1. fázis Dóziskeresés
Ez a vizsgálat nyílt elrendezésű dóziskereső része, amely értékeli a BNT162b2 biztonságosságát, tolerálhatóságát és immunogenitását 2 adagos (körülbelül 21 napos) adagolási rend szerint, legfeljebb 3 korcsoportban (a résztvevők 5 és 12 év közöttiek). év, ≥2 és <5 év, és ≥6 hónap és <2 év között).
A C4591001 vizsgálatban a 30 µg-os dózis 12 és 15 éves kor közötti elfogadható vak biztonságossági értékelése alapján ebben a vizsgálatban az 5-től 12 évesnél fiatalabb résztvevőknél kezdik meg a dózis megállapítását.
Az 1. fázis célja a BNT162b2 előnyben részesített dózisszintje(i) azonosítása legfeljebb 3 különböző dózisszintből minden korcsoportban.
A vizsgálat során keletkezett biztonságossági és/vagy immunogenitási adatoktól függően előfordulhat, hogy a dózisszinteket nem kezdik meg, korán leállítják, és/vagy hozzáadják a legalacsonyabb megadott dózis alatti dózisszintekkel.
Frissítés a 6. protokollmódosítás részeként: Minden résztvevő kap egy harmadik adag BNT162b2-t. A 6 hónapostól 5 évesnél fiatalabb résztvevőknél a harmadik adagot legalább 8 héttel a második adag után kell beadni. A ≥5 és <12 év közötti résztvevőknél a harmadik adagot legalább 6 hónappal a második adag után kell beadni. Fázis 2/3 Kiválasztott dózis

A kijelölt egyesült államokbeli helyszíneken további, opcionális, körülbelül 10 ml-es teljes vérmintát vesznek az 1. adag előtt és 7 nappal és 6 hónappal a 2. adag után legfeljebb körülbelül 60, 10 évesnél idősebb résztvevőtől. További mintákat vesznek a 3. adag előtt és 1 hónappal a 3. adag után (csak az eredeti BNT162b2 csoport). A 6 hónapos utóellenőrző látogatáson minden résztvevő megvakul. Azok a résztvevők, akik eredetileg placebót kaptak, lehetőséget kapnak arra, hogy a vizsgálat részeként BNT162b2-t kapjanak. 2./3. fázis, alacsonyabb dózisú értékelés
A vizsgálat nyílt elrendezésű része, amely értékeli a kiválasztott adagolási rend biztonságosságát, tolerálhatóságát és immunogenitását minden korcsoportban az 1. fázis alacsonyabb dózisú értékeléséből, összesen körülbelül 600 aktív résztvevővel.
Ebben a fázisban minden korcsoportban körülbelül 300 aktív résztvevő járul hozzá az immunhidrid elemzéshez a 2. adag után 1 hónappal, valamint az immunválasz fennmaradásának átfogó elemzéséhez a 2. adag utáni 6. és 12. hónapban.
2./3. fázis Szérumminták beszerzése a potenciális troponin I teszteléséhez

A BNT162b2 második és harmadik adagjának dózisszintje az oltáskori életkoron alapul: a második/harmadik dózis beadásakor 5-12 évesnél idősebb résztvevők 10 µg-os dózist kapnak, és a második/harmadik adag beadásakor 12 évesnél idősebb résztvevők 30 µg-os dózisszintet kapnak.

