'inclusion :

  • Dispose d'un diagnostic documenté actuel d'hypersomnie idiopathique (IH) selon les critères de la Classification internationale des troubles du sommeil, troisième édition (ICSD-3) ou de la révision de texte (ICSD-3-TR) avec un polysomnogramme (PSG) confirmatoire avec test itératif de latence d'endormissement (MSLT) ; et le cas échéant, un rapport d'actigraphie avec journal de sommeil archivé qui a conduit au diagnostic et a été réalisé au cours des 10 dernières années.
  • Présente des symptômes d'IH modérés à très sévères.
  • S'il prend un médicament ou un complément concomitant chronique autorisé, y compris les antidépresseurs non interdits, doit être à une dose stable depuis au moins 3 mois avant le dépistage et accepter de continuer à cette dose stable pendant la période de traitement en double aveugle de l'étude. Tout traitement pouvant affecter la somnolence diurne (y compris, mais sans s'y limiter, les stimulants, le modafinil et l'armodafinil) utilisé à la demande n'est pas autorisé.
  • Pour les participants traités pour l'apnée obstructive du sommeil (AOS) ou d'autres conditions d'hypoventilation, doit être conforme à leur dispositif médical ou appareil oral tel que déterminé par l'investigateur. Les participants doivent maintenir la conformité au traitement de l'AOS tout au long de l'étude.

Critères d'exclusion :

  • Présente une hypersomnie due à un autre trouble médical.
  • A des antécédents d'utilisation de pitolisant dans les 5 demi-vies avant le dépistage.
  • A un diagnostic principal de maladie psychiatrique qui n'est pas bien contrôlée.
  • A des antécédents d'insuffisance hépatique modérée ou sévère.
  • A un débit de filtration glomérulaire estimé (DFGe) corrigé par la surface corporelle (SC) <60 mL/min.
  • A des antécédents connus de syndrome du QT long ou tout antécédent significatif d'anomalie grave de l'électrocardiogramme (ECG).
