Semaglutide per il trattamento dell'intolleranza al glucosio nelle donne con precedente diabete gestazionale: un RCT in doppio cieco

Il diabete gestazionale (GDM) è un importante contributo alla crescente prevalenza del diabete di tipo 2 (T2DM). Le donne con intolleranza al glucosio nel primo periodo postpartum sono un gruppo particolarmente ad alto rischio con circa il 50% che svilupperà il T2DM entro 5 anni dal parto. Inoltre, le donne con una storia di GDM progrediscono più rapidamente verso il T2DM rispetto alle donne con livelli di glucosio elevati in modo simile. Popolazione di pazienti: le donne con una storia recente di diabete gestazionale (GDM) e persistente intolleranza al glucosio nel primo periodo postpartum sono un gruppo a rischio particolarmente elevato, con circa il 50% che sviluppa il diabete di tipo 2 (T2DM) entro 5 anni dal parto. Semaglutide è un agonista del peptide-1 (GLP-1) simile al glucagone a lunga durata d'azione con molteplici effetti metabolici benefici, tra cui effetto ipoglicemizzante, perdita di peso ed effetti protettivi cardiovascolari. Ipotizziamo che nelle donne con precedente GDM e intolleranza al glucosio nel primo periodo postpartum, il trattamento con semaglutide ridurrà il rischio di sviluppare T2DM a lungo termine rispetto al placebo.

Intervento e confronto: RCT belga multicentrico in doppio cieco con 12 centri per confrontare semaglutide (una volta alla settimana) con placebo in donne con una storia recente di GDM e intolleranza al glucosio [ridotta glicemia a digiuno (IFG) e/o ridotta tolleranza al glucosio (IGT) ] 6-24 settimane dopo il parto. I partecipanti saranno randomizzati 1/1 a semaglutide o placebo su uno sfondo di misure dello stile di vita. Semaglutide sarà titolato a 1 mg/settimana per un periodo di 8 settimane. I partecipanti saranno seguiti per 3 anni. I partecipanti riceveranno un test di tolleranza al glucosio orale da 75 g (OGTT) 3 mesi dopo l'interruzione dell'intervento. La randomizzazione sarà stratificata in base al BMI alla prima visita postpartum (

Risultati: l'endpoint primario è lo sviluppo del T2DM definito da OGTT e/o HbA1c. Importanti endpoint secondari includono la necessità di una terapia di salvataggio per il diabete, la regressione alla normoglicemia, la perdita di peso, la funzione delle cellule beta, l'insulino-resistenza e la sindrome metabolica. Per raggiungere l'80% di potenza, pianifichiamo un campione di 206 persone per rilevare una riduzione stimata del 50% del rischio di sviluppare T2DM tra i due gruppi, ipotizzando una perdita del 30% al follow-up durante lo studio.

Criterio di inclusione:

  1. È stato ottenuto il consenso informato scritto volontario del partecipante prima di qualsiasi procedura di screening
  2. Uso di metodi altamente efficaci di controllo delle nascite
  3. Storia di GDM (diagnosticato con i criteri dell'OMS del 2013 24-32 settimane di gravidanza) e intolleranza al glucosio 6-24 settimane dopo il parto (basata sui criteri ADA)