Befogadási kritériumok
- Férfi vagy női résztvevők ≥6 hónapos és 12 év közötti életkorúak a randomizálás időpontjában, az 1. vizitnél a dózismeghatározás/kiválasztott dózis értékeléséhez, valamint a 12 és <18 év közötti résztvevők esetében a randomizálás időpontjában , az 1. látogatásnál az alacsonyabb dózis értékeléséhez. A vizsgálat potenciális troponin I vizsgálatához szükséges szérumminták gyűjtése: 5 és 16 év közötti ≥ férfi vagy női résztvevők.
- A résztvevők szülője(i)/törvényes gyámja(i) és a résztvevők életkorának megfelelően, akik hajlandóak és képesek betartani az összes tervezett látogatást, kezelési tervet, laboratóriumi vizsgálatokat, életmódbeli megfontolásokat és egyéb vizsgálati eljárásokat.
Egészséges résztvevők, akik a kórtörténet, a fizikális vizsgálat és a vizsgáló klinikai megítélése alapján jogosultak a vizsgálatba való felvételre.
Megjegyzés: Azok az egészséges résztvevők, akik már fennálló stabil betegségben szenvednek, azaz olyan betegségnek minősülnek, amely nem igényel jelentős változtatást a terápiában vagy kórházi kezelést a betegség súlyosbodása miatt a felvételt megelőző 6 hétben.

- Fogamzóképes korú női résztvevő vagy gyermeket szülni képes férfi résztvevő, aki hajlandó a jelen protokollban leírt, rendkívül hatékony fogamzásgátlási módszert alkalmazni legalább 28 napig a vizsgálati beavatkozás utolsó adagját követően, ha fennáll a terhesség veszélye partnerével ; vagy nem fogamzóképes női résztvevő vagy gyermeknemzésre nem tudó férfi résztvevő.
- A résztvevő vagy a résztvevő szülője/törvényes gyámja képes aláírt, tájékozott beleegyezését adni, amely magában foglalja a BNO-ban és a jelen jegyzőkönyvben felsorolt követelmények és korlátozások betartását. A résztvevő életkorától függően és a helyi követelményeknek megfelelően a résztvevők hozzájárulását is kérik (szóban vagy írásban).
Kizárási kritériumok
- Csak az 1. fázis: múltbeli klinikai (csak a COVID-19 tünetein/jeleken alapul, ha nem volt elérhető SARS CoV 2 NAAT eredmény) vagy mikrobiológiai (COVID-19 tünetek/jelek és pozitív SARS-CoV-2 NAAT eredmény alapján) COVID 19 diagnózisa.
- Csak az 1. fázis: Ismert HIV-, HCV- vagy HBV-fertőzés.
- A COVID-19 megelőzésére szolgáló gyógyszerek átvétele.
- A MIS-C korábbi vagy jelenlegi diagnózisa.
- Egyéb egészségügyi vagy pszichiátriai állapot, beleértve a közelmúltban (az elmúlt egy évben) vagy aktív öngyilkossági gondolatokat/viselkedést vagy laboratóriumi eltéréseket, amelyek növelhetik a vizsgálatban való részvétel kockázatát, vagy a vizsgáló megítélése szerint alkalmatlanná tehetik a résztvevőt a vizsgálatra. - Terhes vagy szoptató nő.
- Korábbi oltás bármely koronavírus elleni vakcinával.
- Azok az egyének, akik immunszuppresszív terápiában részesülnek, beleértve a citotoxikus szereket vagy szisztémás kortikoszteroidokat, pl. rák vagy autoimmun betegség miatt, vagy a vizsgálat során tervezett kezelésben részesülnek. - Vér-/plazmakészítmények, immunglobulinok vagy monoklonális antitestek átvétele a vizsgálati beavatkozás beadása előtt 60 nappal, vagy a COVID-19-re specifikus passzív antitestterápia átvétele a vizsgálati beavatkozás beadása előtt 90 nappal, vagy a tervezett átvétel a vizsgálat során.
- Részvétel a vizsgálati beavatkozást magában foglaló egyéb vizsgálatokban a vizsgálatba való belépés előtt 28 napon belül és/vagy a vizsgálatban való részvétel során.
- Korábbi részvétel más, LNP-ket tartalmazó vizsgálati beavatkozást magában foglaló tanulmányokban.
- Azok a résztvevők, akik a vizsgálati helyszín személyzetének vagy a Pfizer/BioNTech alkalmazottainak közvetlen leszármazottai (gyermekei vagy unokái), akik közvetlenül részt vesznek a vizsgálat lefolytatásában, valamint a helyszíni személyzet, akit egyébként a vizsgáló felügyel, és családtagjaik.
