Descrizione
Criterio di inclusione:
- Pazienti diabetici che fanno uso di farmaci per i fattori di rischio CVD (terapie antiipertensive, terapie antiiperlipidemiche e terapie antiiperglicemiche orali),
- Pazienti diabetici considerati scarsamente aderenti ai farmaci per i fattori di rischio CVD nei 12 mesi precedenti
- Pazienti diabetici che non hanno utilizzato la farmacia per corrispondenza per riempire i farmaci prescritti almeno una volta nei 12 mesi precedenti
Criteri di esclusione:
- Diagnosi di gravidanza o demenza/lesione cerebrale traumatica nei 12 mesi precedenti
- Ricoverato in ospedale, in struttura infermieristica qualificata, hospice o assistenza domiciliare al momento della randomizzazione.
